POSITION SUMMARY
The MentalHealth Coordinator is responsible for the development, implementation, clinical and/or administrative supervision, and monitoring of a Behavioral Health services delivery system in MA DOC Secure Treatment Units meeting NCCHC/ACA standards. The MentalHealth Coordinator works with the Regional Manager, Site Administrator, Medical Director, and Psychiatrist to coordinate total contract services in MA DOC Secure Treatment Units

MINIMUM REQUIREMENTS
Must be fluent in English language, including reading, writing, and must be able to communicate and exchange accurate information. Must be a supportive team member, who contributes to and demonstrates teamwork and team concept. Able to make independent decisions when circumstances warrant such action.
REQUIRED CERTIFICATIONS, EDUCATION, AND EXPERIENCES
Master's degree in Psychology, Social Work, or related field from an accredited college or university.
Licensed to practice behavioral health services in the state where services are being delivered (LICSW or LMHC required).
ESSENTIAL FUNCTION
Acts as point of contact for administrative staff for discussion of treatment unit programs and problem resolution as needed.
Coordinates development and revision of policies and procedures for the identification, assessment, and provision of required clinical services for inmates housed in Secure Treatment Units.
Supervises the planning, development and implementation of treatment programs contracted to VitalCore.
Provides clinical and/or administrative supervision to staff of Behavioral Health Services within Secure Treatment Units.
Monitors efforts of Behavioral Health Services in Secure Treatment Units to ensure clinical services are being provided in a timely, professional manner.
Provides and assigns clinical on-call duties.
Provides direct clinical services including individual and group treatment, crisis intervention, as well as providing shadowing and mentor opportunities to front-line clinical staff.
Ensures treatment programs contracted are integrated and appropriately staffed.
Provides administrative support to behavioral health staff.
Coordinates with the facility's Behavioral Health Quality Improvement Program.
Attends training and meetings as required.
Maintain proficiency with all software required for the position, and more specifically, clinical documentation in the designated electronic health record system.
Other duties as assigned.

Your Job as a Substance Use Counselor:
The Substance Use Counselor is instrumental in our patients' treatment and recovery from opioid use disorder. Substance Use Counselors provide high-quality, compassionate care through various mediums, which include both individual and group counseling sessions.
Job Responsibilities:
Provide high quality, compassionate guidance in both individual and group counseling sessions.
Plan, oversee, facilitate and document patient's recovery.
Co-facilitate assigned group or family sessions as needed.
Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner.
Prepare individual treatment plans for each assigned patient.
Initial assessments as well as follow up assessments.

Required Education, Skills, and Qualifications:
Associate's degree from an accredited college or university required.

Previous experience in addiction recovery or behavioral health settings, such as outpatient, residential, or correctional facilities.
* Familiarity with Medication-Assisted Treatment (MAT), including methadone, buprenorphine, and naltrexone.
* Experience conducting individual and group counseling sessions focused on substance use recovery.
* Knowledge of evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and relapse prevention techniques.
* Documentation experience, including maintaining clinical notes, treatment plans, and progress updates in accordance with state and federal guidelines.
* Familiarity with state regulatory standards (e.g., 42 CFR Part 2, HIPAA, Joint Commission standards).
* Experience collaborating in a multidisciplinary team with medical staff, case managers, and peer support specialists.

This may include assisting patients with bathing, eating, and other daily living activities. Sets limits, confronts counterproductive activities and behavior, and reassures and orients patients. Encourages and facilitates appropriate interactions between patients.
Records all appropriate information on patient charts; prepares regular reports describing each patient's mental status and functioning, as well as significant interactions. Reports and documents overt, covert, verbal, and non-verbal communications by patients.
Provides support and education to family members and significant others regarding the patient's status. Works with representatives of outside agencies in obtaining or disseminating information on individual patients. Participates in discharge planning and referral, if necessary.

Participates as a member of the mentalhealth team on an inpatient and/or outpatient mentalhealth unit. Provides direct patient care including treatment planning, implementing plans, documentation, and milieu safety assessments and activities such as group activities.
I. Major Responsibilities:
1. Serves as a member of the MentalHealth Team and serves both internal and external customers including patients, families, nurses, physicians and other colleagues who support the delivery of care and services.
2. Provides clear, timely communication to the Registered Nurse (R.N.) regarding observations and changes seen in the patient. Records behavioral data on appropriate data sheets.
3. Maintains patient safety by performing patient safety checks, environmental safety rounds, utilizing fall prevention alternative devices/strategies as directed utilizing de-escalation and calming tools and strategies with anxious patients and following the Restraint Policy standards when caring for a restrained patient.
4. Participates and shares responsibility for the development and maintenance of a safe and therapeutic environment.
5. Serves as a leader or co-leader for group activities such as goal setting group, dual diagnosis groups, various activity groups, and structuring and supervising recreational activities. Handles routine situations and must request assistance from identified resources when more direction is needed.
6. Assists in medical emergencies as directed by R.N. Utilizes proper safety techniques and body mechanic in all work-related activities.
7. Obtains and records vital signs (temperature, pulse, and respiration, blood pressure and pulse oximetry). Measures and records patients' height and weight as assigned by R.N.
8. Performs a variety of test and procedures which may include but not limited to EKGs, phlebotomy, and intake and output as assigned by R.N.
9. Documents daily progress notes, and group notes (per policy) and as assigned by R.N.
10. Assists the R.N. in other medical and psychiatric interventions as directed.
11. Provides personal care as appropriate, working to optimize patient's independent functioning in performance of daily routines.
12. Works to utilize interventions to de-escalate patients and if unsuccessful participates in restraints in accordance with training and education as directed.
13. Performs safety checks on patients and unit as directed. Documents findings.
14. Reviews and follows treatment plan with patient. Provides feedback to clinical team regarding implementation of the plan. Implements special treatment strategies as indicated in the plan.
15. Maintains logs, organizes patient care charts for admission/discharge/transfer and maintains medical records as instructed.
16. Attends staff meetings and in-service programs as required or directed. Keeps current with hospital and unit changes by reading communication boards and/or books, emails, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.
17. Participates in unit-based quality improvement initiatives.
18. Compiles with required health, safety and HIPAA requirements.

