PRIMARY RESPONSIBILITIES
* Provide brief behavioral health assessment and intervention services for patients across the lifespan (children, adolescents, and adults) who present with a broad range of psychopathology, substance misuse, and health compromising behaviors contributing to chronic medical conditions.
* Provide services in a manner consistent with the Primary Care Behavioral Health Consultant Model of integration, including same day access, team-based care, and a flexible, population-based approach to care and provide screening, brief intervention and referral to treatment (SBIRT).
* Maintain a balance in daily schedule of same-day access to BH services, and a caseload of scheduled follow-up patients. Support PCPs and consulting psychiatric providers with regard to psychiatric medications, including basic patient education and helping to monitor adherence, side effects and effectiveness
* Facilitate time-limited behavioral therapy groups, including psycho-education and skill-building. Maintain patient electronic medical charts in accordance with the organization's policies and procedures, including appropriate use of Disclosure/Consent forms, documentation completed within 72 hours of visit, and accurate use of CPT coding
* Coordinate care with staff at community mentalhealth centers to facilitate exchange of information on services and shared patients. Work with clinic reception and other support staff to maximize patient access to behavioral health services
* Perform other duties as assigned.

Required for this job:
* Actively promotes team-based care within multidisciplinary medical teams including comfort in communicating with medical colleagues about clinical care
* Demonstrates skill in assessing and intervening on patients with complex presentations, including those with comorbid medical and psychiatric symptoms
* Ability to utilize brief behavioral health assessments to form diagnostic impressions and develop initial care plans. Ability to provide evidence-based treatments for behavioral health concerns in a brief format (e.g., 30-minute sessions and shorter courses of therapy)
* Demonstrates good knowledge of substance use disorders and ability to provide brief screening and evidence-based behavioral interventions for them

* Ability to utilize interpretation services to provide care to non-English speaking patients. Ability to maintain appropriate professional boundaries with patients, families, co-workers, and community resources
EDUCATION/EXPERIENCE REQUIREMENTS
Required for this job:
* Required: Graduate degree (doctoral or masters) in a clinical mentalhealth field
* Preferred: Experience providing behavioral health care in a team- based medical setting

* A graduate degree in a clinical mentalhealth field and a full, active, and unrestricted mentalhealth license in Washington State and are able to independently bill for services.

The Correctional MentalHealth Counselor 3 (CMHC3) is an integral member of the Skill Building Unit in Mission Housing and provides essential services for the incarcerated individuals with cognitive disabilities in the unit. Under the direction of the SBU Program Manager, the CMHC3 conducts trainings, groups, and provides individual case management care, related to functionality, ability to attend activities of daily living, interpersonal skills, soft skills and communication. The CMHC3 participates in unit community meetings, staff meetings, Facility Risk Management Team (FRMT) meetings, and multidisciplinary team meetings associated with release planning as well as disciplinary sanctions. The CMHC3 works with custody staff and classification staff to ensure safety and security, as well as progression of the incarcerated individual. The CMHC3 will identify incarcerated individuals who indicate risk for harm to self, risk for harm to others and grave disability, as well as make the appropriate referrals to mentalhealth per the Health Services policy as well as the Washington Department of Corrections (DOC) Healthcare Plan.
The holder of this position works to ensure the safety and security of the incarcerated individuals coming into Stafford Creek Corrections Center, Skill Builder Unit and those individuals that are transitioning into the community from incarceration, by providing educational opportunities to assist in preparing themselves for successful reentry into the community. This includes knowledge of resources and skills that will assist them to becoming successful in the community as well as identifying key needs to maintain success.
The CMHC3 provides professional services in an ethical and humane manner to participate in the Skill Building Unit. The services are gender specific as well as culturally competent. The CMHC3 uses evidence-based modalities based on the development of the individualized service plan. The CMHC3 functions ethically and maintains confidentiality.
The functions of the CMHC3 for the SBU program also maintains professional and ethical communications with both staff and incarcerated individuals to ensure continuous communication in maintained for the health and welfare of the incarcerated individuals as well as professionalism with other members of the SBU program staff.

The Edward Thomas House Medical Respite is a unique medical respite harm reduction program that provides shelter care with embedded nursing, mentalhealth, and case management services to an at-risk homeless population. Medical Respite is a part of the Healthcare for the Homeless Network and is supported by funds from multiple sources including King County Public Health, MIDD, and local hospitals. The Edward Thomas House admits patients referred from all King County area hospitals 7 days a week and operates 24 hours a day, 365 days a year. Essential Job Functions for the Licensed MentalHealth Counselor Associate include but are not limited to:
• Provides oversight and quality assurance of all mentalhealth services including record keeping and direct services, while under the supervision of licensed professional.

