The LPHA Interventionist is primarily responsible for monitoring services and providing intervention to individuals with a positive screening for various measures, especially those centralized around DSRIP. Such interventions include, but are not limited to, tobacco cessation, alcohol, depression, and/or suicidal ideation. The LPHA Interventionist uses a basic knowledge of diagnostic characteristics, symptomology of primary mental illness and knowledge of medications, side effects and benefits to provide services in a person-centered, supportive, office-based environment. This position is monitored by periodic supervision. Responds to crisis or potential crisis situations and ensures the appropriate level of care through the coordination of services or the provision of community support services. Documents services in the automated clinical record keeping system with 24 hours after services have occurred. Provides overall service coordination, referral, linkage, and advocacy to consumers with varying needs. Performs duties in concert with other members of an interdisciplinary treatment team Participates in interdisciplinary team staffing, presents psychosocial assessment findings and encourages consumer to attend staffing.
