Essential Duties: Provide action oriented brief therapy to clients starting services Provide Evidence Based treatment for a range of mental health and substance use disorders Provide on-going assessments, crisis intervention and case management to clients and their families Develop and monitor service plans, treatment progress, and maintains clinical documentation and other reporting data Coordinate services in a treatment team model with internal as well as external services Have a strong understanding of how to adapt service delivery to meet the client's needs. Provide individual, marital, family, group and rehabilitative services as appropriate to client needs. Make appropriate referrals as needed during course of treatment.
