Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff. General Summary: The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing serious mental illness and/or addiction challenges to ensure their needs are addressed. The Case Manager supports clients in the development of their short and long-term goals and acquisition of community resources using a person-centered approach. Services will be provided with respect to cultural preferences and in accordance with client's rights. The person in this position participates in the development of comprehensive care coordination and treatment planning, advocates for persons on their caseload, utilizes evidence-based assessments, provides linkage and referral to additional community resources as needed, coordinates crisis intervention services and generally supports the client in their pursuit of emotional and physical health and wellness. Levels of support required by people on the caseload may vary greatly. The Case Manager works with the client in the community, home environment, and at agency locations.
