About the Position
We are looking to hire a On Call Mental Health Associate at our programs in the Lane County area, including Eugene, Springfield, and Junction City, Oregon. These position works as a member of a direct care team serving adults with severe and persistent mental illness. Primary responsibilities include supporting the development of life skills and therapeutic interventions, performing housekeeping, preparing meals, administering medication, and keeping accurate clinical documentation. In addition, you will receive crisis de-escalation training, which requires performing various physical, hands-on maneuvers and techniques.
Please keep the following things in mind:
On Call staff must work a minimum of 32 shift hours per month
On Call staff must attend a minimum of one staff meeting per month (and clinical supervision if a QMHA)
On Call staff must stay current on company training
On Call staff must respond to facility requesting coverage within 2 hours of initial call. In some cases, more notice can be provided.

Vocational Training Program
Persons with three years' experience or a bachelor's degree in a related field may qualify to register with MHACBO (Mental Health & Addiction Certification Board of Oregon) as a QMHA-R and start accruing hours towards their 1000 required supervised hours to apply for certification as a QMHA-I. ColumbiaCare offers a Vocational Training Program to accelerate the training, on-the-job experiential learning, and testing preparation for qualified employees to be able to complete their certification in as little as 6 months. This is an optional program.

$19.00 - $22.00 per hour DOE/Credentials.
An additional $2.50 per hour night shift differential will apply during the work hours of 10:00pm - 6:00am.

equivalent combination of behavioral science education in addition to satisfactory supplemental behavioral health experience and training

This includes negotiation with Oregon Health Authority (OHA) to advocate for Rate Reviews for complex cases.
* Oversee completion of LSI and LOCUS assessments for licensed providers in order to support rate setting for licensed care.
* Organize, facilitate, and/or attend person-centered interdisciplinary team (IDT) treatment planning meetings for individuals enrolled in Choice Model services in collaboration with providers and agencies involved in individuals' care.
* Authorize funding for Choice Model services and licensed care placements with supervisor guidance.

* Review requests and conduct State Hospital interviews to write and review conditions of a Trial Visit.
* Log, track, and report Trial Visit of State Hospital data to Marion County Pre-Commitment Unit.
* Monitor adherence to Trial Visit conditions for OSH discharges within Marion County. Coordinate with the court system to complete Trial Visit revocation for those who violate these conditions.
* Coordinate Transfer of Jurisdictions between counties as needed.
Assessment & Treatment Planning
* Complete intake and annual comprehensive mental health assessments, develop individualized treatment plans, develop crisis prevention plans, and direct provision of treatment services.
* Provide therapeutic intervention and intensive services to individuals, requiring advanced clinical skills.
Documentation
* Maintain accurate and timely clinical documentation in accordance with Federal, State, County, and treatment program requirements under the direction of the clinical supervisor.
* Meet performance expectations for time and billing standards.
* Compile and submit Choice Model reports to the Oregon Health Authority.

Position Purpose:
The Mental Health Counselor facilitates the coordination of care and individual support for students experiencing significant mental health challenges. The purpose of this role is to enhance the conditions and likelihood of students' personal and academic success and to contribute to a campus community that strives to be supportive and conducive to positive student mental health and overall wellbeing.
Essential Functions:
Case Management
* Consults with students, faculty, staff, and parents regarding mental health issues and the well-being of students.
* Helps manage any crisis situations.
* Responds to consultation requests from faculty and staff.
* Maintains a database of local referral resources and serves as a liaison to community mental health providers.
* Maintains a database of student records and progress notes.
* Facilitates off-campus referrals for students that need long term or specialized care.

Counseling and Crisis Intervention
* Responsible for providing individual crisis intervention counseling services to students.
* Provides consultation services to faculty and staff as well as outreach activities to enhance knowledge of mental health issues in the campus community.
* Provides case management and clinical services to students with diverse mental health concerns that include referring and bridging students to mental health services in the community.
* Provides crisis intervention, assessment, and referral for students on a scheduled or walk-in basis.
* Conduct evaluations of students' current mental health state, and make a determination of immediate next steps.
* Assists students in developing a care plan, goals, and assessing progress regularly.
* Makes contact with, follows-up with, and/or refers students who express thoughts of suicidal ideation or make threats of suicide to the appropriate resources.

Intern Coordination
* Coordinates and leads day-to-day work of counseling interns.
* Provides feedback regarding counseling sessions, case notes, and student follow-up.
* Reviews case notes before they get entered into a student file.
* Assists in providing feedback for work evaluations.
* Recommends changes in workflow and assignment of student cases.
* Shadows appointments with students and necessary follow-up.
* Develop and/or coordinate and offer training opportunities.

