The Licensed Professional MentalHealth Counselor (LPMHC) serves as a member of the Behavioral Health Interdisciplinary Program (BHIP). BHIP is a multi-disciplinary team of mentalhealth professionals which provides comprehensive, accessible, Veteran-centered mentalhealth care. The BHIP MentalHealth Treatment Coordinator (MHTC) will be a member of all BHIP teams within the clinic and is responsible for various aspects of assessing, diagnosing, treating, and coordinating the treatment of Veterans within BHIP and the MentalHealth Service Line. BHIP MentalHealth Treatment Coordinator (MHTC) is responsible for various aspects of assessing, diagnosis, treating, and coordinating the treatment of Veterans within BHIP and the MentalHealth Service Line. This includes, but is not limited to, providing services through the telehealth modality across several sites of care with the VA Texas Valley Coastal Bend Health Care System (VATVCBHCS). The LPMHC serves on committees, work groups, and task forces at the facility, VISN and national level, or in the community as deemed appropriate by the supervisor, and will work closely with the BHIP Supervisors and the MentalHealth Service Line leadership on identifying and addressing process issues within BHIP and/or the MHSL. This assignment is to be relatively few in numbers based on the size of the facility/service and applying sound position management.
The Licensed Professional MentalHealth Counselor (LPMHC):
* provides a full range of psycho-diagnostic services which requires advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of Veterans with a range DSM diagnoses, substance abuse/dependence, and poly-traumatic injuries.
* serves as the primary licensed independent clinician to meet the same day access, and crisis intervention needs of Veterans requesting mentalhealth care. This includes conducting the same day evaluation through telehealth modalities, in-person, or by telephone, according to policy. Given that a significant portion of these Veterans are new to MentalHealth treatment, this involves providing a diagnosis and provisional treatment plan.
* provides initial and ongoing assessment of needs and completes comprehensive psychological intake evaluations for coverage or mentalhealth updates, including diagnosis, to Veterans in BHIP. In addition, comprehensive chart reviews, completion of consults, referrals, and psychoeducation related to resources for self-management, whole health, and linkages to the community available for Veterans and families. Serves as advocate for Veterans and serves an ongoing point of contact and clinical resource for Veterans receiving mentalhealth services.
* ensures mentalhealth treatment for Veterans within BHIP and the care continuity between levels of care. Has the primary responsibility of MH Care Coordination on the team, across the continuum of specialty MH care, and participates in care planning to ensure integration of care coordination within the overall care plan.
* conducts risk evaluation, treatment, care coordination, or referrals for Veterans within BHIP, including those who are in the COMPACT Act, Post discharge engagement, REACHVet program, and those with a High Risk for Suicide Patient Record Flag. Collaborates with Suicide Prevention, other care coordinators/ managers and MentalHealth providers to identify Veterans at risk for suicide and others with complex needs to increase monitoring or enhance care with targeted suicide prevention strategies as indicated.
* monitors clinic availability, dashboards, and panel size of all BHIP team members in order to be able to coordinate the flow of patient care
amongst BHIP teams and BHIP team members. This includes knowledge of availability of specialty mentalhealth services, as well as working actively with program support staff to ensure that team assignments in the Patient Care Management Module (PCMM) are accurate and up to date.

Job Summary
Provides support for care transition activities. Facilitates transitional care processes and coordination for member discharge from hospital admission to all other settings. * Works with participating ancillary providers, public agencies or other service providers to make sure necessary services and equipment are in place for safe transition.
* Conducts face-to-face visits of all members while in the hospital and, home visits high-risk members post-discharge as needed.
* Coordinates care and reassesses member needs using the Coleman Care Transition model post-discharge.
* Educates and supports member focusing on seven primary areas (Transition of Care Pillars): medication management, use of personal health record, follow-up care, signs and symptoms of worsening condition, nutrition, functional needs and or home and community-based services, and advance directives.
* Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts.
* Assesses for barriers to care, provides care coordination and assistance to member to address concerns.
* Facilitates interdisciplinary care team meetings (ICT) and collaboration.
* Provides consultation, recommendations and education as appropriate to non-behavioral health care managers.
* 40-50% local travel may be required (based upon state/contractual requirements).
Required Qualifications
* At least 2 years experience in health care, with at least 1 year of experience in hospital discharge planning, care management or behavioral health setting, or equivalent combination of relevant education and experience.
* Licensed behavioral health clinician to include: Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC), License must be active and unrestricted in state of practice.

- Ensures safe and appropriate transitions by collaborating with the hospital discharge planner, as well as collaborating with hospitalists, outpatient providers, facility staff, and family/support network.
- Ensures member transitions to setting with adequate caregiving and functional support, as well as medical and medication oversight support.
- Works with participating ancillary providers, public agencies or other service providers to make sure necessary services and equipment are in place for safe transition.
- Conducts face-to-face visits of all members while in the hospital and, home visits high-risk members post-discharge as needed.
- Coordinates care and reassesses member needs using the Coleman Care Transition model post-discharge.
- Educates and supports member focusing on seven primary areas (Transition of Care Pillars): medication management, use of personal health record, follow-up care, signs and symptoms of worsening condition, nutrition, functional needs and or home and community-based services, and advance directives.
- Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts.
- Assesses for barriers to care, provides care coordination and assistance to member to address concerns.
- Facilitates interdisciplinary care team meetings (ICT) and collaboration.
- Provides consultation, recommendations and education as appropriate to non-behavioral health care managers.

The BHIP Psychologist, is administratively within Psychology Section within MentalHealth Service Line and functions as part of a multidisciplinary general mentalhealth team, diagnoses and treats mental disorders, conducts psychological and cognitive assessments, and provides consultation to other providers regarding mental disorders. The core of the incumbent's duties is general service delivery for a wide range of mental and emotional concerns, especially treatment of, and consultation on, disorders considered to be serious mental illnesses (e.g., mood disorders, personality disorders, schizophrenia spectrum disorders); whereas metal disorders of mood, anxiety, and trauma-adjustment of mild to moderate severity, can often be treated with Primary Care MentalHealth Integration (PCMHI). We are growing in the form of teams composed of multiple therapists and a prescriber. With onboarding, you meet multiple eligible psychotherapists looking for additions to their team and a match is made.
We emphasize regular staffing with your collaborating therapists, at least monthly, to discuss patient cases or clinical topics.
Clinical work is performed from home, days and hours are per the clinician's choosing (this includes mornings, afternoons, evenings, weekends, holidays, etc.). Clinical team meetings are also virtual.
Clinical conferences are held four times a week. It is required that you attend at least one per week for the first 90 days of employment; after that, it is required that you attend one monthly.
Job Requirements:
Must have a PMHNP license and open to licensing in alternative states.

ICMHR-RANGE is a complex program with a current maximum capacity of 110 Veterans with severe functional impairments who have chronic severe and persistent mental illness.
