Conduct assessments, and provides clinical treatment interventions such as brief counseling, group therapy, and individual counseling. b. Consults and collaborates with other disciplines for the care and treatment of patients and participates in interdisciplinary care planning and coordination. c. Performs assessments that assist with and facilitates transitional care planning and coordination that links patients with community resources and services as they release from CDCR. d. Conducts assessments to determine appropriate level of intervention, education, and counseling services within the context of marriage, couples and family systems as well as individual biopsychosocial dimensions with specific solution-focused, attainable therapeutic goals. e. Conducts Cognitive Behavior Therapy for individuals with Substance Use Disorders (SUD), either in 1:1 and/or group settings to modify intrapersonal and interpersonal behaviors. f. Conducts assessments for patients preparing for release from CDCR. g. Provides education and counseling services to individuals that encourage the development of life skills and strategies for coping and confronting issues related to SUD. h. Develops and/or contributes to treatment and care plans. i. Reviews patient files, gathers data, and completes reports and other required documentation. j. Makes recommendations to the care teams and effectively communicates and collaborates with the patient and all disciplines involved in the care of the patients. k. Documents and evaluates patients' progress with programming and facilitates discharges and transfers in coordination with other treatment and rehabilitative programs. l. Works with institutional rehabilitative partners and custody staff to assess patient's participation and progress in services. m. Attends all case conferences, trainings, meetings and committees as directed, and performs other duties as required. LMFT Qualifications: A. All LMFT's assigned to CDCR/CCHCS performing services under this Agreement must have the minimum qualifications outlined below: 1. Minimum of twelve (12) continuous months of experience within the last three (3) years performing services similar in scope to those defined herein, in a public or private institution. Internship does not count towards the required experience. 2. Must have the required licenses, permits, registrations, and/or certifications noted herein. 3. Education: Completion of a master's degree program from an accredited school for Licensed Marriage and Family Therapy, that meets the requirements stipulated in Business and Professional Code 4980.36. Licenses, Permits, and Certification Requirements: Page 1 of 5 CDCR/MRN APP Prior to providing services as outlined in this Agreement, Vendor shall: app REQUIREMENTS APPENDIX 1 1. Ensure that each LMFT shall possess and maintain throughout the term of this Agreement, a current and valid LMFT license issued by the California Board of Behavioral Science in order to perform services in the State of California. 2. Provider shall comply with all federal, State, and local requirements including the California Code of Regulations (CCR) Title 22, Division 5, Chapter 12, Article 1, Section 79543. 3. Provide copies of any required licenses, permits, registrations, and/or certifications for each LMFT to the institution CEO/CME or designee to be kept on file at the institution throughout the term of this Agreement. Vendor shall ensure that all required licenses, permits, registrations, certifications, and any other requirements outlined herein are current and in effect at all times during the term of this Agreement. 4. In the event the required licenses, permits, registrations, and/or certifications are to expire, Vendor shall provide current/renewed licenses, permits, registrations, and/or certifications to CDCR/CCHCS not less than thirty (30) calendar days prior to their expiration. If, during the course of this Agreement, any of the licenses and/or requirements are found to be inactive or not in compliance, CDCR/CCHCS may withhold payment for services rendered during the expiration period and/or immediately terminate this Agreement. 5. Assume responsibility for verifying through the appropriate licensing boards that no current or unresolved adverse actions have been taken by the State licensing authorities against any LMFT assigned to CDCR/CCHCS, and that all licenses are active and void of misconduct. CDCR/CCHCS may, at its discretion, verify the current status of certification for any LMFT assigned. T. Licensed Professional Clinical Counselor - (LPCC) The Licensed Professional Clinical Counselor (LPCC) shall provide services as permitted within the scope of practice for a LPCC in accordance with institution policies and procedures. Under the direction/supervision of the CEO/CME/Supervising Psychiatric Social Worker or designee. LPCC's duties/responsibilities shall include, but are not limited to the following: a. Periodically serve on-call during scheduled workdays for patient emergencies, provide consultation and crisis intervention services on an as-needed basis. b. Conduct initial mental health (MH) evaluations including criminal, psychological, and substance abuse case history to assess patient current needs and make treatment recommendations in the initial evaluation. c. Conduct clinically sound suicide risk evaluations, including clinical review, applying suicide risk assessment protocols, pertinent data/chart reviews, proper documentation, and consultations as needed. d. Meet with patients for crisis intervention, group psychotherapy, in-person interviews, etc. The clinician may be required to provide clinical, in-person interviews at cell fronts in inmate housing units. In some institutions/facilities, due to physical plant layout and multi-tiered housing units, the ability to climb stairs and walk long distances may be required. e. Identify the relevant signs and symptoms of psychiatric disorders in order to comprehensively assess, diagnose and manage the presenting symptoms, consults with medical personnel Page 2 of 5 CDCR/MRN APP app REQUIREMENTS APPENDIX 1 regarding the findings of medical examinations and evidence of organic disturbances related to behavior disorders, and with nursing, social work, education, custody and other staff regarding precipitating events, complications, and potential for adjustment. f. Consult on complex cases with supervisor, treatment team members, CDCR/CCHCS institutional health care personnel, and other divisions within the department. h. Document all care provided to the patient, including in-person contacts and Interdisciplinary Treatment Team (IDTT) meetings. i. j. Monitor patient progress using CDCR/CCHCS MH approved documentation formatting for progress notes. Effectively apply community standard of practice for non-medical treatment modalities and ethical standards. k. Perform mental status exam (MSE) in-person