Examples of Typical Tasks:
Provides mentalhealth counseling and psycho-education services to clients with less complex cases.
Participates in the interviewing and case history taking process by utilizing appropriate assessment and screening tools.
Consults with relevant mentalhealth professionals regarding the diagnoses, prognoses, and recommendations for disposition on individual cases; contributes to the formulation of client comprehensive treatment plans.
Coordinates with appropriate personnel and/or departments to counsel and assist clients with the completion of applications and documentation for educational, vocational, and entitlement programs.
Acts as a resource to clients seeking additional support on a broad variety of available services within the healthcare facility and the community.
Prepares and maintains accurate and complete records and files of patient contact information which includes consumer interactions, clinical case histories, phone contacts, agency interactions, and other related information.
Participates in appropriate internal and/or external training conferences.
May participate in treatment programs assisting other personnel and/or departments in providing counseling/therapeutic services.
May be assigned to coordinate and supervise the activities of support staff in related areas.
May be assigned to assist in the instruction and supervision of volunteers, interns, and students in the area of mentalhealth counseling as appropriate.
Assignment Level I

POSITION SUMMARY
The Residential Youth Counselor is responsible for the day-to-day operations of the housing program and the direct care of residents during 8-hour shifts. Youth Counselors create and supervise a physically and psychologically safe and affirming environment for residents, helping them meet their needs and progress on their goals. Youth Counselors provide individual support and group programming, and oversee the functioning of the site. Youth Counselors must be organized, able to work and solve problems independently, and communicate effectively with supervisors.

Conducts qualitative field visits to ensure consumers needs are concerns are being met in their living environment

The Role
Under general direction, the MentalHealth Specialist is responsible for providing counseling, group therapy, family therapy, and casework services to individuals and families. Assesses patient's mental status through interviews and observations and then develops treatment plans provides therapy, or makes referrals to appropriate services. Qualifications
Who You Will Be
NYSED LMSW or LMHC license;
OR
other QHP (Qualified Health Professional) status such as LMSW-Limited Permit and must be obtained within 90 days from the date of hire and licensed within One year from the date of hire;

Job Description
Community of Veterans (CoV) is the first and largest online social network exclusively for paperwork-confirmed Iraq and Afghanistan veterans. CoV provides veterans with a safe space to share what they're going through with others who “get it” because they've been there too. Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.

SCOPE OF ROLE:
Traditional ACT teams are multi-disciplinary teams consisting of 68 clients, who are seen twice per week by varying members of the team. Flexible (“Flex”) ACT Teams consist of 100 clients, 40 of whom are considered “ACT Lite”, and receive two to five visits per month in comparison to six to eight for traditional clients. The ACT Lite team consists of a small team of two specialists, LPN, peer staff and Psych Provider. The Flex Lite component focuses on graduation and transitioning clients to outpatient treatment and supporting them around independence and community integration.
The Senior MentalHealth Specialist is an experienced professional responsible for providing expertise and guidance in the field of mentalhealth treatment and harm reduction strategies. They play a crucial role in creating, implementing, and evaluating transition plans for clients who are ready to step-down to a lower level of care or graduate from the team altogether, as well as providing support and guidance to other professionals in the field. They are the lead clinician for the clients on Flex ACT identified on the ACT Lite caseload, shared amongst a small team of 5 staff.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Clinical Skills
Support and educate staff around best practices for transition and recovery, including treatment modalities and harm reduction strategies.
Develop and deliver training programs to educate staff, community workers, and stakeholders on best practices for transition and recovery, including the latest research, treatment modalities, and harm reduction strategies.
Have a strong understanding of substance use disorder assessment, diagnosis, and treatment planning
Conducts transition needs assessments amongst individuals served
Extensive knowledge of evidence-based practices, treatment modalities, and harm reduction strategies
Data and Accountability
Develops, conducts, and monitors treatment interventions for specific target populations and ensure these interventions are incorporated into treatment plans
Performs consumer centered, comprehensive assessments to determine needed services for all consumers; develops both initial and ongoing treatment plans; assigns the most appropriate staff to the individual treatment team and actively participates in all activities in revising treatment plans as the participant's needs change
Conducts qualitative field visits to ensure consumers needs are concerns are being met in their living environment
Complete Progress Notes, which should be entered into system within 24-48hrs of event date
Support around facilitating groups, as needed.
Qualifications
REQUIRED EDUCATION AND EXPERIENCE
· Must be a LICENSED clinician and meet criteria for professional staff /
licensed practitioner of healing arts
based on ACT Guidelines (ie: LMHC, LMSW, LMFT, licensed Psychologist) · Master's degree in social work, psychology, rehabilitation counseling, or a related field· Must have excellent verbal, written and interpersonal communication skills· Must have 2 years or more experience working with dually diagnosed individuals