This position works in varied environments. International Health and Medical Services is looking for an experienced Behavioral Health Specialists to assess, diagnose, and treat mental health disorders among patients, ensuring they receive appropriate psychiatric care and support. This role includes collaborating with the healthcare team to implement treatment plans, monitor progress, and provide crisis intervention as needed, fostering mental health and well-being within the facility.
Key Responsibilities
Provides direct psychiatric care to patients via onsite and telepsychiatry services for scheduled appointments, emergency, or urgent care visit.

Basic Requirements/Certifications:

Throughout this program, you will learn the essential concepts and skills needed to function as a Community Health Worker (CHW) to become a vital bridge between healthcare, social services, and the people in your community who need them most. After completion, you will receive the credit hours necessary to apply for CHW licensure in Oregon. This program is ideal for anyone committed to helping patients and their families gain timely, individualized access to the quality health care and specialized community support services they deserve. It's designed for students of any background, even those newer to health care.
Note: This program includes both online classroom training and on-site hands-on internship/externship training.

Training Program Eligibility Requirements:
Must be 18 years or older
**Eligible participants MUST NOT have prior professional or volunteer experience in healthcare/community health work (no healthcare licenses/certifications) **
Participants must live within 30min-45min from the Service site location of Eugene, OR
Must have a high school diploma or GED
Must be a U.S. Citizen or Permanent Resident/Green Card holder (not open to non-citizens or Visa holders)
Familiarity with Microsoft Office Suite, particularly Word, Excel, and PowerPoint, is beneficial for this role
Must be able to pass a national criminal background check successfully.
Training Program Details:
This is a temporary training program lasting approximately 550 hours.
Approximately 150 hours of didactic learning and a total of 400 hours of on-site in-person internship
This training program requires the learners to participate Mon-Fri,8 AM-5 PM for the entire duration of the program.
Total Stipend: $7,500. This stipend is distributed over approximately 16 weeks for participation in both learning and hands-on internship training at a partner organization. Stipend payment is made on OCHIN's semi-monthly pay schedule.

CHW Training Program Description: Community health workers (CHWs) play a vital and unique role in linking diverse and underserved populations to health and social service systems. Community Health Workers will primarily be working out in the community with specific target populations. CHW's will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. Community health workers (CHWs) have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Community health workers also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. The CHW duties include, but are not limited to:
Establish trusting relationships with patients and their families while providing support, encouragement, and feedback.
Coach patients in effective management of their chronic health conditions and self-care.
Convey the purposes and services of a program to the user population and the impact the program or service would have.
Assist patients in understanding care plans and instructions.
May need to visit patients in their homes to assess the patient, their living conditions, and meet with family members or caretakers.
Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
Advocate for patient and act as a liaison between the patient/family and community service agencies (I.e., schools, hospitals, support groups, etc.).
Help patients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
Assist patients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.
Facilitate communication and coordinate services between providers.
Motivate patients to be active, engaged participants in their health.
Effectively work with people (staff, clients, doctors, agencies, etc) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.
Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff.
Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations.

Requests for exemptions based on medical or religious grounds will be reviewed and must comply with applicable laws and site-specific policies. Essential Responsibilities:
Clinician Care Managers facilitate the achievement of client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation.

Interact with other PacificSource personnel to assure quality customer service is provided. Act as an internal resource by answering questions requiring medical or contract interpretation that are referred from other departments, as well as physicians and providers of medical services and supplies. Assist employers and agents with questions regarding healthcare resources and procedures for their employees and clients.
Practice and model effective communication skills: both written and verbal.
Utilize and promote use of evidence-based tools.
Utilize lean methodologies for continuous improvement.

SUCCESS PROFILE
Work Experience: Minimum of three (3) years of clinical experience, including case management. Insurance industry experience preferred.
Education, Certificates, Licenses: Clinical Social Worker, Licensed Professional Counselor, or Licensed Independent Clinical Social Worker with unrestricted license required in current state of residence. OR Registered Nurse with current appropriate unrestricted state license based on line of business: Commercial and Medicare: Oregon, Idaho or Montana, and Washington; Medicaid and DSNP: Oregon. Certified Case Manager Certification (CCM) as accredited by CCMC (The Commission for Case Management) strongly desired at time of hire. CCM certification required within two years of hire.
Knowledge: Knowledge of health insurance and state mandated benefits. Experience and expertise in case management practice including advocacy, assessment, planning, communication, education, resource management and service facilitation. Ability to deal effectively with people who have various health issues and concerns. Knowledge and understanding of contractual benefits and options available outside contractual benefits. Knowledge of community services, providers, vendors and facilities available to assist members. Ability to use computerized systems for data recording and retrieval. Assures patient confidentiality, privacy, and health records security. Establishes and maintains relationships with community services and providers.
