JOB SUMMARY: This position reports to the Director of Behavioral Health. The Mental Health Counselor is responsible for providing quality and appropriate clinical services to the patients of Franklin Primary Health Center, Inc. ESSENTIAL FUNCTIONS: Responsible for completing Mental Health Assessments for Ryan White and Healthcare for the Homeless in order to establish patient needs for services. Provides individual group and/or family counseling to patients and families affected by HIV/AIDS. Provides treatment options, bereavement counseling, crisis intervention and emergency services. Works with patients to develop bio-psycho-social, treatment plans, discharge plans, and follow-up care programs. Provides case management for patients, when appropriate. Refers patients to other social services for financial assistance, legal services, health care services and other required services. Establishes and maintains clinical charts, referrals, and other related documents for the treatment of FPHC patients; prepares related documentation; keeps abreast of patient data to ensure appropriate treatment and care is delivered. Works in collaboration with other program staff, and local community based organizations, to coordinate services in order to avoid duplication of services. Responsible for providing Crisis Counseling as needed and Screening/ Intake/Referral for the Level One Outpatient Substance Abuse Program Assists in preparing monthly, quarterly and final reports for funding sources and administration.
