Job Summary
This is an ideal opportunity for an individual who enjoys working in a team atmosphere and is dedicated to providing both behavioral health care and social services to the individuals we serve in the community.
The FACT Program is a multi-disciplinary, clinical team that assumes responsibility for directly providing needed treatment, rehabilitation and support services to adult persons with severe and persistent mental illness promoting recovery, empowerment, self-determination, and decision-making for each FACT participant.
Some of the responsibilities of the FACT team include providing clinical and supportive services through advocacy, crisis intervention, assessments, evaluations, treatment planning, casemanagement, and medication management. Participants are also provided with community referrals, symptom education, work-related vocational and educational services, social and interpersonal relationship training, and support for the participants families and care givers.
Most of a FACT team member's day is spent in the community completing many of the above duties. Only about 15% of their time is spent in the office. Company vehicles are available, however, members of the FACT team also need to use their personal vehicles to provide services and are eligible for reimbursement for mileage.
The FACT team operates both during and after normal business hours, rotates holiday and weekend responsibilities, and team members rotate carrying an emergency on-call cell phone.
Responsibilities
Provides treatment, rehabilitation, and support services in accordance with the FACT model, which is based on the Program for Assertive Community Treatment (PACT) manual
Assures the accuracy, completeness, and confidentiality of clinical records
Conducts mentalhealth status assessments
Coordinates referrals to community providers and support services
Provides assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills
Provides education related to symptoms, medications, and mentalhealth issues
Serves as a primary casemanager for an assigned group of individuals and promotes recovery, empowerment, self-determination, and decision-making for each FACT participant
Qualifications
In order to be considered candidates must have a Bachelors degree in Human Services or a related field.
Previous experience working with adults with severe and persistent mental illness (i.e. schizophrenia, bipolar disorder, borderline personality disorder) is required.
Each member of the FACT team must meet MHRC/RBHS vehicle driver requirements, including a valid Florida Driver's License, automobile insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits, and no more than eight points on their license for any combination of violations.
The FACT CaseManager position requires individual and group interaction with persons who have a serious and persistent mental illness in their home environment, a closed office, hospital, nursing home, and/or locked unit.
The FACT CaseManager needs to be able to make sound decisions independently on a daily basis, including but not limited to recognizing emergencies and life-threatening situations and responding appropriately.
Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including families, caregivers, community service providers, supervisory staff, and other department professionals.

