Position Summary:
The Outpatient Physical Therapist administers Physical Therapy services under medical prescription in accordance with Hospital regulation, State of Connecticut Practice Act and quality standards. Responsible for clinical oversight of PTA
Key duties and Responsibilities:
Develops a plan of care for each patient with the patient's input and agreement, which includes safety, educational, psychological and developmental needs of the patient.
Interprets and assesses data about the patient as evidenced by appropriate goal setting and revision of goals throughout the plan care while explaining treatment goals, plans, techniques and outcomes to patient and or family.
Displays expertise in recommending various therapeutic procedures and modalities.
Complies with professional and departmental documentation standards.
Addresses a home exercise program with each patient
Maintains an adequate caseload, adhering to departmental productivity standards.
Attends continuing education seminars as required by licensure and schedules and presents an educational program, including tests, to Rehab staff or other medical professionals.
Communicates regularly with Rehabilitation Supervisor, other team members and referring physicians for patient care.

Minimum Requirements:
• Graduate of an accredited School of Physical Therapy
• Licensed or eligible for licensure to practice in the State of Connecticut

Working within Adult Services, the Case Manager 2 works on the Residential Behavioral Health Treatment care team to support clients' recovery from mental illness, substance use, and homelessness.
Requirements:
High School diploma or GED
3 years related experience in the direct service and case management field
The ideal candidate will have experience working with the SPMI population in a residential setting and with individuals who are re-entering the community after a period of incarceration.

Provide clinical services that are strength based and client centered to children and their families.
Coordinate with other clinical and social service providers, care coordinators, and patient navigators
Document all aspects of clinical care in the EHR and in a timely manner
Participate in professional activities such as: continuing education activities, clinical conferences, meetings with representatives of community groups when required to do so
Ensure patient confidentiality at all times
Perform other necessary duties as required by FHCHC to achieve the goal of providing high quality health care
Qualifications
Master's degree (M.A., M.F.T., M.S.W.) required. Must have an independent license in the state of CT i.e., LMFT, LPC, LCSW, or an associate license LPCA, MFTA, LMSW or Psychologist (PhD or PsyD). One year of behavioral health experience, and oral and written fluency in both Spanish and English required.
The successful candidate will have:
Experience working with children, adolescents, and families impacted by ASD or other developmental disabilities is preferred.
Strong understanding of autism and its impact on development, as well as the ability to adapt therapy methods to individual needs.
Training, knowledge and ability to independently carry out all the above duties
Experience in providing clinical services to children and adolescents
Strong knowledge base of medical terminology/procedures for work in an integrated healthcare clinic desired
Previous experience in health care setting, ideally in an integrated outpatient setting desired
Ability to work in a high energy, fast paced environment and maintain a professional demeanor and attitude
Excellent interpersonal and communication skills with patients, their families, internal and external staff

We are currently seeking a full-time licensed or license-eligible bilingual (Spanish/English) Clinician to provide multi-faceted client centered services to children, adolescents and their families with behavioral health needs. The ideal candidate should be comfortable counseling individuals, families, couples or groups regarding psychological, behavioral or emotional problems by developing and implementing clinical assessment and clinical treatment plans. This position will split time between our Norwich and New London BHS facilities.
Essential Responsibilities -
Provide clinical assessment and treatment client centered care consistent with quality standards of care including evidence based practices that result in improved health outcomes for clients.
Assess level of risk and appropriate level of care with routine and emergency clients of all ages who may present in crisis with complex and long term mentalhealth and/or substance abuse issues.
Provide on-call support on site during hours of operation for connected and non-connected clients. Includes, but is not exclusive to: arranging for referrals to other levels of care or emergency response, reporting suspected child abuse/neglect to DCF, adhering to legal and ethical guidelines for standards of care.
Participate in clinical supervision, and maintain open lines of communication with supervisor and support staff, and coordinate treatment with psychiatrists; work actively toward obtaining professional licensure
Coordinate services with relevant providers involved in a given case within and outside of UCFS (e.g. school personnel, Department of Children and Families, courts, etc.) and support continued collaboration with other providers.

UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are currently seeking a full-time (40hr) Clinician to join our growing team to provide treatment services to adult parents or caregivers of young children to address substance use and mentalhealth issues that could impact their parenting. Along with their team members, clinicians will be fully trained to provide families the Multidimensional Family Therapy and Recovery (MDFTR) model. MDFTR is an evidence-based approach that seeks to eliminate barriers to accessing treatment by providing family therapy to clients in the clinic, in their homes, or in the community. The MDFTR model uses a team approach in working with families. Clinicians work under the direction of a supervisor and are paired with a therapist assistant to deliver MDFTR to up to six (6) families at a time. This small caseload allows the MDFTR team to work focused and intensively with families to address their needs.
Clinicians will be paid a $6,000 sign-on bonus after successful completion of 90 day introductory period!
Essential Responsibilities:
Conduct clinical assessments to evaluate and diagnose mental, emotional and behavioral disorders in adults and families
Develop treatment plans and treatment plan reviews based on assessments
Provide weekly MDFTR sessions to clients
Maintain line of communication with supervisor, therapist assistant, and other support staff, including coordinating care with psychiatrists
Maintain collaborative relationship with family members and other service providers
Complete training in the UCFS electronic health record system, EPIC.
Complete all required documentation within a timely manner.
Participate in required training to become certified to deliver MDFTR. These trainings are at no cost to you and may provide continuing education credits toward your licensure.
Respond to crises by phone, assess need and implement appropriate response.
Rotate after-hours on-call with other program staff;
