We are currently seeking a full-time Youth Community Based Case Manager. The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing serious mental illness and/or addiction challenges to ensure their needs are addressed. The Case Manager supports clients in the development of their short and long-term goals and acquisition of community resources using a person-centered approach. Services will be provided with respect to cultural preferences and in accordance with client's rights. The person in this position participates in development of comprehensive care coordination and treatment planning, advocates for persons on their caseload, utilizes evidence-based assessments, provides linkage and referral to additional community resources as needed, coordinates crisis intervention services and generally supports the client in their pursuit of emotional and physical health and wellness. Essential Duties and Responsibilities:
Assesses client strengths and needs with the client and applicable team members, conducts comprehensive care coordination, and develops treatment plans. Monitors and evaluates client progress toward goal attainment. Reviews and modifies treatment plans as needed.
Maintains contact with the client as indicated in the treatment plan or as needed to ensure stability. Communicates with and provides support to clients in achieving goals and outcomes. Assists clients in tracking and keeping appointments.
Assists clients with developing skills that lead to independence
Provides back up for other staff.
Completes required documentation in the electronic health record in accordance with timelines and standards required by the agency i.e. within 48 hours of interaction, regulatory bodies, and payors. Demonstrates excellent written and oral communication skills.
Carries out policies and procedures of the agency; may participate in community work, staff development, agency-wide projects; serve on agency committees/teams, attend meetings, and performs other related duties as requested.
Training requirements include CPR, CPI, new hire, Sanctuary and annual boosters in addition to any required trainings assigned during employment
Must be able to safely drive-in all-weather conditions in support of client needs
Perform other duties as assigned.

Employees may interact with a diverse group of individuals, some of whom may display anger and frustration due to stressful situations. These individuals may also be under the influence of drugs or alcohol and require treatment.

Requirements

Position Summary:The Licensed mentalhealth provider/Non-Clinical Case Manager shall provide year-round dedicated mentalhealth and wellness services to Washtenaw Community College students. The primary goal is to facilitate student success and retention by providing short-term counseling and mentalhealth support , non-clinical case management, referral to internal/external counseling resources, crisis intervention, engaging students in self-care, promoting student success and wellbeing, and fostering behaviors that contribute to student degree completion. The Licensed mentalhealth provider/NCCM will accomplish this through best practices and clinical interventions based on the established WCC counseling scope of practice, community referrals, and follow-up services.Essential Job Duties and Responsibilities:
Respond to daily counseling and case management functions including student emergencies, student referrals, walk-in requests, and faculty/staff/administrative consults. This includes regular responses to crisis and urgent mentalhealth needs where wellness checks on students may be appropriate and follow-up necessary.
Ability to quickly make a strong therapeutic alliance with students of any background to support their emergency or day-to-day mentalhealth needs.
Provide specialized support and intervention for WCC students experiencing complex mentalhealth issues and crises. This includes comprehensive suicide risk assessment, safety planning (including lethal means management), and consistent application of WCC emergency mentalhealth protocols.
Provide timely and appropriate referrals to internal and external support services, and coordinate intervention plans in collaboration with community resources using a case management model
Assist with long-term needs of students including follow-up to ensure students have been successful in addressing issues/questions related to student crisis. This may include creating and managing post-intervention strategies for those students returning after a psychological emergency and/or hospitalization.
Monitor and track individual cases and student progress to ensure compliance with recommendations, including safety assessment and safety planning
Manage a caseload of students through an electronic documentation system and communicate with relevant campus partners regarding students of concern. This includes making sure all correspondence, notes and action items are accurately documented
Create and maintain mentalhealth/wellness presentations, educational programs, and materials for faculty, staff, and students.
Perform outreach to faculty, staff, and students as well as other community stakeholders to support students in need
Prepare reports, proposals, and audits as required by the Executive Director of Student Success Services
Conduct research on best practices and evaluate benchmarks associated with the proactive assessment of initiatives supporting the mentalhealth/wellness of current students.
Assist in the development of strategic goals that align with institutional goals relevant to student mentalhealth/wellness needs
Collaborate as part of the college CARE team, a multi-disciplinary team designed to provide assessment and support to students of concern.
Represent the Counseling & Success Services departments on additional relevant college committees, campus functions and community agencies
Establish and maintain effective collegial partnerships with related campus departments including Campus Safety, Dean of Students, Student Activities, Success Coaching, Student Resource Center, Advising, and LSS as well as campus departments and external agencies determined relevant to the mentalhealth success of WCC students.
Regular attendance on campus is required for this position.

