The Therapist works in a dynamic multidisciplinary team providing treatment services according to the Discovery Clinical Model. 1. Coordinate and implement individual, group, and family counseling in conjunction with plan of care goals.
2. Inform participants and their families of available community services and to refer participants, as necessary, to agencies providing these services.
3. Write monthly progress notes in participant’s record.
4. Complete an admission social work assessment and BIMS within 14 days of participant’s admission.
5. Complete BIMS assessment annually.
6. Schedule initial therapist evaluation and ongoing treatment plan.
7. Assist with discharge planning and refer participant to outside agencies.
8. Reports any and all suspicions of abuse/neglect to APS and documents accordingly.