clinical interviews. l. Examine patient information from electronic medical record, inpatient record, custody file, institutional chronos, probation reports, court records, and other available documents to assist in the formulation of patient diagnosis and aid in the development of patient treatment plans. m. Conduct health education and develop additional resources for patients. n. Assist patients in applying for entitlement benefits community programs, and services. o. Facilitate patient transition between various settings within the institution/facility. p. Perform pre-release functions for releasing patients, to include patient education, while facilitating transition to county MH, county medical services and community MH treatment programs including county addiction services; coordinate discharge planning activities and act as resource on accessing appropriate community support and services to be utilized upon release. q. Perform medical social work functions as needed. r. Participate in and/or lead IDTT and other multidisciplinary meetings, develop, monitor, and modify individualized patient treatment plans in collaboration with the interdisciplinary treatment team, identify and recommend appropriate services and level of care based on assessment and, where applicable, civil or penal code commitment. s. Develop and distribute draft and final discharge plans. t. Provide information (e.g., appointment documentation, no-shows, etc.) to support staff for data entry. u. Mental Health Clinician I - Recovery (229) Hanford, CA Under clinical supervision, the Mental Health Clinician I, provides mental health direct services in an outpatient setting that includes individual therapy, intake assessments, and facilitating skills classes/groups for the Recovery Oriented level of care. Direct services are provided for adult consumers of the Kings County area, including some travel to Avenal, Corcoran, or Kettleman City. How will I contribute Provide direct services to individuals assigned to caseload. Job Description The Substance Use Counselor (SUC) is responsible for providing adult client supervision in a shelter or recuperative care facility, and assisting in the maintenance of a safe, secure environment that enhances behavioral health and substance use counseling while utilizing the harm reduction and recovery model to all clients being served. The pay offered for this role is $24.00 per hour. The schedule for this role is Monday - Friday, 12:00pm to 8:30pm. Responsibilities Within a harm reduction model complete substance use assessment, care plan, and goal setting to clients interested in services Works with a multidisciplinary team, lead site activities such as; current events groups, community meetings, goals groups, fitness groups, activities of daily living groups, and/or recreational activities, as appropriate to the specified client population promoting safety and comfort in a supportive, therapeutic environment Provide individual and group counseling to clients struggling with substance use Recognize the need for and provide crisis intervention services to clients as needed according to PRO-ACT principles Documents all client interactions or activities complete on behalf of the client (including collaboration with other disciplines, case consultations, telephone calls, leaving voicemails, and collateral contact) in AICA & KIPU according to the documentation standards of the agency and within 96 hours of service provision Meet documentation standards as assigned by Clinical Supervisor, Manager or Director Informs Supervising Manager or Director, within 12 hours of making an incident report, APS/CPS and/or 5150 report within a mandated time frame Collaborate (on a need-to-know basis) with teams including site nursing, case management, behavioral health therapists, site-staff, food service and transportation regarding client's substance use counseling needs, ensuring the highest level of client care Maintain confidentiality of work-related information and materials according to HIPAA standards Assists client with activities of daily living; attend to client behavioral issues and provide assistance in crisis intervention, as needed Tracks and maintain client observation, sets boundaries on inappropriate behavior, and recognizes the need for and provide crisis intervention services to clients Act as a liaison with clients, families of clients, or external facilities and agencies Follow established institutional policies and procedures, objectives, quality assurance program, safety environmental, and infection control standards Research and build relationships with community agencies for client resources and linkage Familiarization with supportive services in county/counties served, including but not limited to linking clients to outside therapy, mental health, substance use treatment, job training, schooling, church, AA/NA meetings, etc. Participates in the development and implementation of client treatment programs as directed Attends and participates in staff meetings, in service trainings, workshops and other required meetings as directed Drive to any IF location for coverage, trainings, or support, as directed Support volunteers as needed; may be required to supervise volunteer(s) Circulate throughout the facility every hour and check-in with security guards and medical staff concerning any client related issues. Be available to assist staff immediately with the de-escalation process when asked to do so by security guards, medical staff, and site-staff Other responsibilities, as assigned by Supervisor, Associate Manager, Manager or Director Preferred Experience/Minimum Qualifications: Required: High School graduate or GED equivalent Must possess a valid Drug & Alcohol Certification from one of the following agencies: CAADE, CCAPP & CADTP Addiction counseling: 1 year Ability to work flexible hours when needed (2nd and 3rd shifts) Basic computer skills including the ability to prepare simple correspondence and reports in Microsoft Word and summarize and report data in Excel spreadsheets. Preferred: Bilingual in English and Spanish Experience and knowledge with electronic filing system (KIPU & AICA) Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams). Two years of experience working with homeless and dual diagnosis (mental illness and comorbid substance abuse problem) populations. Able to successfully input required progress notes and court summary information into computerized database in a timely manner. Transports clients when necessary. Demonstrates interest and growth in specialty area by attending relevant workshops, seminars and classes; shares this information with program staff Comply with all CAP and program rules, policies and procedures including HIPAA standards. Minimum Qualifications (Knowledge, Skills, and Abilities) Certification through a recognized alcohol and drug certification entity, required.
