About The Role

* Coordinates the child studies for mentalhealth consultants: arranges observation and assessment dates, notifies teachers and parents, and ensures that all release forms are on file. Organizes the information for referrals for individual educational plans (IEPs) and IEP staffing with Lake County Public Schools, as well as IFSPs and IFSP Child & Family Team Meetings with the CFCs.
* Assists Education Coordinators in developing the Student Learning Plans and Positive Guidance plans, and assists in the implementation of plans in the classroom and refers children to local public school or CFS for services, and advocates as cases need assistance in negotiating the services at the public school or with the CFC
* Attends the staffings with the parents, advocates for and interprets the results of the IEP into work plans for the classroom teachers and parents with the Education Coordinators, provides the in-class follow-up for all IEPs from the Lake County Public Schools and work plans from the MentalHealth Consultant.
* Maintains a tracking system for all program options; assessments, IEPS and up-dates,ypes of disability or special needs, and other data as needed for the PIR and other reporting.
* Assures that all files, documents and records reflect the program standards of the various entities to which the program is responsible; ensures that the documentation is complete, accurate and up to date, and that all files are secure.
* Conducts internal referral staffings at all program sites and complies with and submits statistics and other reports as directed.
* Develops and schedules the classroom observations by the MentalHealth Consultant for each site. Conducts primarily individual MH therapy sessions, with family and couples therapy sessions being provided, as deemed necessary & appropriate to the client's treatment protocol.
Maintains case files (i.e., clinical documentation) for all clients assigned to caseload, with all files being subject to regular internal quality assurance audits.
Works as part of a team to further develop program initiatives and to increase program referral network.
Performs community outreach & advocacy work with the goal of increasing the visibility & understanding of Perinatal Mood Disorders and Mental Illness, and to promote community treatment options.
Identifies and provides referrals for additional services, as needed.
Engages in regular clinical peer review of cases, particularly those involving the potential risk for self-harm or harm to others (particularly in cases where an infant or child could potentially be at risk).
Attends Group Supervision/Team Clinical Case Review meetings/Discharge Reviews/COD.
Participates in agency sponsored trainings, agency meetings, community outreach events, health fairs, fundraising events, etc., as directed by the Program Manager.

REQUIRED SKILLS/ABILITIES
Should have some familiarity with IL-DMH Rule 132 & IL-HFS Rule 140 and their general provisions & requirements pertaining to the delivery & documentation of Community MentalHealth services.
Must possess above average written and verbal communication skills.
Must possess the ability to work independently while supporting the larger team.
Must be highly self-motivated, disciplined, organized, and require minimal oversight.

EDUCATION & EXPERIENCE

Caseloads will start small with the opportunity to grow into full time positions. Therapists may not work in other groups, private practices, or outpatient settings, but may work in other therapeutic settings that do not present a conflict of interest as they build their caseload.

This individual will be responsible for developing, monitoring and actively implementing programming to meet the psychological needs of the individuals with intellectual and physical disabilities as well as those dually diagnosed.
Shift: Monday - Friday 7:00 AM - 3:30 PM OR 8:00 AM - 4:30 PM (No Weekends)
Pay Rate: $50,000 annually
Responsibilities:
Designs individualized treatment plans for the individuals served
Actively participates in crisis intervention and de-escalation techniques
Presents clinical information to the agency's consulting psychiatrist
Completes clinical documentation
Participates in and attends all Community Support Team meetings for consumer caseload
Consults with and advises and actively participates with staff from the Residential and Vocational Departments regarding implementation of Behavior Management Programs
Conducts regular site assessments including the correct implementation of Behavior Management Programs and the collection of reliable data for the programs
Responds to and manages emergency clinical issues including facilitating psychiatric hospitalization when necessary
Observes consumers both in residential and vocational settings to evaluate level of functioning, behavioral issues and mental status
Provides individual and group psycho-social rehabilitation or therapy as assigned by Supervisor of the department
Completes all required documentation in a timely manner and in accordance with regulatory and licensing standards
Complies with all agency and departmental policies and procedures
Maintains professional conduct and works cooperatively with all other staff
Evaluates the effectiveness of current consumer interventions and makes appropriate changes to improve consumer functioning and independence
Attends training, in-service, and other opportunities for professional development as required
Maintains a safe environment for all consumers
Qualifications:
MA or MS in Psychology/Counseling or an MSW
One-year clinical experience under a qualified mentalhealth professional
Valid Illinois Driver's License
Pass a criminal background check and physical/drug test