Job Summary
The Multidisciplinary Forensic Team (MFT) is a comprehensive approach to divert individuals with severe and persistent mental illness from involvement in the criminal justice system, as well as commitment to Forensic State Hospital Facilities and other residential forensic programs by providing community-based services and supports.
The CaseManager provides needed treatment, rehabilitation, and support services to individuals served by the MFT Program with the goal of diverting these individuals from the criminal justice system or commitment to a Forensic State MentalHealth Treatment Facility. Promotes recovery, empowerment, self-determination and decision-making for each MFT participant.
Responsibilities
Provides treatment, rehabilitation, and support services in accordance with the Office of MentalHealth and Substance Abuse Guidance document for Forensic Multidisciplinary Team (MFT), which is adapted from the Florida for Assertive Community Treatment (FACT) model:
Provides initial and ongoing assessments.
Provision of treatment, rehabilitation, and support services as assigned.
Provides direct services to persons served by the program on an individual, group and family basis.
Maintains close working relationships with the judicial assistants in the circuit, the Office of the Public Defender, the Office of the State Attorney, and the Clerk of the Courts Office.
Works with the Forensic department to screen individuals for the MFT program when early notification of orders appointing experts and orders of commitment are received.
Assists as a resource person to community casemanagers, state hospital staff, LSFHS staff, DCF staff and jail/correctional personnel, on an as needed basis in matters involving forensic persons.
Completes regular risk assessments and monitors compliance with the conditional release plan to ensure public safety.
Prepares written reports for the court, including placement plans, progress reports, violation reports and other correspondence.
Provides competency restoration services.
Provides service coordination for an assigned group of individuals, which includes monitoring activities on the individuals recovery plan, conditional release plan, and competency restoration services.
Participates in scheduling team services.
Participates in team organizational meetings.
Provides crisis intervention as necessary, including evenings, weekend and holidays hours as assigned.
Participates in providing 24 hour a day on call coverage for the MFT program.
Assists the psychiatrist/ARNP in medication management and in monitoring individuals medications for effectiveness and side effects.
Provides stage-based substance abuse treatment and motivational interviewing through both individual and group meetings.
Advocates for individuals served by the program to ensure acquisition of necessary social services to include activities of daily living, including medical, dental, legal, housing, and entitlements and monitors and evaluates the effectiveness of the service and the persons satisfaction.
Facilitates each persons acquisition of and continuance of housing in the least restrictive environment available contingent upon court requirements.
Facilitates case conferences and family conferences, as needed.
Assesses and develops the individuals natural support system.
Communicates the needs of the individuals served to the MFT team.
Assesses and monitors for risk, symptoms of trauma, and indications of abuse, neglect and/or abandonment. Uses appropriate reporting mechanisms.
Assures the accuracy, completeness and confidentiality of clinical records.
Documents or obtains documentation of past medical, psychiatric and social history for each person served by the program.
Maintains current and comprehensive information in each record.
Completes clinical records within established time. Ensures records are maintained according to MHRC policies and procedures and in accordance with contractual and licensing requirements.
Develops and maintains the individuals recovery plan in coordination with other MFT team members. Ensures recovery plans contain all required services and signatures (including psychiatrists/ARNP). Coordinates recovery plan reviews/addendums, when clinically appropriate.
Maintains administrative records and responsibilities.
Maintains all records required for the completion of the monthly report(s).
Completes monthly report(s) within scheduled time periods.
Additional Duties for CaseManager Assigned as the MFT Housing Specialist:
Provides regular trainings and updated housing/resource information to MFT staff.
Acts as a liaison to the landlords of individuals served by the MFT.
Additional Duties for CaseManager Assigned as the Benefits and Resource Specialist:
Identifies resources for individuals and educates the other team members regarding the resources.
Certified as a SOAR processor and assists with Social Security applications using the SOAR model.
Assists individuals with accessing available entitlements.
Additional Duties for CaseManager Assigned as the MFT Forensic Specialist:
Educates team members about the forensic system.
Creates and manages a tracking system for Court reports.
Ensures the team completes and submits all required Court Reports.
Additional Duties for CaseManager who is a certified Peer Recovery Specialist: Provides peer counseling, support, coaching and mentoring to promote hope and empowerment.
Provides and facilitates peer support, such as Wellness Recovery Action Planning (WRAP).
Assists individuals to recognize their own strengths and identify internal and external resources.
Helps individuals increase their support network by making new friends and beginning to build a social networks.
Provides expertise, from a mentalhealth consumers perspective, to the individual served and service providers.
Qualifications

The FACT Program is a multi-disciplinary, clinical team that assumes responsibility for directly providing needed treatment, rehabilitation and support services to adult persons with severe and persistent mental illness promoting recovery, empowerment, self-determination, and decision-making for each FACT participant.
Some of the responsibilities of the FACT team include providing clinical and supportive services through advocacy, crisis intervention, assessments, evaluations, treatment planning, casemanagement, and medication management. Participants are also provided with community referrals, symptom education, work-related vocational and educational services, social and interpersonal relationship training, and support for the participants' families and care givers.
Most of a FACT team member's day is spent in the community completing many of the above duties. Only about 15% of their time is spent in the office. Company vehicles are available, however, members of the FACT team also need to use their personal vehicles to provide services and are eligible for reimbursement for mileage.
The FACT team operates both during and after normal business hours, rotates holiday and weekend responsibilities, and team members rotate carrying an emergency on-call cell phone.
Responsibilities
Provides treatment, rehabilitation, and support services in accordance with the FACT model, which is based on the Program for Assertive Community Treatment (PACT) manual
Assures the accuracy, completeness, and confidentiality of clinical records
Conducts mentalhealth status assessments
Coordinates referrals to community providers and support services
Provides assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills
Provides education related to symptoms, medications, and mentalhealth issues
Serves as a primary casemanager for an assigned group of individuals and promotes recovery, empowerment, self-determination, and decision-making for each FACT participant
Qualifications
In order to be considered candidates must have a Bachelor's degree in Human Services or a related field.
Previous experience working with adults with severe and persistent mental illness (i.e. schizophrenia, bipolar disorder, borderline personality disorder) is required.
Each member of the FACT team must meet MHRC/RBHS vehicle driver requirements, including a valid Florida Driver's License, automobile insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits, and no more than eight points on their license for any combination of violations.
The FACT CaseManager position requires individual and group interaction with persons who have a serious and persistent mental illness in their home environment, a closed office, hospital, nursing home, and/or locked unit.
The FACT CaseManager needs to be able to make sound decisions independently on a daily basis, including but not limited to recognizing emergencies and life-threatening situations and responding appropriately.
Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including families, caregivers, community service providers, supervisory staff, and other department professionals.

