RESPONSIBILITIES
The Social Worker will conduct intake assessments.
Provide individual and group therapy to adults, adolescents and children with the full range of DSM diagnoses, including both mentalhealth and substance use disorders, in an acute care, outpatient setting.
The Social Worker will complete all clinical documentation in accordance with NYS Office of MentalHealth in an electronic medical record.
Provide linkages and follow up to clients during stabilization period and immediately following.
The Social Worker will coordinate with outside providers and collateral contacts.
Attend routine supervisions and team meetings.
Coordinate treatment with a multidisciplinary team.
Complete trainings and maintain proficiency with de-escalation and crisis intervention skills.
All other duties as assigned.
QUALIFICATIONS
A masters degree in a relevant clinical field is required.
Clinical Licensure (LMSW, LCSW, LMHC or limited permit) is required.
At least six months experience in a behavioral health setting is required. Clinical experience is preferred.
Proficient computer skills, including Microsoft Office, are required. Experience with an Electronic Health Record is preferred
Fluency in Spanish is preferred.
Overnight hours required, daytime or evening availability required for training
Daytime or evening availability required periodically for supervision and meetings.

Provides interventions (individual, family) that promote the amelioration of problematic symptoms and behaviors, from both a clinical and non-clinical lens.
Evaluates client risk on an ongoing basis and formulates and implements appropriate safety plan for inside and outside sessions.
Collaborates regularly with internal CMHA programs and external agencies such as schools, PCPs, pediatricians, DCF, probation, court, and other social services.
Maintains accurate documentation and submits within program and funder timeframes.
Maintains appropriate caseload as determined by CMHA and funder.
Performs other related duties as assigned, which may include urine toxicology screens, Reunification Services, Therapeutic Family Time, etc.
Requirements:
Master's degree in behavioral health (psychology, social work, counseling, marriage and family therapy).
LMSW, LPC-A, MFTA required and must be eligible for LCSW, LPC and LMFT.
Must have a valid driver's license and be able to obtain a public service license.
Preferred Experience:

Qualifications

RESPONSIBILITIES
The Social Worker will conduct clinical crisis assessments as part of a Mobile Crisis team serving all of Suffolk County for adults, adolescents and children with the full range of DSM diagnoses, including both mentalhealth and substance use disorders.
Screen and collect referral and collateral information from third party callers to inform assessment and linkage to immediate and long term supports.
The Social Worker will provide telephonic triage, assessment and de-escalation to determine the best disposition for individuals in crisis.
Utilize resources such as PSYCKES to review relevant clinical information.
Work closely with law enforcement to accept referrals and provide a co-response to individuals in crisis, when clinically necessary.
Complete all clinical documentation in accordance with NYS Office of MentalHealth in an electronic medical record.
The Social Worker will provide in-person/in-community de-escalation, clinical assessment and linkage to individuals in crisis.
Coordinate with outside providers and collateral contacts.
Work as part of a multidisciplinary team, conferencing cases, communicating outcomes and providing follow up.
Attend routine supervision/team meeting.
Complete training and maintain proficiency with de-escalation skills.
All other duties as assigned.
QUALIFICATIONS
Masters Degree in a relevant clinical field is required.
LMSW, LCSW, LMHC or limited permit is preferred.
At least six months of experience in a behavioral health setting required; clinical experience with mentalhealth and/or substance use disorders is preferred.
Proficient computer skills, including Microsoft Office required; experience with an Electronic Health Record is preferred.
Excellent interpersonal and verbal and written communication skills are required.
Ability to problem solve, multitask, make clear decisions and work as part of a team are required.
Valid and clean New York State Drivers License and ability to safely operate a motor vehicle required.
Bilingual in Spanish is preferred.
