JOB SUMMARY: Under the general supervision of Mental Health Supervisor, this position is responsible for psychiatric case management including direct patient care and education around mental health and social service needs; monitoring and mental health referrals; managing prescription refill requests, prescribed by mental health providers.
The case load will be drawn from Lombardi, Vista, and Caritas Health Psychiatry services.
SPECIFIC TASKS/DUTIES:
Work as a key member of the care team for complex patients who are part of the psychiatry services at Santa Rosa Community Health in the role of a nurse case manager.
Work closely with psychiatry team members, including other Mental Health Nurse Case Managers and Social Work Care Manager, to coordinate care for patients transferring to and from Sonoma County Behavioral Health, hospitals, and other psychiatric facilities.
Work closely with the SRCHs Mental Health and psychiatry teams in case consultation and discussion for a defined set of patients.
Serve as back-up during absences for other Mental Health Nurse Case Managers.
Support Joint Commission Readiness to support quality and safety for patients.
Under direction and with standing orders, use independent judgment within the licensed capacity to meet as needed with individual clients, on site, in other locations, or by telephone.
Assist mental health staff in coordinating mental health referrals to outside community resources for patients in the mental health department.
Monitor and track referrals to inside programs, including other mental health services.
Educate patients about community based mental health recovery programs.
Perform telephone callbacks, prescription refills, ordering labs, etc. as assigned by SRCH mental health providers.
Follow all established clinic protocols for patient care and clinical duties and assist with quality assurance functions.
Participate in triage of patient needs.
Contact patients with abnormal labs or tests.
Provide ongoing medical management of SRCH Clozaril patients.
Assist with coordination of patient transportation for referral services when needed.
Provide one-on-one and/or telephone case management for mental health patients in need of community resources.
Act as a resource/contact person to clinic staff.
Perform input and analysis of data related to care of patients receiving mental health services.
Assist with chart audits and other means of documentation of on-site grant programs.
Participate in ongoing training.

Qualifications
REQUIRED QUALIFICATIONS:
Registered Nurse (RN) with current, active license.
Minimum of one-year experience with direct patient care.
Experience working with chronically mentally ill patients preferred.
SKILLS AND KNOWLEDGE:
Bilingual Spanish and English strongly preferred.
Familiarity with general community resources for patients in Sonoma County.
Knowledge of principles of nurse case management for the patient with chronic disease.

SoMa RISE is an innovative program that will provide low barrier services to people who use drugs in and around the SoMa/Tenderloin area with a particular focus on individuals who are marginally housed/experiencing homelessness. We are recruiting compassionate staff who are familiar with harm reduction and who are able to provide non-judgmental supportive service to individuals who are using drugs. The program is one of the first of its kind in the USA, and is part of the Mental Health SF initiative.
Program participants are 18 and over adults who are under the influence of substances and need short term assistance and support. HR360 will be operating our SOMA RISE site in a manner that treats all participants with compassion, dignity and respect; are staffed with EMTs, a wellness team including health workers, and safety monitors. The team works to engage participants in health resources, however great or small. The program will operate a 24-hour, welcoming, compassionate, trauma-informed, culturally attuned, and user-responsive space that focuses on the principles of harm reduction. Alongside providing a safe, comfortable space for clients, our emphasis is on supporting people at increased risk of substance use-related high-risk behaviors and trauma, homelessness, and death due to overdose. Priority will be given to clients' safety, and all staff will be trained in OD Prevention, Recognition, Response and Narcan Administration. Health Workers in Harm Reduction Services provide direct care, engagement, support, and documentation for participants in the HR360 SOMA RISE program. As a part of a collaborative team, along with EMTs, Safety Monitors, and supervisors, Health Workers will ensure participant safety and comfort while supporting them in adhering to the participant guidelines of the program. Health Workers will model and demonstrate healthy COVID transmission prevention behaviors, wearing appropriate PPE and practicing physical distancing, while they observe and monitor participants, and support them in meeting their needs in accordance with the program. Health Workers will be required to complete minimal documentation and assist in connecting our participants to ongoing services and supports, and others as they arise. May be required to work nights and weekends.