Conducts suicide risks screenings on referred individuals.
Completes face to face contacts within three working days of any missed follow-up appointments (i.e., medication management, counseling).
Coordinates a transition of care plan with the Co-Responder team when individuals are ready to be discharged from the program.
Documents or requests documentation of past medical, psychiatric, substance abuse, and social history for each individual.
Ensures care plans and/or treatment plans contain all required services and signatures.
Maintains, current, accurate, and comprehensive information in each record to include all activities, contacts and communications.
Compiles and submits reports as assigned.

Job Summary The Multidisciplinary Forensic Team (MFT) is a comprehensive approach to divert individuals with severe and persistent mental illness from involvement in the criminal justice system, as well as commitment to Forensic State Hospital Facilities and other residential forensic programs by providing community-based services and supports.
The CaseManager provides needed treatment, rehabilitation, and support services to individuals served by the MFT Program with the goal of diverting these individuals from the criminal justice system or commitment to a Forensic State MentalHealth Treatment Facility. Promotes recovery, empowerment, self-determination and decision-making for each MFT participant.
Responsibilities
Provides treatment, rehabilitation, and support services in accordance with the Office of MentalHealth and Substance Abuse Guidance document for Forensic Multidisciplinary Team (MFT), which is adapted from the Florida for Assertive Community Treatment (FACT) model:
Provides initial and ongoing assessments.
Provision of treatment, rehabilitation, and support services as assigned.
Provides direct services to persons served by the program on an individual, group and family basis.
Maintains close working relationships with the judicial assistants in the circuit, the Office of the Public Defender, the Office of the State Attorney, and the Clerk of the Courts Office.
Works with the Forensic department to screen individuals for the MFT program when early notification of orders appointing experts and orders of commitment are received.
Assists as a resource person to community casemanagers, state hospital staff, LSFHS staff, DCF staff and jail/correctional personnel, on an as needed basis in matters involving forensic persons.
Completes regular risk assessments and monitors compliance with the conditional release plan to ensure public safety.
Prepares written reports for the court, including placement plans, progress reports, violation reports and other correspondence.
Provides competency restoration services.
Provides service coordination for an assigned group of individuals, which includes monitoring activities on the individual's recovery plan, conditional release plan, and competency restoration services.
Participates in scheduling team services.
Participates in team organizational meetings.
Provides crisis intervention as necessary, including evenings, weekend and holidays hours as assigned.
Participates in providing 24 hour a day on call coverage for the MFT program.
Assists the psychiatrist/ARNP in medication management and in monitoring individual's medications for effectiveness and side effects.
Provides stage-based substance abuse treatment and motivational interviewing through both individual and group meetings.
Advocates for individuals served by the program to ensure acquisition of necessary social services to include activities of daily living, including medical, dental, legal, housing, and entitlements and monitors and evaluates the effectiveness of the service and the person's satisfaction.
Facilitates each person's acquisition of and continuance of housing in the least restrictive environment available contingent upon court requirements.
Facilitates case conferences and family conferences, as needed.
Assesses and develops the individual's natural support system.
Communicates the needs of the individuals served to the MFT team.
Assesses and monitors for risk, symptoms of trauma, and indications of abuse, neglect and/or abandonment. Uses appropriate reporting mechanisms.
Assures the accuracy, completeness and confidentiality of clinical records.
Documents or obtains documentation of past medical, psychiatric and social history for each person served by the program.
Maintains current and comprehensive information in each record.
Completes clinical records within established time. Ensures records are maintained according to MHRC policies and procedures and in accordance with contractual and licensing requirements.
Develops and maintains the individual's recovery plan in coordination with other MFT team members. Ensures recovery plans contain all required services and signatures (including psychiatrists/ARNP). Coordinates recovery plan reviews/addendums, when clinically appropriate.
Maintains administrative records and responsibilities.
Maintains all records required for the completion of the monthly report(s).
Completes monthly report(s) within scheduled time periods.
Additional Duties for CaseManager Assigned as the MFT Housing Specialist:
Provides regular trainings and updated housing/resource information to MFT staff.
Acts as a liaison to the landlords of individuals served by the MFT.
Additional Duties for CaseManager Assigned as the Benefits and Resource Specialist:
Identifies resources for individuals and educates the other team members regarding the resources.
Certified as a SOAR processor and assists with Social Security applications using the SOAR model.
Assists individuals with accessing available entitlements.
Additional Duties for CaseManager Assigned as the MFT Forensic Specialist:
Educates team members about the forensic system.
Creates and manages a tracking system for Court reports.
Ensures the team completes and submits all required Court Reports.
Additional Duties for CaseManager who is a certified Peer Recovery Specialist: Provides peer counseling, support, coaching and mentoring to promote hope and empowerment.

