Under the supervision of the Center Manager, this position is responsible for the delivery of clinical services and community supports to persons with severe and persistent mental illness, as defined by the state authority, The Department of State Health Services (DSHS). Services are to be provided according to the Texas Resiliency and Recovery Model, and must conform to the Performance Contract, Community Service Standards, and Helen Farabee Center policy and procedures, and Medicaid standards.
Responsibilities include, but are not limited to:
The incumbent assists consumers in making informed choices educating them on community resources and assist with accessing services. The incumbent schedules Adult Needs and Strengths Assessments (ANSA) and updates treatment plans in accordance with consumer need. This position will be required to serve on-call after regular business hours, for which compensation is provided. The incumbent must provide face-to-face assessment and crisis intervention services to any person requesting help or presenting a crisis at any time. Teamwork, initiative and positive example in work habits are always expected. Provides consultation and support to fellow staff as needed. This position requires an
aptitude
for working with severely mentally ill, often impoverished people; and for seeing this work as a clinical specialty in which we take great pride. The incumbent should have knowledge and skills in assessment, diagnostics, treatment planning, and community support services for children, adults, and geriatric persons with mental illness, as well as knowledge of psychotherapeutic medications and basic casework skills. This position aids keeping unassigned caseloads up to date and/or assistance to caseworkers with assigned caseloads as needed. The incumbent is accountable to the Center Manager. HFC incorporates principles of trauma-informed care, which includes cultural sensitivity, in all areas of service delivery. All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position. Reassignment is a condition of employment that may occur at the employer's discretion. Your work location and hours could change based on program needs.
Qualifications
Education, Training, and Experience
Bachelor's degree from accredited college or university with major in social, behavioral or human services field. Licensure is not required. Experience with Severe and Persistent Mental illness is preferred. Must have valid driver's license and be insurable to drive Center vehicles. Position may require use of personal vehicle in performance of job-related duties and proof of PIP insurance. Must pass all Center mandated trainings.
Knowledge, Skills, and Abilities
Ability to read and proof correspondence, reports and computer printouts manually, etc.
Good spelling and grammar skills.
Ability to write progress notes according to standard, assessments, brief mental status exams, memos, letters, various complex reports and miscellaneous other documents with acceptable spelling and grammar.
Ability to relate verbal instructions, communicate in a courteous and effective fashion with co-workers, consumers and families, agencies and the public.
Display a customer service orientation; always treating customers as equals and partners.
Skilled in the use of personal and network computers, word processing, database, spreadsheet and office management software (Microsoft Office), SmartCare, familiarity with CARE.

Typical Functions:
Provides initial and ongoing mentalhealth screenings and evaluations identifying at-risk and vulnerable inmates needing prevention, intervention, referral, and the immediate provision of mentalhealth services according to policy and procedures. Assigns mentalhealth diagnoses to inmates as appropriate.
Provides crisis, suicide and harm-reduction intervention/procedures to inmates identified as high-risk for self-harm, harm to others and/or have the potential for violence.
Identifies, monitors, and provides mentalhealth services to inmates with diagnosable mentalhealth, behavioral, or emotional disorder to facilitate and support their wellness and recovery.

Coordinates with other medical and mentalhealth professionals to ensure quality and continuity of care.
Schedules, monitors, and may participate in appointments with internal and external mentalhealth professionals and providers to ensure medication compliance. Assists with providing medication education to and management reviews with inmates.
Develops and implements an individualized mentalhealth continuity of care/aftercare plan to inmates discharging to prevent decompensation and provides referrals and linkage to community resources.
Assists in developing and carrying out comprehensive, culturally attuned training programs for the recognition and handling of behavioral and psychological problems most common in a correctional setting involving difficult-to-manage inmates and the seriously mentally ill.
Assists in conducting psychological, social, and personality adjustments and suicide prevention in-service training for the correctional and other professional staff members in the recognition, prevention, and handling of emergent crisis situations.
Attends professional conferences and keeps abreast of the most recent research, trends, and therapeutic modalities in the field of mentalhealth including ethics training necessary for the improvement of quality of care and systems of service delivery.
Participates as a member of the assigned facility's interdisciplinary team in meetings, correctional conferences, and other related facility and state-wide trainings.
Conducts assessments for inmates placed in restrictive housing according to policy and procedures.
May be required to respond on an on-call basis to crisis situations.

Education and Experience:
Education and experience requirements at this level consist of a master's degree in psychology, counseling, human relations, clinical/direct practice social work, or a closely related field which must have included one semester of at least 300 hours of applied clinical practicum in psychology or counseling. Must hold a current license as a Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Behavior Practitioner (LBP), Licensed Clinical Social Worker (LCSW), or Licensed Alcohol and Drug Counselor with MentalHealth (LADC-MH).
Preferred: One-year post-master's experience providing professional counseling services.

Your Role:
Deliver personalized, evidence-based therapy to patients with orthopedic, neurological, post-surgical, and sports-related conditions
Document care accurately in Raintree EMR
Maintain a caseload of approximately 50 visits per week with the support of AI-driven tools that help streamline and reduce documentation time
Contribute to a culture of clinical learning and peer support
Requirements
Degree from a CAPTE-accredited Physical Therapy program
Active state Physical Therapy license or eligibility for licensure
Strong communication, interpersonal, and clinical reasoning skills
