The Adult MentalHealth Outpatient Case Manager must be able to complete the following essential job functions:
Complete DLAs (or other agency identified case management needs assessments) at the required frequency in order to create individualized goals and objectives.
Use information from the DLA to complete individualized service plans in support of overall mentalhealth treatment goals.
Maintain agency standards for productivity and documentation.
Engage with clients' treatment teams to support clients and coordinate treatment objectives.
Maintain an active caseload.
Coordinate and advocate for care that ensures basic needs are met. This may require coordination and advocacy with multiple local and State agencies.
Work with clients and their families or other support systems to ensure basic needs are met.
Be knowledgeable about the crisis system and know when to activate it.

Employment Classification & Schedule
This position is classified as full-time, non-exempt.

Location
The DBH Main Street Clinic is located at 934 South Main Street, Layton, UT 84041.
While this position will have office space within the agency, case management activities are expected to be performed across multiple settings including client homes and community locations. Significant fieldwork is required, including frequent travel throughout the community to conduct outreach, attend meetings, and provide direct client services. The position may require the use of a personal vehicle; mileage reimbursement is provided in accordance with organizational policies.
Compensation
The hourly rate for this position is outlined below:
BSW Degree + SSW License
$26.00 per hour base wage + 1% per year of licensure
Degree without SSW License
$22.00 per hour base wage + 1% per year of direct social work experience, up to 10 years.
Introductory Period
All new employees at DBH have a 6-month introductory period to assess whether they can perform the essential functions of the job at the expected level, within the specified timeframes, and are a good fit for the job.
Required Qualifications
Education
Bachelor's degree in social work or a related field is required.
Licensure/Certification
Strong preference for active licensure as a Social Service Worker (SSW) in good standing in the State of Utah. BHCM certification is not required for licensed individuals, OR

Position Summary
This position is based out of the Geraldine E King Women's Resource Center (GEK) which is a 200 bed women's emergency shelter. Under the direct supervision of the GEK Program Director, provide therapeutic services to women out of the GEK via crisis intervention, Brief therapy, and on-floor support. This position assists with providing daily program oversight including management of clinical and/or non-clinical staff; clinical oversight for case management, peer support specialists, and program services, as needed; and provide a safe and nurturing environment for clients in accordance with licensing, agency and contract standards. This position will also assist with clinical program development and community networking as needed. In alignment with our mission, VOA, Utah encourages individuals with lived experience to apply.
Essential Duties
Provide clinical services to clients experiencing homelessness and mental illness in a women's shelter, including making referrals to higher level of care, creating safety plans, referrals to ongoing mentalhealth services, facilitating group therapy, overseeing the facility's group therapy schedule, providing brief individual counseling, and crisis intervention.
Holds weekly Clinical Supervision meeting with housing case managers and peer support specialists to go over client cases and provide appropriate clinical direction and guidance.
Oversee the training and implementation of evidenced based best practices such as motivational interviewing, stages of change, trauma informed practices and procedures, etc.
Consistently monitor client flow between case managers and peer support specialists to assure client connection to ongoing supportive services, community connection, and housing focus.
Collaborate with Management team to identify high shelter utilizers and clients with high/acute behavioral health needs to create strategies to assist clients through service engagement.
Provide regularly scheduled supervision, performance evaluations, and disciplinary processes with direct reports.
Oversee high acuity cases, assisting with referrals, clinical support, and interventions in collaboration with case management and management team.
Review, on an ongoing basis, the clinical services offered at the Women's Resource Center and develop new programs and/or services as need emerges and assist in the integration of behavioral health.
Participate in audits, site visits, and provide oversight that all client files and related paperwork is maintained in accordance with licensing and contract requirements and submitted in a timely manner.

This position provides professional counseling services of a preventative and developmental nature through direct consultation with patients. Therapy activities include individual, group and couples counseling. The incumbent develops program criteria for area of specialization such as couples or addictive behavior counseling. MH1 team members will be cross-trained for other service lines, such as SafeUT and Mobile Crisis Outreach Teams (MCOT).
A partnership with Huntsman MentalHealth Institute (HMHI), the University Counseling Center (UCC), and University Housing and Residential Education (HRE) has been established to provide free crisis response and stabilization treatment and referral programs that aim to keep the University of Utah student community safe. These crisis service programs are designed to provide students with a full range of options to help solve their crisis in the best setting possible. Follow-up services can be provided or coordinated to included additional therapy sessions within the MH1 program, referrals to the University Counseling Center (UCC) or community-based mentalhealth services, or higher levels of care.
Our team of professionals are highly trained in mentalhealth crisis management and suicide prevention.
The Licensed Clinical Social Worker (LCSW) or Clinical MentalHealthCounselor (CMHC) in this position is responsible for responding to crisis cases by providing diagnostic assessment, crisis intervention therapy, mobilization of internal and external resources, establishing appropriate stabilization and disposition of each case and referral for follow-up care. This position has the authority to assess and diagnose psychiatric emergency patients; deem if and when to hospitalize or civilly commit patients; and determine what resources a client and family requires in order to resolve or diminish a crisis.
Responsibilities
Works in close collaboration with all members of the treatment team including medical doctors, to further their understanding of significant social and emotional factors contributing to the patient's well being.
Identifies appropriate outside resources, such as community agencies, to assist patients with resuming life in their own community and how to live with the patient's limitations.
Develops and implements individualized treatment approach to meet the needs of each patient and their families.
May perform therapy activities for individual, group, and couples counseling.
Assists in the development of departmental policies and procedures.
Provides practicing oversight of Certified Social Workers, Social Service Workers, and interns in accordance with Utah state law.
May perform Case Management duties.

We're seeking a full-time, state-licensed mentalhealth therapist (LMFT, LCSW, CMHC). At Telos, a residential treatment center, you will help adolescents and young adults facing challenges such as anxiety, depression, ADHD, addictions, processing disorders, and executive function issues.
Duties and Responsibilities:
Maintains a caseload of 4-7 students conducting individual, family and group therapy
Cultivate a working relationship with referral sources
Managing cases in a weekly treatment team
Refers and manages cases that are assigned to the Telos step-down program (Anthem House)
Manage all crisis with each assigned client including potential AWOLs, self harm, suicidal threats or any other ‘danger to self' behaviors.
Completes special assignments as directed by their supervisor

Our therapy is tailored to individual needs and includes various evidence-based methods suitable for both children and adults, such as play therapy, sandtray therapy, art therapy, EMDR, TFCBT, and more.
Our clinical team includes LCSW, RPT-S and EMDR certified therapists who provide excellent supervision to new therapists working toward licensure and/or RPT credentialing. We offer many accessibility programs to the community so all our billing and scheduling is done in-house. You would be working from a private office in a very home-like, friendly environment.