Qualifications
Required:

About PACT:
The Program for Assertive Community Treatment (PACT) is a nationally recognized, evidence-based approach to mentalhealth treatment which utilizes a highly collaborative model to positively impact the lives of individuals challenged by the most severe and persistent mental illnesses.
The DESC PACT team is centered in the concept that recovery is more successful when all providers work closely together to provide integrated support services for all aspects of participants lives, including medication, therapy, social support, employment and housing.
Team scheduling requirements reflect this commitment to provide participants with intensive wrap-around, 24/7 services at their homes and in a variety of community settings. Team members provide rotating coverage for 12 hours per day Monday to Friday and 8 hours per day on weekends & holidays. In addition, all team members participate in on-call rotations to provide 24-hour crisis coverage.
MAJOR JOB RESPONSIBILITIES:
Manage and facilitate PACTs SUD Programming:
The PACT model requires the SUDP to divide their time into approximately 80% facilitation of the SUD program and 20% direct case management work.
The PACT SUDP will update substance use treatment plans monthly according to the clients identified stage of change and propose effective, evidence-based interventions to meet each stage.
Act as the PACT team's subject matter expert for substance use disorders.
Provide consultation to other team members regarding SUD issues, treatment, and education, including Harm Reduction best practices.
Provide staff training on substance use issues, as appropriate.
Provide individual and group substance use disorder (SUD) treatment in office and in community settings using a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance use, and has client-determined goals. With the clients participation, develop support and treatment plans; facilitate linkages to collaborative resources when appropriate.
Plan, organize and facilitate treatment and support groups for clients, as appropriate.
Monitor and coordinate replacement of PACT harm reduction supplies.
Advocate for client access to community resources and services, ensuring that client needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care.
Develop and maintain strong collaborative relationships with DESC staff, and other service and resource organizations to ensure full continuity of care for clients.
Participate in psychiatric consultation, supervision, program team meetings and in-service trainings; participate in clinical reviews and case conferences.
Comply with the agency's clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance reviews when assigned.
Comply with data gathering/submission requirements, and with applicable program research and evaluation procedures.
Participate in verbal de-escalation in emergent situations and be willing and able to assist other staff as needed to maintain a safe and secure environment.
Participate in the planning, organizing and facilitating of unit mitigations for clients on your caseload. Unit mitigations are coordinated efforts to support clients with maintaining healthy living conditions. This can include but is not limited to attending care conferences related to unit mitigation, outreaching and supporting clients in their residential units with tools and skills to maintain their units, coordinating with housing staff to ensure proper work orders are filed in a timely manner, participating in cleaning out clients units, and documenting barriers to unit mitigation.
Collaborate with the greater DESC SUD team by attending the weekly SUD team meeting and consulting with the SUD Clinical Supervisor and SUD Senior Program Manager at least monthly.
This is a multidisciplinary role, serving clients with dual diagnoses.
Other duties as assigned.

Complete and update monthly substance use assessments, according to each clients stage of change for all PACT enrolled clients.
Prepare assessments for inpatient treatment programs.
Coordinate inpatient treatment referrals through King County.
Be comfortable with Harm Reduction and Motivational Interviewing approaches.
Living Conditions:
Support your clients with achieving and maintaining healthy living conditions. This can include but is not limited to attending care conferences related to living conditions, outreaching and supporting clients in their residential units with tools and skills to maintain their units, coordinating with housing staff, participating in cleaning out clients units, and documenting barriers to maintaining healthy living conditions.
Other:
Participate in daily morning meetings, psychiatric consultations, clinical supervision, program meetings and in-service trainings; participate in clinical reviews and case conferences.
Comply with the agency's clinical accountability policies and procedures; maintain current, timely and complete clinical records; participate in quality assurance reviews when assigned.
Comply with applicable program research and evaluation procedures.

Requirements:
MINIMUM QUALIFICATIONS:
Licensed by the State of Washington as a Substance Use Disorder Professional (SUDP) in good standing.

As an associate provider, you can expect starting pay at about $45-50,000+ per year for 20 clinical sessions per week. This is our foundation, and your pay is directly tied to the number of sessions you complete. As you build a full-time caseload, your earnings can grow significantly, with top-performing licensed providers making $90,000+ annually.
