Practices principles of professional development and performance improvements by maintaining clinical knowledge, using best and or evidence-based practices, and serving as a Clinical instructor. Practices principles of employee and patient safety in daily job activities. Summary:
Children's Health Specialty Centers for outpatient therapy offer a comprehensive range of pediatric therapy services aimed at supporting children's physical, developmental, and emotional needs. Our services include care coordination, developmental screenings, occupational therapy (OT), physical therapy (PT), speech/language therapy, parent education and support, and specialized treatment programs. We provide tailored services such as transitional skills evaluations for children ages 11-17, focusing on developing independence in self-care, work skills, and functional abilities.
In addition, we offer specialized assessments, including augmentative communication evaluations and feeding therapy for children with neurological and behavioral disorders. Our therapy programs also address handwriting disorders, sensory integration, and neuromuscular electrical stimulation, among others. Group classes are available to enhance patients' abilities in areas like articulation, feeding, motor skills and social needs.
Responsibilities:
Provide direct Physical Therapy evaluations and re-evaluations, including assessment of prior and current level of function, setting individualized goals and making discharge recommendations in accordance with standards of practice; provide direct PT treatments according to standards of practice, including patient and family education, progression of treatment plan and implementation of appropriate therapeutic interventions; report on patient progress through appropriate written documentation including evaluation forms, progress notes, education forms; and collaborate with physicians, patients, family and other members of the health care team relative to patient treatment and progress as well as other affiliated agencies with regard to effective discharge planning

The QMHP-Adult Intensive position provides psychosocial/rehabilitative service to adults with serious mental illness. Services may include, but are not limited to, assessment, development of measurable treatment goals and objectives, engagement into treatment, referral, linkage, advocacy, monitoring, crisis intervention and continuity of care. The overall goal of this position is to maximize the individuals potential level of functioning, reduce hospitalization and aid in the successful reintegration of individuals into the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide overall service coordination, psychosocial rehabilitation, emergency services/assessment, referral, transportation, linkage, and advocacy to individuals with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team.
Ensure authorization for clinical services.
Formulate individual recovery plan based on assessment findings on all new admissions.
Develop measurable objectives and goals agreed upon by the individual.
Actively assist in obtaining and modifying goals as needed.
Document all attempts to involve individual, service providers, and caregivers (unless opposed by the individual) in service plan process.
Review service plans every 180 days or as clinically indicated.

Follow-up with assigned individuals who miss a scheduled clinic appointment. Document attempts to contact individual. Report repeated unsuccessful efforts to contact the treatment team.
Oversees and assists assigned individuals medication adherence. Provides med training, arranges transportation to scheduled office based appointments, and prompts needed refills.
Document clinical services by close of next business day.
Perform follow up hospital assessments the same day as requested by the hospital and report assessment outcome to the Clinical Manager.
Provides crisis intervention/on-call services during the work week and by weekend rotation. Make home and hospital visits, initiates mental illness warrants, work with police and other public servants as a needed to address crisis for assigned clients.
Facilitate inpatient admission upon request of the treatment team and/or hospital. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning with hospital. Meet with the individual within 2 days of discharge in the community.
Evaluate progress of clinical session, solicit feedback from individual(s), and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues.
Co-facilitate family education workshops.
Identify and assist individuals in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.

Education, Training, and Experience

This position will be responsible for overseeing the timely completion of the Texas Resilience and Recovery (TRR) assessments, Person Centered Recovery Plans (PCRP), annual consents, and various other documentation and assessment tools needed for enrollment in behavioral health services. This position will require ability to work outside of normal business hours in office or community-based settings in order to meet individual and family needs and preferences. This position will monitor upcoming expiration dates for those seeing clinic prescribers, med distribution and pickup, as well as support intake for completion of initial recovery plan development. The Assessment Specialist will work collaboratively with program leadership in both Youth and Family Services and Adult Services to meet the unique needs of those served.
This Assessment Specialist exemplifies service excellence and will be available as needed to support the case management team meet the needs of individuals in services through seeing individuals in crisis, taking walk-in appointments, filling in for case managers, or updates needed prior to prescriber appointments. Is responsible for initiating and collaboratively completing TRR and PCRP prior to expiration and as needed based on individual change. Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record.
This Assessment Specialist practices good stewardship of resources by ensuring all required trainings are completed prior to due date and center-issued equipment is kept in good repair. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, Medicaid billing rules, contracts, grants, and Lifepath Systems policies and procedures.
This Assessment Specialist builds positive relationships with community-facing agencies and always maintains a professional relationship with all community stakeholders and partners. Professional conduct with Peers, Leadership and Support teams is expected at all times. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with improving access to social determinants of health.
Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and community partners; to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve. Ability to assess and de-escalate behavioral health crisis situations in calm and respectful manner.
Specific Targets: At minimum, complete 3 TRR/PCRP and/or crisis assessments a day and 150 contact attempts per month. Expired TRR/PCRP and autodischarges will be monitored for decrease month-over-month. 100% of TRR/PCRP development will result in subsequent service provided within 2 business days. 90% of service notes are signed within 2 business days of service. 75% of full chart reviews receive a score of 80% or higher. All required trainings are completed prior to due date.

Bachelor's degree in psychology, social work, or related field from a four-year college or university.
At least two years of related experience and/or training in the field of mentalhealth preferred.

This position will provide telephonic crisis assessment with the City of Dallas 911 call center dispatch. The role of the behavioral health clinician is to screen and assess for risk harm, to identify the level of priority for the call. The clinician will work in conjunction with the City of Dallas staff and Dallas Police Department and assist in communicating, prioritizing, and coordinating care as deemed clinically appropriate at the time of the call.
HOURS OF RESPONSIBILITY: Shift 1-7am-3:00pm and Shift 2- 2:00pm-10:00pm
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide crisis assessment services in a variety of modalities.
Behavioral Health Clinician will screen/assess the crisis, provide an evidence-based intervention, and stabilize the crisis. The caller is then linked with service coordination to assess the level of further stabilization needs and appropriateness for other services in the crisis continuum.
Develop person-centered crisis plan and assist the individual in executing the plan during and post crisis intervention.
Ensure continuity of care post crisis intervention to appropriate outpatient services, including but not limited to, outpatient mentalhealth, substance abuse services, and/or primary care services.
Be accessible for referrals and communication through our electronic health applications, by phone and email to provide a timely and comprehensive response.
Interacting with internal and external partners, including but not limited to City of Dallas personnel, dispatch team, supervisor(s), Right Care Team and others, to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services.
Develop close working relationships with internal and external referral resources.
Maintain confidentiality of information concerning individuals and family members
Follow all relevant City of Dallas procedures.
Interacting with internal and external partners, included but not limited to City of Dallas personnel, Dallas Police personnel, Integrated Public Safety Response Teams - RIGHT Care Unit and Crisis Intervention, Dallas Fire Rescue personnel and other city departments to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services.

QUALIFICATIONS
EDUCATION AND EXPERIENCE:
Required: Master's degrees with advanced licensure for independent practice, such as LCSW, LMFT, or LPC.

Preferred: Bilingual English/Spanish.
Preferred: Experience with trauma-informed services; Seeking Safety, Illness Management and Recovery, and Motivational Interviewing.
