Job Description: KEY ACCOUNTABILITIES: * Assesses, diagnoses when necessary, and evaluates patient's status and the effectiveness of mental health care using tools deemed appropriate by clinical setting, such as the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), to create an individualized plan of care that includes cultural diversity and special needs appropriate to the patient population served. * Continually assesses patient's progress and provides interventions and strategies to support the mental health diagnosis. * Prepares and maintains all required treatment records, reports, and other clinical information on time and accurately using current technology in accordance with regulatory requirements and department procedures. * Works collaboratively with the patient and others to develop a comprehensive treatment plan that addresses the needs of the patient, desired outcomes, and agreed upon plan of care. * Develops treatment plans and objectives in collaboration with patient and family, as appropriate, based on patient needs/goals. Anticipates and intervenes in areas of patient safety and symptom reduction. Is able to work in a team environment, remaining attentive to ideas, recognizing responsibilities, and actively participating with others to accomplish assignments and achieve desired goals. * Coordinates referral options and serves as a link between patients and treatment providers. Establishes and maintains effective working relationships with other clinicians or other referral agencies * Provides education and mentoring for students, interns, residents, observers and volunteers. * With direction from your clinical supervisor, you may be asked to provide peer-to-peer review, clinical collaboration, chart auditing, coaching and mentoring in an effort to support team member development.
