Duties & Responsibilities Complete assessment and intake services in a timely fashion for all assigned clients. Develop individualized service plans in conjunction with the family and other agencies/professionals. Document all services in accordance with Community Works policy and procedures and submit in a timely fashion. Ensure Medicaid and insurance guidelines compliance, as applicable. Plan and administer therapeutic treatment identified in the individualized service plan. Change method and degree of therapy when indicated by client reaction/needs. Collaborate with other professionals, family, and agency clinical staff to ensure consistency and timely completion of goals identified in the ISP. Maintain regular contact with other agencies/professionals and family regarding client progress via phone calls, letters, emails and/or monthly reporting. Prepare court reports as needed. Attend agency required meetings; i.e. staff meetings, treatment team meetings, IEP meetings. Comply with certification/licensing standards and provide all related documentation to administrative office.