1. Associate degree or at least 2 years' experience in behavioral health required
2. American Heart Association Basic Life Support or Heart Savor certification required within one month as a condition of employment

Job Summary:
Position includes admission intakes, individual, and group therapies. Counselor will participate in planning, implementing and evaluating the Patient Therapy program. The MentalHealthCounselor will also work in cooperation with other health care disciplines to provide psychiatric service to clients.

Systematically gathers and evaluates relevant information and data from the patient interview and/or from patient information sources.
Assures that the patient exams, tests and/or procedures are consistent with the nature of patient's needs and orders.

Plans for and provides patient teachings.

Assesses and reassesses the patient's condition.
Coordinates and communicates with other healthcare professionals to provide comprehensive patient service and care.
When appropriate, delegates to other healthcare professionals.
Works together to complete assignments with departmental and healthcare team members.
Supports comprehensive case management to assure patient success and meet standards of care and standards of practice.
Collects patient information from all available resources to formulate a comprehensive database and initial treatment plan
Coordinates care with the patient care provider (PCP), outpatient providers, and other services.
Participates in multi-disciplinary team in treatment planning to assure care delivered is matched to identified patient needs.
Responds to all assigned case management within Heywood Hospital for the provision of differential diagnostics, treatment planning and disposition.
Identifies insurance and obtains approval and/or a referral for services provided.
Provides individual and group therapies according to MentalHealth Unit standards to meet identified patient needs.
Documents the interventions provided and responses of each patient according to established policies and procedures.
Supports the MentalHealth Unit Staff in the supervision of students and interns so that performance standards and documentation requirements are met.
Seeks professional growth experiences to gain knowledge skills, and abilities, as well as, share what is learned with other team members.

Often, youth and their families need additional supports to make home a safe place for a child that is struggling with their mental and behavioral health. Safe at Home is a team of therapists who work intensively with a child and his or her family in their home for a period of 3 weeks or 3 months.
The Home for Little Wanderers' Safe at Home program in Somerville/Cambridge provides In-Home Therapy and Therapeutic Mentoring services to the Greater Boston area. Comprised of master's level clinicians and bachelor's level therapeutic training and support staff, In-Home Therapy teams provide intensive family therapy services, working to keep children and their families together in the community. Working from a variety of therapeutic modalities, Safe at Home emphasizes a family systems approach that works with every member of the family to help meet therapeutic goals.
The ideal candidate will have familiarity with working with children and families. Candidates should also possess strong verbal and written communication skills, superior time management skills, and be able to work both independently and on a team.
How You Will Be Making a Difference
Provide age-appropriate, culturally sensitive In Home Therapy services for clients and families.
Provide intensive family therapy.
Act as a liaison with collateral agencies and programs.
Identify and utilize community resources that support children's and family's growth.
Act as an advocate for families and help parents to build advocacy skills for themselves and their children.
Act as part of CBHI wrap-around service model.

Qualifications

The GLFHC Behavioral Health Team is currently seeking an experienced MentalHealthCounselor. Under the supervision of a LICSW, works collaboratively with all members of the interdisciplinary healthcare team, provides consultation to the clinical site team and provides individual and group counseling to GLFHC patients referred by primary care clinicians
Scope of Duties:
Serves as liaison between patients, family member and health care team. Helps to educate patients and their family about patients' behavioral health treatment. Explains treatment in a clear, concise manner, with sensitivity to patients' level of understanding, culture, age and language. Documents all teaching materials given to the patients, or family members including their response, comprehension and need for further follow-up.
Upon referral from the care team, reviews the patient's biopsychosocial history, and completes the assessment and care plan. Maintains accurate and timely records according to policy.
Provides short term individual, group and family intervention. Collaborates with community behavioral health providers in the coordination of behavioral health care.
Develops and facilitates support groups as determined by patient/client need, interest and team member recommendation.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Is able to identify and categorize each patient's age specific grouping of needs. This may include children, couples, families, and adults.
Collaborates with GLFHC clinical staff and administration to develop and maintain referral resources throughout service area. Refers patients to services within GLFHC and to other community agencies.
Develops working relationship with leaders and community agencies, representing GLFHC at meetings as indicated.
Demonstrates clinical competence and professional commitment by adhering to established standards of practice and utilizing appropriate scientific principles.
Develops and maintains systems for patient educational materials and staff.
On a regular basis participates in ongoing quality improvement activities related to individual, team, and organizational performance improvement.
Travel between sites required.
Collaborates with designated behavioral health professionals for patient care and regular case review.
