How you make a difference
As a Licensed Mental Health Professional working in a correctional facility, your responsibilities include conducting intake evaluations, providing therapeutic interventions and suicide prevention techniques, and developing treatment plans for inmates. You will also be responsible for monitoring the clinical needs of inmates on your caseload, providing staff training on mental health topics, and documenting all findings in accordance with company and facility policies. Key Responsibilities

Position Summary
The Mental Health Practitioner will provide mental health services to assist persons with active mental health diagnoses to live as independently as possible. Your services will be provided in an environment conducive to learning while assigned to the Adult Rehabilitative Mental Health Services (ARMHS) team.
Responsibilities
Assess functioning levels, strengths, needs, and vulnerabilities of people served to create a functional assessment plan using the individual's participation
Develop and implement ARMHS Treatment Plan reflecting needs identified in functional assessment
Provide access to referrals, outreach, coordinate and authorize funding for services including medical/psychiatric, vocational, social, educational, financial, legal, housing, and chemical dependency
Advocate for individual to ensure they are receiving the appropriate services for which they are eligible, in need of and requesting, particularly when communicating with other service providers, family members, and other people
Complete and maintain accurate case records, reports, and paperwork in a timely manner
Complete CEU and training requirements as prescribed
Establish professional rapport and communication with people served, partner agencies
Requirements
Bachelor's degree in human services, related field OR at least two years providing services for people living with severe and persistent mental illness
At least two years experience working with a Community Support Provider (CSP); community case management or inpatient experience with a focus on discharge and community resources
Has about 2000 hours of supervised direct service experience or other Mental Health Practitioner qualifications
A clear understanding of symptoms and psychosocial rehabilitation principles in providing services for people with major mental illnesses
Knowledge of chemical dependency issues, co-occurring mental illness and chemical health, and problems associated with domestic and sexual abuse survival.
Hardworking, flexible, and willing to work collaboratively with a team
Demonstrated experience using
Strong written and verbal communication skills
Ability to lift and carry at least 25 pounds

We are seeking a Mental Health Practitioner to join our Children and Families team. This full-time role will primarily operate in a school-based setting, and will also keep a small in-home caseload. The general purpose of this position is to provide mental health services to youth and families referred to Northern Pines Mental Health Center.
Essential Functions include:
Review and promptly respond to referrals. Clarify frequency and type of services referring worker is requesting
Coordinate and conduct intake sessions to include: complete intake paperwork with the family, obtain billing information, authorizations and continued care requests as needed. This will be done in conjunction with the clinical supervisor
Complete a treatment plan consistent with the needs identified in the evaluation, by the family and by the referring worker. Ensure the clinical supervisor signs the treatment plan
Ensure all documentation is completed as mandated (current diagnostic assessments, ROI's and renewing ROI's, treatment plans, functional assessments, progress notes within 24 hours of service delivery)
Prepare and provide content to clients in an individual and group setting
Facilitate and co-facilitate client groups as agreed upon with the clinical supervisor and colleagues
Maintain contact with the referral source as agreed upon
Ensure a diagnostic assessment has been completed initially and annually thereafter
Conduct transition planning and referrals as needed
Participate in Rule 29 and CTSS Supervision and Consultation at which time progress notes are submitted to the supervisor
Qualifications include:
Must qualify as a mental health practitioner as defined by the Minnesota Department of Human Services in one of the following ways:

Maintain knowledge of and compliance with our 245G Substance Use Disorder (SUD) Treatment Program Policy and Procedure Manual.
Ensure compliance with Department of Human Services (DHS) 245G Statutes including timely documentation completion, clinical thoroughness and accurate billing for services provided. Conduct chart audits as requested.
Understand the Grievance Policy to assist clients in navigating the process to resolve grievances at the lowest level in a timely manner and document accordingly.
Maintain knowledge of and apply American Society of Addiction Medicine (ASAM) Criteria. Appropriately diagnose clients using the DSM -5.Provide screening and assessment through SUD Evaluations/Assessments and required documentation.
Provide intake and orientation to programming and required documentation.
In conjunction with the client, create and monitor an individualized treatment plan based on client's needs, language, reading skills, cultural background and strengths.
Conduct counseling sessions in alignment with treatment plan goals, updating based upon ongoing assessment of client's needs.

Participate in consultation with other professionals by attending Weekly SUD Case Consultation. Present client cases monthly, implement feedback into client's treatment plan and document accordingly.
Address co-occurring disorders of clients, including client education on symptoms of mental illness, the possibility of comorbidity, and the need for continued medication compliance while recovering from substance use disorder.
Complete drug screens with clients based on clinical necessity. Therapeutically address drug screen results with clients.
Maintain the integrity of the SUD Program Structure including the need for weekly SUD Individual Sessions as part of ASAM Levels of Care.
Participate in scheduled individual clinical supervision meetings with supervisor.
Assist with training of professional program staff and provide supervision to assigned interns as requested.