Hours/Schedule:The Licensed mentalhealth provider/NCCM shall be accountable for 200 reporting days, within the 12-month academic year. This position will work a flexible schedule as needed to accommodate day, and evening students.
Minimum Qualifications:
Minimum Required Knowledge, Skills and Abilities:
Master's in Social Work, Counseling, or relevant field with full, unrestricted independent licensure in the State of Michigan (e.g., LMSW, LPC, LMFT, LP).
4 years of full-time experience providing mentalhealth care or equivalent.
3 years of experience working in a higher education setting.
Training in two of the following therapeutic modalities: Solution-Focused Therapy, Acceptance and Commitment Therapy, Dialectical Behavior Therapy, Interpersonal therapy, Cognitive Behavior Therapy, or short-term psychodynamics.
Exceptional ability to independently assess suicide risk, manage means, and facilitate connection to emergency services.
Experience and comfort supporting individuals in crisis with demonstration of sound judgment and in managing complex crisis situations.
Excellent interpersonal and cross-cultural communication skills, demonstrating the ability to quickly establish therapeutic rapport and work effectively with a diverse student population while maintaining professional neutrality.

Responsibilities

Compensation: $55,000 - $59,000
Outpatient MentalHealthTherapists with National Youth Advocate Program work as part of an integral service team. In this role, you will deliver Mental and Behavioral Health Services to adolescents, youth and children, as well as their families, who have been identified by public and private referrals and outside agencies. Referral sources may include County and non-County Agencies, self-referrals, schools, and healthcare providers.
Under the general supervision of the Clinical Supervisor or designee, you will work closely with youth, family, foster parents, and community partners, providing counseling or therapy, advocacy and support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
Provides individual, group, and family mentalhealth therapy, case management, transportation, and other professional services to clients served
Coordinate and monitor services for the youth and family in the community mentalhealth, medical, educational, psychological, vocational, and social services.
Regularly reviews and modifies youth-based plans to assure consistent progress and success on written plan goals.
Establishes and maintains strong relationship with adolescents, youth and children as well as assigned foster parents and families of origin.
Assists in family reunification and/or stabilization efforts with the youth's primary family or other permanent living arrangement consistent with the youth's case plan.
Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served
Minimum Qualifications
Master's Degree in Social Work or comparable Human Services field from an accredited institution.
LSW or LPC required; LISW LPCC preferred
Experience in providing both individual and family therapy preferred
Experience working in direct service with youth and families preferred.
Experience with TF-CBT preferred
You have good computer and technology skills
Must be willing to accept other job duties as assigned

Position Summary:
Under the supervision of the assigned Program Coordinator, provides the full range of behavioral health treatment, including diagnostic assessment; individual, family and group therapy; and crisis intervention. The scope and volume of services provided will vary as related to specific job role.

Ensures PCS standards are met and actively utilizes and responds to strategies provided through supervision when there are challenges in meeting these standards.

Therapists rendering mentalhealth services are required to have a Bachelor's or Master's degree and hold an active, independent license through the Ohio Counselor, Social Worker and Marriage and Family Therapist Board (CSWMFT Board) - LPCC or LISW or LPC or LSW with a minimum of two years, full -time equivalent experience in the diagnosis and treatment of behavioral health conditions, under supervision.
Counselors rendering chemical dependency services are required to have a Master's Degree and an active, independent license through the Ohio Credentialing Board - LICDC to qualify for this wage scale.
Therapists are required to maintain professional licenses through compliance with rules set forth by the licensing Boards including, but not limited to, completion of CEs necessary to maintain licensure and completion of license renewal applications.
Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
Successful completion of a 180-day probationary period.
