Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.

Position Summary
Brooklyn Community Services PROS is strengths-oriented psychiatric rehabilitation program devoted to helping adults increase independence and overall well-being. The PROS Counselor aids people living with persistent mental illnesses in developing life role goals, in managing symptoms, and in taking concrete steps toward a better life. The PROS Counselor does so through the provision of evidence-based individual and group services aimed at the development of therapeutic and life skills. Working in collaboration with an interdisciplinary team, they help program participants identify an individualized, meaningful course of care to become as self-sufficient as possible.
Responsibilities
* Manage a caseload of up to twenty-five participants, providing individual counseling and case management as clinically appropriate
* Develop and maintain collateral relationships with outside supports including family, case managers, housing program staff, and medical providers
* Conduct required assessments including the biopsychosocial, psychiatric rehabilitation, vocational readiness, cultural, and substance dependence screenings
* Develop Individualized Recovery Plans (IRPs) in collaboration with participants based on the above listed assessments on a semi-annual basis, or more often as needed
* Develop and facilitate approximately four to twelve skills-based workshops per week
* Document participants progress/regression towards goals in monthly progress notes and determine the need for service modifications on an ongoing basis.
* Provide services according to evidence-based practices including Wellness Self-Management, Family Psychoeducation, Integrated Dual Disorder Treatment, Individual Placement and Support
* Coordinate follow-up services once a participant is ready for discharge from the program.
* Act as a member of an interdisciplinary treatment team, including participation in weekly administrative and clinical meetings
* Possess/develop proficiency in Microsoft Word, Excel, and AWARDS electronic health record
* Manage responsibility for maintenance of records, including timely documentation of services as required by the Center for Medicaid and Medicare Services and the Office of MentalHealth
* Conduct intakes, as needed
Qualifications Required
* Masters degree in one of the following: Social Work, MentalHealth Counseling, Creative Arts Therapy, Occupational Therapy, Rehabilitation Counseling, Psychology, Marriage and Family Therapy, or related mentalhealth qualifications
* Computer skills and knowledge of service documentation required
* Must undergo pre-employment screenings such as the Criminal Background Check required by OPWDD
* Professional experience in a PROS program, inpatient psychiatric setting, or program serving adults experiencing psychosis preferred
* Professional experience serving adults with persistent mental illnesses, particularly psychosis preferred
* Professional experience with group facilitation preferred

Job Description
Community of Veterans (CoV) is the first and largest online social network exclusively for paperwork-confirmed Iraq and Afghanistan veterans. CoV provides its more than 24,000 members with access to message boards, affinity groups, resources, and live chats with experts on a range of issues - especially those related to mentalhealth. CoV provides veterans with a safe space to share what they're going through with others who “get it” because they've been there too. An informal peer-to-peer support system and means of connecting with resources and experts, CoV is an invaluable gathering space for veterans grappling with PTSD, Traumatic Brain Injuries, depression, and even thoughts of suicide.
The Online Community Intern will be responsible for supporting the IAVA Programs department to promote engagement within CoV through outreach and communications with members, posting content and resources, and scheduling live chats and other events. The Online Community Intern will report to the Senior Program Manager.
The Online Community Intern will:
Interact with Iraq and Afghanistan veterans online and off by conducting outreach to promote CoV sign-ups at IAVA events and interacting with veteran members in CoV
Learn about methods for digital outreach and engagement, through drafting social media announcements and blog posts, and through planning and executing online events, such as webinars and live chats
Learn about mentalhealth issues affecting this generation of veterans and gain exposure to key resources
Enhance skills in data analysis by monitoring and reporting on trends among CoV member needs and interests
Assist with creating and executing a crisis prevention and response system within CoV
Qualifications
The Online Community Intern should possess:

Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Clinical Social Worker in State applying for.
Effective oral and written communication in English and Spanish
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.
Excellent time management and organizational skills.

Job Description
General Purpose:
The Behavioral Health Counselor is working with a PROS program to conduct individual and group services.
Personalized Recovery Oriented Services (PROS) is a comprehensive model that integrates rehabilitation, treatment, and support services for people with serious mental illness and possible other co-occurring disorders.
The Behavioral Health counselor works with PROS participants towards goals in different areas:
Living independently
Building natural supports
Finding and keeping a job
Reaching higher levels of education
Securing preferred housing
Improving medication management
This will be carried out as indicated by NYS OMH Part 512 Regulations and Medicaid requirements.
Essential Functions:
Manage a caseload of up to 25 consumers, including case management, collateral contacts, referrals, vocational assistance, and other needs, to coordinate services.
Conduct required assessments including Wellness Recovery Action Plans, Psycho-socials, Psychiatric Rehabilitation, and Substance Use screenings and assessment.
Develop Individualized Recovery Plans (IRP) in collaboration with participants, on a bi-annual basis, or more often as necessary.
Maintain and update demographic, treatment and employment data in electronic health records (Foothold AWARDS, OMH CAIRS database, and NYESS).
Facilitate member access to community support services including medical, mentalhealth, substance abuse, housing and entitlements.
Develop and facilitate approximately 6 -8 groups, according to protocol and curriculum, which will cover CRS, ORS, IR and Clinical Services as dictated.
Conduct individual counseling sessions to assist program participants with attainment of the life role goals specified in their IRPs.
Document consumers' progress towards goals in monthly progress notes and determine the need for addition or subtraction of services offered to each participant on an ongoing basis.
Coordinate treatment including the sharing of Individual Recovery Plans and Progress Notes for participants who are co-enrolled with other agencies as necessary.
Provide services according to Evidence Based Practices including Wellness Self Management, Family Psychoeducation (CCFC), Medication Management, Co-occurring Disorders (IDDT), and Supported Employment (IPS).
Coordinate follow-up services once a participant is ready for discharge from the program.
Act as a member of an interdisciplinary treatment team, including participation in weekly administrative, clinical and utilization review meetings.
Assume overall responsibility for maintenance of client records, including timely documentation of services in Electronic Health Record (AWARDS), as required by Medicaid and the Office of MentalHealth.
Develop and maintain collateral relationships with consumers' other providers and family as clinically indicated
Carry a 24-hour support cell phone in rotation with the clinical team.
Other duties as assigned.
Qualifications/Basic Job Requirements:

A minimum of a Bachelor's Degree in Psychology, Social Work, Human Development and related field, 3+ years of experience working with adults with mental illness and/or families dealing with mental illness.
Masters Degree preferred.
Team-based skills are essential.
Previous experience working with individuals with psychiatric/co-occurring disorders preferred. Knowledge of running groups preferred.
Computer skills and knowledge of service documentation required.
Criminal background check and fingerprinting required; screening for the GSA, OIG and OMIG lists of persons disallowed from working in Medicaid funded programs
