The Assessment Specialist serves as a specialized case manager for individuals and their families with behavioral health needs in order to help guide treatment preferences and recovery-oriented services. This position will be responsible for overseeing the timely completion of the Texas Resilience and Recovery (TRR) assessments, Person Centered Recovery Plans (PCRP), annual consents, and various other documentation and assessment tools needed for enrollment in behavioral health services. This position will require ability to work outside of normal business hours in office or community-based settings in order to meet individual and family needs and preferences. This position will monitor upcoming expiration dates for those seeing clinic prescribers, med distribution and pickup, as well as support intake for completion of initial recovery plan development. The Assessment Specialist will work collaboratively with program leadership in both Youth and Family Services and Adult Services to meet the unique needs of those served.
This Assessment Specialist exemplifies service excellence and will be available as needed to support the case management team meet the needs of individuals in services through seeing individuals in crisis, taking walk-in appointments, filling in for case managers, or updates needed prior to prescriber appointments. Is responsible for initiating and collaboratively completing TRR and PCRP prior to expiration and as needed based on individual change. Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record.
This Assessment Specialist practices good stewardship of resources by ensuring all required trainings are completed prior to due date and center-issued equipment is kept in good repair. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, Medicaid billing rules, contracts, grants, and Lifepath Systems policies and procedures.
This Assessment Specialist builds positive relationships with community-facing agencies and always maintains a professional relationship with all community stakeholders and partners. Professional conduct with Peers, Leadership and Support teams is expected at all times. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with improving access to social determinants of health.
Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and community partners; to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve. Ability to assess and de-escalate behavioral health crisis situations in calm and respectful manner.
Specific Targets: At minimum, complete 3 TRR/PCRP and/or crisis assessments a day and 150 contact attempts per month. Expired TRR/PCRP and autodischarges will be monitored for decrease month-over-month. 100% of TRR/PCRP development will result in subsequent service provided within 2 business days. 90% of service notes are signed within 2 business days of service. 75% of full chart reviews receive a score of 80% or higher. All required trainings are completed prior to due date.
If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.
Qualifications
Bachelor's degree in psychology, social work, or related field from a four-year college or university.
At least two years of related experience and/or training in the field of mentalhealth preferred.
Must have reliable transportation and a valid driver's license.
Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.
Ability to assess individual needs, to coordinate services, and to communicate effectively.
Knowledge of the social services system and the ability to connect individuals with appropriate services.
Ability to write clearly and professionally.

This position will provide telephonic crisis assessment with the City of Dallas 911 call center dispatch. The role of the behavioral health clinician is to screen and assess for risk harm, to identify the level of priority for the call. The clinician will work in conjunction with the City of Dallas staff and Dallas Police Department and assist in communicating, prioritizing, and coordinating care as deemed clinically appropriate at the time of the call.
HOURS OF RESPONSIBILITY: Shift 1-7am-3:00pm and Shift 2- 2:00pm-10:00pm
