ESSENTIAL DUTIES AND RESPONSIBILITIES Provide bilingual and culturally responsive Adult Mental Health Targeted Case Management (AMH-TCM) services to help adults with Serious and Persistent Mental Illness (SPMI) gain access to medical, social, educational, vocational and other necessary services connected to the person's mental health needs. Complete required client assessments and plans in a timely manner, which includes the Functional Assessment (FA), Individual Community Support Plan (ICSP), and client crisis plans. Review and update these plans every 180 days. Maintain up-to-date client documentation, including case notes and agency required reporting. Based on a person-centered assessment of each client's needs and existing supports, refer and link clients to mental health and other services, ensuring coordination of services across settings, and monitoring the delivery of services. Support clients at mental health related meetings, appointments, and court appearances. Provide transportation as needed and deemed appropriate. Support clients around the process of civil commitment and re-entry into the community, including monitoring client progress and making recommendations to the court, when necessary. Respond promptly to client crisis situations and seek support from supervisor in high-risk situations. Collaborate with Behavioral Health leadership and staff to provide integrated care within the CCBHC model. Participate in individual and group supervision as required within the team and the department. Attend other meetings as required by contract or agency. Support supervisor in cultivating and maintaining a positive working relationship and regular communication with contract stakeholders and service collaborators. Meet goals and outcomes set for case load and in-person client contact requirements (e.g., meeting at least monthly with each client, and serving a pre-determined number of clients per month, etc.). Candidates must satisfy at least one of the three scenarios below: Be a mental health practitioner as defined in section 245I.04, subdivision 4; Have a bachelor's or master's degree in one of the behavioral sciences or related fields including, but not limited to, social work, psychology, or nursing from an accredited college or university; or Candidates who are not mental health practitioners or don't have a bachelor's degree may only be considered if one of the following applies: Have 3 to 4 years of experience as a case manager associate; Be a registered nurse and have a combination of specialized training in psychiatry and 3 years of work experience consisting of community interaction and involvement or community discharge planning in a mental health setting; or Be qualified as a case manager under the 1998 Department of Human Service waiver provision and meet the continuing education and mentoring requirements. Bilingual in Spanish and English required. Must have a deep understanding of Latinx cultures, values, and traditions, and demonstrate cultural responsiveness in every aspect of the role, including communication with colleagues, staff, clients, community members; creating forms and documents; hiring and training staff; and in clinical supervision and consultation. Experience in case management or other direct client services preferred. Must be organized, have the ability to work under pressure and stress, have attention to detail, be self-motivated, and have the ability to multi-task in a fast-paced environment. Must be skilled in the process of identifying and assessing a wide range of client needs. Must be comfortable discussing sensitive issues with clients and staff in an appropriate, effective manner. Crisis de-escalation skills and experience preferred. Knowledgeable about local community resources and how to use those resources for the benefit of clients preferred. WAGE RANGE: $27.00 - 30.00/hourly JOB SUMMARY The Mental Health Case Manager will be embedded in one of Catholic Charities' permanent supportive housing program and will be assigned to manage a case load of clients with mental health needs. The Mental Health Case Manager will report to the Program Manager with oversight from the Director of Behavioral Health. The practitioner will provide services that are rehabilitative and enable the client to develop and enhance psychiatric stability, personal and emotional adjustment, social competencies, community skills and independent living skills. The practitioner will support the client in strengthening skill areas needed according to their treatment plan. Catholic Charities provides services to clients in Hennepin or Ramsey County. ESSENTIAL FUNCTIONS Conduct individual assessments, assessing functional strengths, resources and needs. Develop individualized, person-centered, trauma informed community support plans. Implement interventions identified in support plan, support client with referrals to external providers, maintain connection with external providers and provide case coordination support. Develop therapeutic relationship with team and client. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary. Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies. Ensure confidentiality of all records and interactions. Coordinate services both internally and externally including county case manager, family/guardian of client, work personnel, medical personnel, etc. Attend supervision, case consultation meetings and participate in team-based decisions regarding all clients and services. Maintain client files, update client files, update treatment plans a minimum of once every six months and do all required documentation as specified in DHS guidelines, utilizing professional writing skills. Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes. Provide case management to support housing retention, including supporting timely rent payment, renewal of benefits, maintaining a habitable housing unit, etc. May transport clients Other duties as assigned.