ESSENTIAL JOB FUNCTIONS
Provides discharge-planning activities for clients to include assessment of strengths and needs, services planning, linking services to clients, and client advocacy.

Appropriately utilizes Therapeutic Effective Aggression Management (CPI/CARE) to re-direct and prompt clients to safe and appropriate behavior.

Provides direction as necessary; always leads by professional example.

Manages outcomes and unit reports as directed by the Program Manager.
Prepares written documentation for each service conducted.

Performs other duties as assigned.

Appropriately utilizes Therapeutic Effective Aggression Management (CPI/CARE) to re-direct and prompt clients to safe and appropriate behavior.

Provides direction as necessary; always leads by professional example.
Coordinates discharge planning and appropriate aftercare services for assigned clients.
Discharge plan and aftercare services are documented in writing in accordance with Agency protocol including, but not limited to, documenting external referral sources contacted and outcome; internal referral sources contacted and outcome.
May assist in arranging/facilitating transportation which includes but is not limited to picking up bus/train/plane/transport tickets and/or documentation when necessary. Utilizes tracking mechanism for use of incidental funds for transportation.
Administrative assistance to the Program Manager, with maintenance of documentation of the clients identified as high utilizers. Maintains documentation for this population within the portal managed by the Managing Entity.
Manages outcomes and unit reports as directed by the Program Manager.
Prepares written documentation for each service conducted.
Written documentation for each service conducted is performed in accordance with the Agency's standards for service documentation in the client's medical record.
Performs other duties as assigned.

