The Mental Health Paraprofessional provides direct care and supportive services to residents living in a structured mental health residential facility. Under the supervision of licensed clinical staff, this role assists individuals with mental health conditions and/or developmental disabilities in building life skills, achieving treatment goals, and maintaining a safe, therapeutic living environment.
Key Responsibilities:
Support residents with daily living activities, including hygiene, meal preparation, medication prompts, and room upkeep.
Assist residents in developing coping skills, interpersonal communication, and healthy routines.
Observe, monitor, and document resident behaviors and report significant changes to clinical staff.
Participate in the implementation of individual service or treatment plans under clinical supervision.

Qualifications:

Transportation Responsibilities:
Provide transportation for consumers as needed using agency or personal vehicles.

Follow all agency transportation policies, including seatbelt usage, safe cellphone practices, and vehicle documentation.
Immediately report any vehicle issues, accidents, or violations to appropriate personnel.

JOB DUTIES
1. Monitors level of prescription medication available to clients

10. Coordinates laboratory testing services as follows: (a) identify clients in need of laboratory testing, (b) prepare laboratory slips to request testing services, (c) receive and evaluate laboratory testing results, (d) document results accordingly.
11. Performs Pre-Admission Screening functions.
12. Performs the admission and discharge nursing assessment on assigned patients, including appropriate and timely charting and filing.
13. Conducts and records health assessment screening on all admissions.
14. Participates in multi-disciplinary staffing and clinical team meetings.
15. Provides consultation to staff and clients about medical questions and/or concerns and assist staff in planning and implementing appropriate medical interventions.
16. Accompanies staff to physician's appointments for program participants who are identified as being high-risk behaviorally and/or medically.
17. Monitors client vital signs during weekly rounds.

Job Description

Responsible for providing direct in-home therapy to four to six families for a twelve-week period. This includes individual and family therapy.

Facilitate in-home therapy sessions and implement the treatment program without direct supervision.
Must work closely with Department of Human Resources, Family Court and other agencies in the selection and treatment provided to children and their families during the in-home intervention.

Ability to organize and manage one's own activities with a minimum of direct supervision.
Ability to work flexible hours due to the special needs of children and their families.
Participate on a rotating 24-hour on-call roster.

QUALIFICATIONS
Graduation from a recognized college or university with a Master's Degree in Social Work, Counseling, or Psychology.
ALC, LPC, LGSW, or LCSW preferred (see note below)
Some clinical experience preferred.
Degree and experience must meet requirements for Approved Medicaid Provider status.
Must hold a valid driver's license and maintain a driving record that is acceptable to Carastar's insurance carrier.
Must maintain at least liability coverage on personal vehicles.

**Responsibilities**
**Work Schedule:**
Monday - Friday, 9 hours between 6:00am and 6:00pm, including a one hour lunch
**Core Duties:**
- Coordinate appropriate referrals to community and other Government agencies for services.
- Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
- Independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diverse backgrounds.
- Use medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
- Implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals.
- Independently provide counseling and/or psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification.
- Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
- Participate in professional peer review case conferences, research studies, or other organizations required at the MTF.
**Additional Duties** :
5.1. Will perform walk-in duties on a rotational schedule.
5.2. Perform special evaluations on an as needed basis.
5.3. Provide training for junior providers/enlisted upon request.
5.4. See to improve the clinic by contributing to innovation.
5.5. Expected to seek evidenced based training/certificates for application of most up to date treatment modalities.
**Qualifications**
**Minimum Qualifications:**
*** Degree/Education:** Master's degree in social work (MSW). Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).
***** **Experience:** Social Workers shall have a minimum of two (2) years within the past five (5) years in the independent practice of clinical social work for adults in an outpatient clinical setting
*** Licensure:** Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker.
*** Life Support Certification:** Possess current ARC or AHA BLS Healthcare Provider certification.
