Responsible for providing behavioral health services within an integrated care model. With supervision by a LCSW, this position focuses on delivering counseling, care coordination and social support to patients across diverse populations ensuring access to comprehensive health services. In addition, LMSW will conduct evaluations/assessments, develops treatment plan and provide counseling, coordinates discharge planning, counseling, crisis intervention, resourcing, and case management services which require a high degree of independent decision-making and program administration; record appropriate documentation of services in the electronic health record; initiating and implementing behavioral interventions; Work is performed in an office setting under direct supervision by a LCSW.
EDUCATION, TRAINING AND EXPERIENCE:
Master's or Doctoral degree in the field of Social Work from an accredited program
Possess and maintain Licensure as a LMSW in Louisiana which must be active, unrestricted, current, and complete
Maintain CME/CEU requirements necessary for license renewal in Louisiana

JOB FUNCTIONS:
1. Knowledgeable with DSM diagnosis and ICD coding and required review and co-sign/formal approval of supervising LCSW or other clinically licensed mentalhealth professional

Essential Functions:
Principal Duties and Responsibilities:
Provide service coordination for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the ITT (individual treatment team); provide individual supportive therapy, psychotherapy, and symptom management, ensuring immediate changes are made in the treatment plans as clients' needs change; educate and support clients' families, and advocate for clients' rights and preferences.
Acts as liaison and consult with community agencies and families to maintain coordination in the treatment process.
Provide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff.

Provide individual and group treatment in the office and in community settings in a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance abuse, and has client-determined goals.
Coordinate with outside inpatient services to detoxify clients and establish linkage to outpatient treatment, self-help programs (e.g., Alcoholics Anonymous, Narcotics Anonymous), outpatient services, and residential facilities.
Take a lead role or participate in the provision of rehabilitation services.
Activities of Daily Living Services
Provide ongoing assessment, problem solving, side-by-side services, skill training, supervision (e.g., prompts, assignments, monitoring encouragement), and environmental adaptations to assist clients with activities of daily living.

Plan, structure, and prompt social and leisure-time activities on evenings, weekends and holidays.
Provide side-by-side support and coaching to help clients socialize (e.g., going with a client to a basketball game, coaching and supporting a client before he or she goes to a family reunion).
Organize and lead individual and group social and recreational activities to structure clients' time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support.

ESSENTIAL FUNCTIONS:
Demonstrate a sense of urgency related to the importance of patient safety and provide excellent customer services.
Ensure the well-being of patients and provide a positive, supportive and structured environment.
Responsible for conducting safety checks and ensuring that supervision is conducted at 15 minute intervals, as noted in special precautions, or in accordance with individualized supervision guidelines as needed.
Document timely, accurate and appropriate clinical information in patient's medical record.
Assist in providing a safe, secure and comfortable environment for patients, significant others and staff.
Interact routinely with patients, observe behaviors and communicate significant observations to nursing staff.
May oversee or assist patients with activities of daily living, including toileting, bathing, dressing, grooming, oral hygiene, meals, snacks, hydration and changing bed linens.
May obtain patient's vital signs, height and weight as assigned and document in patient record.
Facilitate patient educational-rehabilitative groups which cover a variety of topics including social skills, coping skills, anger management and independent living skills.
Engage patients in activities and interactions designed to encourage achievement of treatment goals.
Complete and maintain required documentation. Assist with follow-up and paperwork as required on incidents and events that may take place in the facility.
