Role Overview:
We are hiring for a Therapist for our Assertive Community Treatment (ACT) and Specialized Treatment Services (STS) Teams.
The ACT and STS Teams are a multi-disciplinary team of: case managers, therapists, providers, medical assistants, and nursing staff. ACT and STS clients are often not successful in typical Outpatient Services and need dynamic approaches and interventions to gain and maintain stability.
Specialized Treatment Services (STS) works with clients with serious and persistent mental illness who typically have co-occurring diagnoses and are often involved in the criminal justice system. Additionally, The STS Team works closely with SummitStone's Competency Team to support clients going through the competency process.
Assertive Community Treatment program is to develop and provide high quality, community-focused behavioral health services to improve the everyday lives of those who may be living with serious and persistent mental illnesses or conditions. The team follows the SAMHSA's ACT Evidence Based Practice as a fidelity program with the State of Colorado.
Essential Duties:
Provide action oriented brief therapy to clients starting services
Provide Evidence Based treatment for a range of mental health and substance use disorders
Provide on-going assessments, crisis intervention and case management to clients and their families
Develop and monitor service plans, treatment progress, and maintains clinical documentation and other reporting data
Coordinate services in a treatment team model with internal as well as external services
Have a strong understanding of how to adapt service delivery to meet the client's needs.
Required:
Master's degree in related field from accredited college or university
1 year of experience providing individual and group therapy
Proficiency in using computers and telehealth systems • Certified Addiction Specialist (CAS), Licensed Addictions Counselor (LAC), or willingness to pursue a CAS or an LAC
Includes driving a company vehicle and requires a valid Colorado driver's license. At times, personal vehicle may need to be used (mileage on personal vehicle use is reimbursed)

Currently looking for a Physical Therapist to fill an opening within an outpatient setting located in the Cheyenne, Wyoming area. Interested candidates should have at least a Bachelor of Science degree in Physical Therapy with a passion to work within an orthopedic setting. Responsibilities of the Physical Therapist (PT) (DPT) (MPT) (Staff Physical Therapist):
Performs and documents initial evaluation and ongoing assessments of patient's condition, adapting treatment plans accordingly
Develops and implements an individualized plan of care to meet required frequency of visits and which includes patient/family.
Communication of patient progress with patient, family, and interdisciplinary team
Patient and family education of proposed treatment plan and progress to include skilled recommendation on AE/DME and community support where available and when appropriate
Supervises activities of assistants, students, and support personnel as needed.
Plan and or assist in patient discharge.
Ability to work in a typical clinical environment, stamina to remain standing or sitting for extended periods of time, and lift/assist patients as needed

Licensed clinician needed to join
a staff of nearly 200 and a clinical team of more than 110 specialists, including one of Wyoming's finest child psychiatrists at an inclusive, private, nonprofit clinic that provides high-quality services affordable for anyone in the community by accepting most insurance, Medicare, and Medicaid while also offering a sliding fee scale based on financial status.
Clinician will receive referrals for services. Conduct intake assessments. Complete required documentation and develop initial Treatment Plans. Provide individual, marital, family, group and rehabilitative services as appropriate to client needs. Make appropriate referrals as needed during course of treatment.
Develop and maintain positive working relationship with other community providers. Coordinate with other staff and community providers in providing joint treatment as reflected in treatment plan. Prepare needed reports of treatment as requested.
Maintain case records to reflect course of treatment and provide required documentation. Provide follow-up and aftercare planning services as needed. Expeditiously discharges inactive clients.
Provide emergency services during work hours and after hours as assigned. Provide program and case consultation and education services internally, and to other community groups and individuals. Participate in agency program development activities.
Provide liaison service for local, regional and state hospitals to ensure continuity of care for Center clients
