Our continuum of care ranges from prevention and early intervention to intensive outpatient services for those with more serious and chronic conditions, including Full-Service Partnership (FSP) services. Staff provide services at our Prototypes sites and in the community. These services can include individual and group services, medication support, case management, and coordination with other healthcare services. The Case Manager I delivers client services by coordinating care within the organization's continuum of services and with external human services providers.
