How you will make a difference: The Mental Health Specialist assesses clients at start of treatment (reviews as needed), develops and maintains treatment plans, and conducts family collateral and discharge planning. Duties include discussing goals and objectives of proposed treatment plans and establishes a contract with the consumer. Interviewing consumers to team with mental health professionals in determining the needs of individuals or groups on a walk-in/telephone basis; respond to potential suicide or other emergency events. Visits clients, individually or as part of a team, in a variety of living situations, to help assess the mental health needs of individuals and families, observe progress, provide counseling, gather data, and report significant changes in the client's condition to designated professional staff. Division/Program Overview: Valley Star's Full Service Partnership is for adults ages 18 to 59 and provides mental health services and linkage support to community services. Services are provided in the office, in-home or in community locations. Clients served are from Tier III service needs.
