The Psychiatric Nurse Practitioner (PNP) provides general psychiatric care and treatment to Grafton's client population (children and adolescents). The PNP assesses the physical/emotional needs of individuals with developmental disabilities and psychiatric conditions, prescribes or recommends drugs or other forms of treatment, and provides direct services as defined in the scope of practice that pertains to his/her license. ESSENTIAL JOB FUNCTIONS
Conducts intake assessments, with multiaxial diagnosis.
Identifies need for, initiates, and coordinates referrals for specialty consultations; Coordinates follow-up with external medical specialists.
Medication Evaluation and Review - Assess clients to determine if pharmacotherapy is warranted. Prescribe appropriate medication for client condition, conduct periodic medication review to monitor medication effectiveness and possible side effects, adjust medication regime according to client needs
Orders and interprets appropriate laboratory studies, x-rays, electrocardiograms, and other special examinations/testing to identify and assess client's clinical problems and health care needs.
Discuss clients with supervising physician, as scheduled.

Major Duties and Essential Functions:
Apply evidence-based practices and best practices critical time intervention and trauma informed care.

The psychiatric nurse practitioner diagnoses prescribe medication, monitors medication side effects and participates in the treatment process.
Essential Functions
Completes initial Psychiatric Evaluation
Evaluates all available clinical data (including laboratory and other instrumental reports) pertaining to clients' physical status when needed.
Performs clinical screening and referral of all conditions in need of consultation from medical, non-psychiatric specialties.
Reviews and follows up on treatments ordered by other physicians.
Obtains or reviews residents' or guardian's signature on the Consent to Treatment with Psychotropic Medication at admission and annually thereafter.
Prescribes follows up and reviews all prescribed neuroleptics.
Reviews all non-psychiatric medical orders and prescriptions.
Orders nonprescription first aid drugs (e.g., analgesics, antipyretics) as needed.

Position Summary
The Therapist is responsible for providing individual and group therapy sessions for clients in a specialized Outpatient facility. Position is responsible for assessing the therapeutic needs of the individuals and families, and for developing appropriate treatment. Implement crisis intervention techniques, such as de-escalation and safety planning to provide immediate support.

QUALIFICATIONS
A master's degree in counseling, or a human service-related field.

Responsibilities
What You'll Do:
The Mental Health Therapist provides services including individual therapy, group counseling, life skills sessions, clinical interventions, crisis management, family therapy, and assists in case management and support system development.
* Conducts a thorough, accurate, clinically sound Psychosocial Assessment with the individual within 24 hours of admission. Maintains the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individuals regarding treatment, as necessary.
* Provides internal education regarding DBT, trauma informed care, harm reduction, and service plan documentation.
* Prepares clinical reports and individualized service plans utilizing agency electronic clinical documentation software. Ensures documentation complies with best clinical practices, contracts, regulatory, and other legal requirements.
* Conducts brief interventions, individual, family and group counselling within scope of practice. Participates in redirecting individual behavior while maintaining a safe milieu when necessary. Documents all services and individual activities appropriately.
* Arranges for referral and placement of individuals in community substance use programs, housing, or employment opportunities.
* Provides case management services to promote recovery and community reintegration. Conducts crisis interventions and follow up as necessary. Maintains a working knowledge of the Virginia Involuntary Commitment process.
* Maintains a working knowledge of the DBHDS and DMAS roles within the Virginia behavioral health system.
* Participates in individual and group supervision as required. Participates in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervision.
* Completes continued authorizations for clinical and billing purposes.
* Completes all required training, attend monthly meetings, and maintain required competencies.
* Coordinates civil commitment process with necessary stakeholders.
* Performs all other duties as assigned.
Qualifications
What You'll Bring:
* Master's degree in social work, counseling, substance dependency, or related field
* Unrestricted license as Licensed Mental Health Professional in Virginia (LMHP, LMHP-R, LMHP-RP or LMHP-S and CATP) Accepted licensure includes: LGPC, LPC, LMSW, LCSW, LMFT, LCP, CSAC
* 3 years of experience in mental health, substance use treatment, or relevant work.
* Knowledge of principles of recovery, family of origin/intergenerational issues, and co-occurring psychiatric disorders
* Knowledge and experience working with special populations (individuals identified as having serious mental illness, substance use diagnoses, brain injuries, and those over 60 years of age)
* Must be able to perform Safe Clinch patient restraint techniques
* If working with youth, experience working within the children's system of care, their families, and stakeholders

This role will be responsible for a cross spectrum of clinical duties ranging from but not limited to, conducting individual, and/or family sessions, facilitating mental health individual sessions, completing mental health clinical assessments, conferring with onsite school advisors and completing documentation in our EMR system within the 48-hour time frame deadlines. The therapist should be comfortable working with adolescents, parents as well as providing services in the home as needed.
