Essential Responsibilities
Demonstrates a high level of clinical skill in assessing the clinical needs of clients, determining appropriate interventions, and conducting individual and group therapy sessions in accordance with these needs.

What We're Seeking
Education and Experience
Position requires a PhD, Psy.D, Ed.D in social work, marriage and family therapy or psychology. Position requires a minimum of one (2) years' experience in a behavioral healthcare setting, preferably with clients with mentalhealth and substance use disorders, strongly preferred. Must have a valid Florida Department of Health license as LCSW, LMHC, LMFT, or Psychologist.
Other Requirements
Position requires incumbent to have a valid driver's license, vehicle insurance and acceptable driving record.
Clearance of TB test and any other mandatory state/federal requirements.
Current CPR and First Aid certification.

JOB DUTIES/KNOWLEDGE
Takes a leadership role at Hospice for all issues relating to children's bereavement.
Promotes, organizes, and conducts the Children's Bereavement Camp in each of our service areas, including volunteer coordination, education and training; camp resources; camp schedule; camp bereavement counseling and education; and child applicants for camp.
Works as a community liaison by promoting our community children's bereavement program in schools and other relevant settings.
Consults with other BV counselors on challenging cases and when necessary arranges visit with high risk cases.
Develops, prepares and maintains individualized client care progress notes with accuracy, timeliness and in accordance with policies.
Completes accurate documentation within the EMR system at the time of care and location of service. Where not possible or otherwise directed, completes at next available opportunity, and by end of shift without exception.
Provides ongoing education related to children's bereavement for our staff and the community when appropriate.
Keeps abreast of children's bereavement and counseling trends and knowledge; participates in ongoing education to support personal growth and development.
Works with local schools and guidance counselors to provide on-going bereavement support to children experiencing traumatic loss.

Qualifications
Master's degree from a school of social work accredited by the Council on Social Work Education or a master's degree in a related field required; LCSW or LMHC in the Applicable State preferred.
Minimum five years' experience working in the field of social work or mentalhealth.
Minimum two years of documented experience working with children, preferably in a home care agency, hospital, outpatient clinic or mentalhealth program.

How You Will Make a Difference
Therapists are responsible for providing individual and group therapy sessions for clients in a specialized intensive outpatient treatment setting. Position is responsible for assessing the therapeutic needs of the individuals and families, and for developing appropriate treatment plans. Program Therapists act as the primary contact for clients and families throughout the treatment process and serve as a client advocate with the treatment team. Position Responsibilities
Assists in identification of treatment/contract goals, completing the initial psychosocial assessment.
Responsible for maintaining contact with the assigned client's referring/outpatient therapist during the course of the client's stay and the family when/if appropriate.
Completes and updates treatment plan for assigned clients.
Assist in providing clinical updates to the insurance company's Case Manager.
Meets with clients two to three times weekly, to help explore identified issues and behaviors in relationship to current symptoms.
Leads and participates in group meetings, meals and other activities in the milieu.
Provides comprehensive understanding of current behaviors and relational dynamics while using a variety of therapeutic modalities.
Attends the weekly team meetings and share observations, comments and important information about clients with the treatment team and provides input into the development of the weekly schedules for all of his/her assigned clients.
Responsible for proper charting and ensuring that all documentation is completed in an accurate and timely manner.
Assists in meal/snack supervision as assigned and attends special occasion activities for the clients.
Assists with discharge planning, including a written discharge summary and obtaining referral sources and necessary support systems once the client is discharged.
Preforms other activities and appropriate responsibilities as requested by the Clinical Director or Executive Director.

What We're Seeking
Position requires a master's degree in social work, marriage and family therapy or psychology. Position requires a minimum of one (2) years' experience in a behavioral healthcare setting, preferably with clients with mentalhealth and substance use disorders, strongly preferred.

Cornerstone Hospice provides social work services to bereavement clients as needed. We assist, educate, and provides support to families with financial, social and emotional concerns. This position services the Mike Conley Hospice House in Clermont, FL.
SHIFT - 8AM- 4:30 PM
JOB DUTIES/KNOWLEDGE
Provides direct service to patients and families as needed in a variety of settings.
Works closely with Bereavement Services Manager in formulating policies and procedures for coordination and delivery of bereavement services.
Collaborates with Bereavement Services Manager in development of a process to evaluate patient/family outcomes in the bereavement services area.
Coordinates all bereavement components.
Identifies patient/family issues related to family bereavement care.
Conducts individual and group support for families. Implements and documents bereavement services.
Provides and documents a comprehensive support program for the bereaved according to policy which reflects respect for personal history and diversity of client.
Continually assesses bereavement needs of patient/family.
Ensures preparation and implementation of a written bereavement care plan for each Hospice deceased patient/family unit on Hospice premises.
Ensures a mechanism exists to address the need for therapeutic intervention, review and referral to manager.
Provides community programs to schools, churches, hospitals, clinics, industry, and business services regarding bereavement and grief.
Assists in memorial services for Hospice bereaved.
Assists bereavement support volunteers in all components of their client & bereavement activities.
Maintains an established method of providing information in a consistent manner to staff on bereavement activities.
Works with qualified staff social workers in the development and implementation of caregiver support groups.
Develops and implements support groups open to the public.
Provides individual bereavement counseling to local residents not affiliated with Hospice.
QUALIFICATIONS
Master's degree from a school of social work accredited by the Council on Social Work Education or a master's degree in a related field required.
Minimum one year of experience in social work, or a related field, preferably in a homecare agency, hospital, outpatient clinic, or mentalhealth program in which bereavement counseling and support group facilitation was primary function.
Must possess a Florida Licensed Clinical Social Worker (LCSW) credential in good standing or similar credential (LMHC, LMFT) required.
Knowledge and Abilities:
Demonstrated knowledge skills and commitment to the Hospice philosophy of care and the Hospice team concept.
Able to assess data reflecting the patients' status and interpret the appropriate information needed identify each patient's requirements relative to their needs.
Valid Florida driver's license and required auto liability insurance.