The CAAP program is a collaborative effort with San Francisco Human Services Agency.
Job Title: Behavioral Health Counselor
Status: Full-Time (Monday through Friday)

Essential Job Duties include, but are not limited to:
Conduct brief biopsychosocial assessments to determine a client's suitability for work assignments, mental health counseling or other referrals.
Participate in and facilitate client outreach at Navigation Centers in San Francisco to further support client engagement in various city and county programs
Provide culturally sensitive, short-term (up to 4 months), mental health and substance abuse counseling to individuals and groups, along with case management
Consult and collaborate with clients, case managers, and internal/external providers to coordinate client care.
Lead workshops teaching soft skills, including SMART goals, conflict resolution, work/life balance, self care, job retention, etc.
Maintain timely administrative and clinical records related to client care, per standards specified by RAMS policies, clinical standards, and oversight agencies (e.g. HSA, DPH, etc.) in Epic and Launchpad.
Provide crisis intervention as needed.
Provide consultation to HSA staff concerning client care.
Participate in case conferences, clinical team and staff meetings.
Provide referrals to resources, both within the HSA program and other agencies within the city of San Francisco.
Participate in individual & group supervision (hours may be obtained toward LCSW/LMFT/LPCC licensure).
Other duties as assigned.
Minimum Required Qualifications:
MS/MA in Psychology, Counseling Psychology, Social Work, or related field, from an accredited institution.
Current BBS licensure or registration
One year of direct services experience in a community-based agency providing case management, vocational assessment/training, linkage to service providers, and mental health & substance abuse counseling to indigent & underserved adult clients with mental/behavioral health issues.
Proficiency in conducting clinical & functional assessments of clients with both behavioral health and physical complaints.
Prior experience in assessment and treatment modalities including short-term counseling, group therapy, crisis intervention.
Prior experience treating clients with dual diagnoses (e.g. behavioral health/substance abuse).
Knowledge of treatment & recovery models including harm reduction and motivational interviewing.
Other Preferred Qualifications:
Prior experience with vocational rehabilitation models of practice and counseling strategies.
Knowledge of San Francisco systems of care and community resources for clinical services.
Experience with the Epic electronic health record system.

HOURS + LOCATION
This is an in-person, part-time position requiring a minimum of 20 hours per week, with the potential to increase hours (up to full-time) as your caseload grows.
Proposed Schedule: Monday, Tuesday, and Thursday from 12:00 PM - 8:00 PM. Associates candidates should also be available to to attend group supervision (virtually) on Fridays from 10:00 AM - 12:00 PM.
Location: This role is based on-site at our Downtown San Francisco office, conveniently located in the beautiful and historic Flood Building located the corner of Powell and Market Street.
Requirements
CANDIDATE REQUIREMENTS
Licensed Therapists:
Hold a valid and current license in the State of California (PsyD, PhD, LMFT, LCSW, LPCC).
Associate Therapists:
Hold a valid and current BBS registration (AMFT, APCC or ACSW).
Have completed a minimum of 500 post-graduate supervised hours.
Clinical Experience & Skills:
Ideal candidates will bring a mix of clinical expertise, cultural responsiveness, and enthusiasm for collaborative care.
Demonstrated experience working with children, teens, and families addressing a wide range of mental health concerns.
Strong ability to build rapport and trust creating a safe and supportive therapeutic space.
Skilled in clinical assessment, treatment planning, and evidence-based interventions tailored to the developmental needs of each client.
Comfort in facilitating client consultations with parents/caregivers.
Competence in collaborating with caregivers, school staff, or other external agencies to ensure coordinated care.
Ability to maintain clear boundaries and compassionate communication, fostering engagement and accountability throughout treatment.
Commitment to cultural humility and providing affirming care for diverse populations, including LGBTQIA+ and BIPOC youth and non-traditional family systems.
Excellent organization and documentation skills, ensuring timely and clinically sound record-keeping.

Preferred:
Advanced training or certification in play therapy, sand tray, PCIT, and/or family systems approaches.

The Role
We are currently hiring a Licensed Child Therapist to provide compassionate, evidence-based care to children and families. This is primarily an in-person role in Berkeley or San Rafael, though hybrid/remote flexibility may be possible.

26 scheduled sessions per week (average 22 sessions per week with cancelations).
