This position services the Mike Conley Hospice House in Clermont, FL. SHIFT - 8AM- 4:30 PM JOB DUTIES/KNOWLEDGE Provides direct service to patients and families as needed in a variety of settings. Works closely with Bereavement Services Manager in formulating policies and procedures for coordination and delivery of bereavement services. Collaborates with Bereavement Services Manager in development of a process to evaluate patient/family outcomes in the bereavement services area. Coordinates all bereavement components. Identifies patient/family issues related to family bereavement care. Conducts individual and group support for families. Implements and documents bereavement services.