This program operates on the Linking, Advocating, Transitioning, & Recovery Support (LATRS) service model as defined in DCF Guidance 36. FACT Lite uses a multidisciplinary approach to deliver care to adults with serious mental illness (SMI) by providing mentalhealth rehabilitation interventions and supports necessary to assist participants to achieve and maintain rehabilitative, resiliency, and recovery goals. FACT Lite also provides clinical casemanagement and recovery support that promotes continuity of care and ease of service access until a full transition to community-based care is optimal.
Services provided to individuals served by the FACT Lite Program include, but are not limited to: casemanagement; care coordination; medication oversight and support; psychoeducation regarding mentalhealth challenges and treatment options; assistance and support in addressing health care needs; life skills development; support with furthering education or gaining employment; and working closely with individual families or other natural supports.
Most of a FACT Lite team member's day is spent in the community completing many of the above duties. Only about 25% of their time is spent in the office. Company vehicles are available, however members of the FACT Lite team also need to use their personal vehicles to provide services and are eligible for reimbursement for mileage.
The FACT Lite team operates Monday through Friday from 8:00 am to 5:00pm. Team members rotate providing crisis on-call coverage after normal business hours, on the weekends and holidays.
Responsibilities
Provides treatment, rehabilitation, and support services in accordance with DCF Guidance 36 Linking, Advocating, Transitioning, & Recovery Support (LATRS) Florida Assertive Community Treatment (FACT), Tier 2
Assures the accuracy, completeness and confidentiality of clinical records
Provides initial and ongoing assessments and evaluations and completes portions of the comprehensive assessment as assigned
Advocates for individuals served by the program to ensure acquisition of necessary social services to include activities of daily living, including medical, dental, legal, housing, and entitlements and monitors and evaluates the effectiveness of the service and the persons satisfaction
Provides assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills
Provides education related to symptoms, medications, and mentalhealth issues.
Provides service coordination for an assigned group of individuals which includes monitoring process made towards goals and objective and developing, writing, implementing, evaluating and revising the recovery plan in partnership with the individual and other providers.
Qualifications
Bachelors degree in a behavioral science field, with preference for individuals also credentialed as a Certified Recovery Peer Specialist (CRPS).
Previous experience working with adults with severe and persistent mental illness (i.e. schizophrenia, bipolar disorder, borderline personality disorder) is required.
Each member of the FACT Lite team must meet MHRC/RBHS vehicle driver requirements, including a valid Florida Driver's License, automobile insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits, and no more than eight points on their license for any combination of violations.
The FACT Lite Case Specialist position requires individual and group interaction with persons who have a serious and persistent mental illness in their home environment, a closed office, hospital, nursing home and/or locked unit.
The FACT Lite Case Specialist needs to be able to make sound decisions independently on a daily basis, including but not limited to recognizing emergencies and life threatening situations and responding appropriately.
Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including families, caregivers, community service providers, supervisory staff and other department professionals.
Each FACT Lite team member contributes to the completeness and confidentiality of clinical records by ensuring documentation, paperwork and system entries meet internal and external guidelines for content, accuracy and timeliness.

MentalHealth Resource Center is a not-for-profit Florida corporation that provides a wide range of mentalhealth and behavioral health care services to the community such as 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults as well as outpatient services such as medication management, casemanagement, and counseling.
Job Summary
Behavioral Health Assistants provide competency-based direct care to children, adolescents, and adults in our inpatient behavioral health units. Behavioral Health Assistants assist in providing a safe, clean, and therapeutic environment for our patients. These inpatient units provide crisis stabilization services and most patients are admitted involuntarily to the facility via the Baker Act, the mentalhealth act of Florida. The normal length of stay on the unit for patients is typically 3-5 days. This is an ideal opportunity for an individual who is dedicated to providing a caring, therapeutic environment and who enjoys working in a team atmosphere. We provide a variety of training, including but not limited to Crisis Intervention, CPR, Diagnosis and Behaviors, Ethics, and Confidentiality.
Each team member contributes to the completeness and confidentiality of clinical records by ensuring documentation, paperwork, and system entries meet internal and external guidelines for content, accuracy, and timeliness.
Responsibilities
Monitors behaviors and documents observed behaviors as indicated in the patient record.
Provides ongoing attention and interaction with patients in order to establish supportive relationships, learn patient concerns and goals, and work to assist patients in achieving successful outcomes.
Assists patients with personal hygiene and activities of daily living skills (ADLs) in accordance with age, cultural consideration, and level of functioning based on current acuity and diagnosis.
Ensures implementation of the daily schedule that meets patients needs and interests.
Performs safety checks at a minimum of 15-minute intervals on patients in accordance with policy and procedure or as directed by the supervisor.
Supervises patients during fresh air breaks and visitation periods.
Remains knowledgeable of admission/discharge status, patient level and privileges/restrictions, special information, treatment plan, and unusual events.
Performs and documents vital signs on patients, under the direct supervision of a Registered Nurse.
Qualifications
In order to be considered, candidates must have a High School Diploma or equivalent.
Certified Nursing Assistant (CNA), Emergency Medical Technician (EMT), or Paramedic (PMD) certification is preferred.
One year of experience working in direct patient care is preferred.
Excellent customer service skills are essential and this individual must be able to interact appropriately with internal and external customers, including patients, families, caregivers, community service providers, supervisory staff, and other department professionals
