JOB DESCRIPTION
As a Behavioral Health Specialist, one of the key responsibilities is to work alongside the social work team to observe and assess patients' behavior in order to develop and execute treatment plans that effectively manage any behavioral issues they may be experiencing. It's important to provide ongoing support throughout the entire treatment process, including communicating with patients' teachers, caregivers, and families to implement treatment plans and address any concerns that may arise. By working together to support patients in managing their health and wellness, we can help them feel more empowered and confident in their ability to overcome any challenges they may face.
DUTIES
Managing participant's behavioral health, by conducting thorough assessments to determine their needs, preferences, and goals. This information can then be used in interdisciplinary team meetings to develop discipline-specific goals and interventions that are tailored to each individual. These meetings are actively participated in to ensure that all aspects of the participants' care are being considered and addressed.
Deliver and document behavioral health interventions as agreed upon in the participants' care plans, including but not limited to symptom management, counseling, crisis intervention, arranging necessary resources and services, assisting with the care transitions, providing individual and group counseling, and care management.
Facilitate individual and group therapies in various forms as appropriate such as art and music therapy, psychosocial drama, counseling, and discussion that restores or expands participants' social roles.
Provide training on behavioral health symptoms and interventions to staff.

EDUCATION AND QUALIFICATION REQUIREMENTS
Master's degree in behavioral health (MSW) or Doctorate in Psychology (PhD)
License by the CA Board of behavioral sciences as an LCSW or Licensed by the CA Board of Psychologists as a Psychologist
Minimum of Three (3) years of behavioral health experience conducting therapy, completing behavioral health assessments, and managing care planning needs.
Minimum of two (2) years of experience with a frail or elderly population.

GENERAL PURPOSE

Under clinical supervision, the Mental Health Clinician I, provides mental health direct services in an outpatient setting that includes individual therapy, intake assessments, and facilitating skills classes/groups for the Recovery Oriented level of care. Direct services are provided for adult consumers of the Kings County area, including some travel to Avenal, Corcoran, or Kettleman City.
How will I contribute
Provide direct services to individuals assigned to caseload.
Evaluate consumers for serious psychiatric problems and provide appropriate treatment plans and carry out treatment using a wide variety of treatment modalities such as individual, group, conjoint and family therapy.
Maintain a monthly productivity standard of 100 hours of billable time per month.
Participate in Access Rotation schedule as directed.

PHYSICAL DEMANDS
Must possess mobility to work in a standard office setting and use standard office equipment, including a computer, and to operate a motor vehicle to visit various County and meeting sites; vision to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone. ENVIRONMENTAL CONDITIONS

The Position
The Mental Health Specialist (MHS) is responsible for providing direct clinical intervention to participants (families and individual adults) in Interim Housing Site(s) or as part of a street-based Multidisciplinary Team (MDT). The Position
The Mental Health Specialist (MHS) is responsible for providing direct clinical intervention to participants (families and individual adults) in Interim Housing Site(s) or as part of a street-based Multidisciplinary Team (MDT). What You'll Do
Provides in-person, individual and group based clinical case management and behavioral health services to people experiencing homelessness in interim housing settings or within a homeless outreach team.
Conducts mental health, substance use and safety/risk assessments on identified participants
Creates relationships with mental health service providers, Department of Mental Health, VA and hospital social work teams. Refer participants for services as well as maintain a resource listing of mental health services for participants engaged in LA Family Housing programs.
Engages in discharge planning and leads care coordination activities
Utilizes Harm Reduction, Housing First, Low Barrier and Trauma Informed Care philosophies when working with individuals experiencing homelessness
Works collaboratively with medical providers to ensure PEH are triaged to needed medical care
Completes risk assessment and safety plan and liaise with Psychiatric Mobile Response Team (PMRT) if danger to self/others resulting from a mental health disorder or grave disability is suspected.

•Communicate with network providers or insurance providers as the participant moves between different levels of care to support a successful transition.
•Work collaboratively with substance abuse counselors and assigned therapists to engage, educate, communicate, and coordinate care with client, their families, referring parties, community agencies, and all others to ensure that all services agreed upon in the individual treatment plan are implemented.
•Create and develop relationships with housing, employment, education, food assistance, childcare support, rehabilitation placement, primary care, mental health treatment, and other supportive services that will benefit the client upon re-entry into the community.
•Attend group meetings to provide information and gather needs for program development and outside agency services.
•May attend court hearings as needed at the director's discretion.
•Stay current with counseling practices, modalities, and certifications by attending needed training, and workshops and submitting a request for certification or re-certification as needed. •Assist clients with transportation, resources, referrals, appointment scheduling, filling out documents, and other tasks as needed.
•Drive company vehicles or personal vehicles to conduct business on behalf of TSSD.
•Complete appropriate documentation requesting per-diem for travel when necessary.
•Provide Treatment Program Placement and transportation for Youth and Adults as needed.
•Attend weekly consultation with direct supervisor.
•Participate in staff meetings, training, conferences, and other meetings as assigned.
•Perform other duties as assigned.
