CLUES' Adult Mental Health Targeted Case Manager collaborates with Behavioral Health leadership and staff to provide integrated care within the CCBHC model. Support clients around the process of civil commitment and re-entry into the community, including monitoring client progress and making recommendations to the court, when necessary. Respond promptly to client crisis situations and seek support from supervisor in high-risk situations. Collaborate with Behavioral Health leadership and staff to provide integrated care within the CCBHC model. Participate in individual and group supervision as required within the team and the department. Attend other meetings as required by contract or agency. ESSENTIAL DUTIES AND RESPONSIBILITIES Direct Mental Health Therapy Services and Program Development Provide culturally responsive mental health therapy services to families, individuals, couples, and groups in English and Spanish in a variety of settings, including in the office, in schools, and via telehealth. Consult with other staff and interns within Behavioral Health and other departments on mental health related topics. Attend and participate in clinical case consultation meetings and other client staff meetings. Cultivate working relationships with potential service collaborators, partners, and other appropriate stakeholders. Provide consultations and presentations to individuals and/or groups in the community, other professionals and organizations. Conduct immigration-related assessments and produce accompanying documentation to support the immigration applications of CLUES clients. Cultivate working relationships with CLUES staff within and across departments, to support one another's work, facilitate client referrals, and improve client flow throughout the agency. Participate in care coordination with other providers who may be supporting clients in different capacities. Contract and Program Compliance Ensure goals for weekly billable client contact are met. Maintain up-to-date clinical documentation per department standards and all requirements. Ensure client data is up to date to facilitate timely processing of service lines for billing purposes. Adhere to contract expectations related to the provision of mental health therapy and related activities. Respond to all internal & external data collection & reporting requests. Participate in all internal and external activities required as stated in program contracts. Assist in the delivery of contracts outcomes and relationships with contractor stakeholders. General Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards. Contribute to the collaboration of the Behavioral Health department with other programs within CLUES to achieve service integration. Participate in weekly supervision meetings, biweekly team/group meetings for Behavioral Health, "All Staff" meetings as scheduled, and other meetings as assigned. Follow the code of ethics of the applicant's professional board. Carry out all other duties as assigned by supervisor. With clinical licensure, must be eligible to be credentialed with all major insurance companies.
