Mental Health Therapist Job In Urban Honolulu, HI
Serves as an integral member of a multidisciplinary inpatient care team. Provides comprehensive psychosocial assessments, coordination, and implementation of age appropriate social medicine services to meet the needs of patients/families/significant others in the hospital setting. The social worker encourages respect for individuality and patient choice, while collaborating on policies and systems that respect the cultural, religious, and ethnic difference among patients and families.
Essential Responsibilities:
Participates as a member of the inpatient multidisciplinary care team providing social medicine expertise in response to care team referrals and case finding of high need populations (e.g. new diagnosis, terminal illness, loss of function, lack of social support, grief and bereavement, homeless, complex finances, etc.).
Completes psychosocial assessments of hospitalized patients. Identifies psychosocial needs, develops, and implements plan of care. Documents according to departmental standards, Federal and State regulations. Assures medical record documentation of patient interaction and related tasks performed in a timely manner.
Upon referral or case finding, assists patients and their support system (e.g. friends, family) with coping strategies related to illness and hospitalization.
Conducts social medicine consultation and assistance for cases invloving abuse, neglect, advance care planning, and legal decision making (e.g guardianship).
Provides information and referrals to community resources for post discharge support.
Applies knowledge base, theoretical concepts, specific functional skills, and essential social values used to effect change in human behavior, emotional responses, and social conditions.
Contributes to the patient discharge planning process by assessing patient and support systems choices and educating them on the risks and benefits of their choices to facilitate discharge.
Assists with coordination of discharge plan and assures patients placement in appropriate community based settings from hospital.
Provides information, resource identification and referral services, mediation services, advocacy services, and education to patients and their support system.
Plans for and participates in development activities to keep abreast of current trends in medical social work. Participates in performance improvement. Implements Quality Improvement processes. Assists with development and implementation of departments annual work-plan. Performs clinical record audits. Maintains educational record and provides evidence of competency. Participates in departmental functions such as staff meetings, staff in-services, and committees, as assigned. Participates in orientation of new personnel.
Basic Qualifications: Experience
Minimum one (1) year experience in health care setting.
Education
Masters degree in social work.
License, Certification, Registration
Licensed Clinical Social Worker (Hawaii) OR Licensed Social Worker (Hawaii)
Basic Life Support within 0.5 months of hire from American Heart Association
Additional Requirements:
Experience with interdisciplinary team function.
Knowledge of medical terminology, the psychosocial impact of serious illnesses.
Demonstrated ability to interrelate with physicians, nurses, support staff, and patients.
Familiar with care processes and systems in settings internal and external to Kaiser Permanente.
Knowledge of the principles of patient teaching and delegation, care planning, and appropriate utilization of acute hospital, long term care, and home care resources.
Demonstrates customer-focused service skills.
Knowledge of community resources, family dynamics, and the dying process.
Keyboarding or typing skills.

SUMMARY OF DUTIES
The Mental Health Professional (MHP) provides services for individuals and their families in the home, school, and community, which are designed to, (1) ensure safety and stability of the child to promote successful functioning in the home, school and community; (2) avert treatment in a residential or inpatient setting; (3) facilitate an earlier return of individuals receiving inpatient or residential care; (4) strengthen family functioning; and (5) assist students in achieving optimal benefits from their education. EDUCATION AND TRAINING REQUIREMENTS
Masters' Degree from a school accredited by a recognized accrediting agency.
Also must meet one of the following sets of credentials:
A physician in training in an ACGME (Accreditation Council on Graduate Medical Education) accredited residency program in child and adolescent psychiatry under program faculty supervision,
Ph.D. or Psy.D. in Clinical or School Psychology from a nationally accredited university,
A Hawaii licensed Social Worker (LSW),
Master's degree from a nationally accredited university as a national board certified behavior analyst, marriage and family therapist, mental health counselor, psychologist, social worker, school psychologist, or psychiatric nurse,
Ph.D. or Psy.D. student in clinical psychology studying in an accredited program under program faculty supervision.
Education may be substituted with certificates in a specialty such as Certified Substance Abuse Counselors (CSAC) OR registered Professional Nurses certified in psychiatric nursing.
EXPERIENCE
Must be supervised by a QMHP and have a minimum of one year full time clinically supervised progressive work experience inclusive of residency, internship or practicum in the care or treatment of youth in a mental health or educational setting (experience may be substituted with certificates in a specialty such as Certified Substance Abuse Counselors (CSAC).

Examples of Duties
* Oversee and monitor the work performance of behavioral health specialists, school social workers, and other assigned staff regarding the provision of school-based behavioral health services within a complex or district.
* Responsible for evaluating the work performance of assigned staff and completion of their performance appraisal (PAS) within the required timeframe.
* Monitor activities and services of staff for conformance to existing laws, policies, procedures, standards, and guidelines, etc. Take corrective action/steps to remedy non-conformance in order to comply with legal requirements.
* Ensure that appropriate services are delivered to students by program staff as specified in student Individual Education Plans/504 Modification Plans (IEP/MP). * Assist the district educational specialist (DES) with program development and the overall plan for the implementation of the district's/complex's school-based behavioral health services program including an evaluation strategy to effectively monitor and utilize behavioral health and the contracted provider resources.
* Administer policies and procedures pertaining to the implementation and operation of school-based behavioral health services.
* Conduct budget surveillance and utilization procurement review for the SBBH DES to assist in identification of resources needed to implement a system of school-based behavioral health services.
* Review requests for contracted services and monitors for appropriate utilization of services and resources in conjunction with the DES and district contracts specialist.
* In consultation with personnel from the DOE district and State level offices, the Department of Health Family Guidance Center, and Children's Community Council (CCC), develop a community-based system of care.

Basic Requirements/Certifications:
