SCHEDULE: Night shift, 3x12 hour shifts
PURPOSE STATEMENT:
Responsible for providing personal care services to patients at the facility under the direction of clinical or nursing leadership.

PURPOSE STATEMENT:
Responsible for providing personal care services to patients at the facility under the direction of clinical or nursing leadership.
ESSENTIAL FUNCTIONS:
· Ensure the well being of patients and provide a positive, supportive and structured environment, under direction of charge nurse or supervisor.
· Patient safety is addressed through required rounds completed timely. Patient room is clean and uncluttered. Observation may be assigned every 15 minutes, within reach of patient and/or 1:1 patient care.
· Obtain patient's vital signs, height and weight as assigned. Ensure documentation of information.
· Identify self to patient and visitors promptly and respond to patient requests quickly.
· Engage with patient in activities and interactions designed to encourage achievement of treatment goals.
· Respond to distressed patient in a non-threatening manner and attempt to understand the source of distress.
· Applies de-escalation techniques to help manage patient's emotions, behavior and participation in treatment and assist with seclusion and restraint when necessary.
· As part of the multidisciplinary team provide safety and increase coping skills of patient and assist in developing strategies for helping patients to be as independent as possible.
· Collaborate with other staff in working with the population of the facility to treat, rehabilitate and return patient to the community.
· Monitor behavioral improvements and reinforce these behaviors. Report changes and responses to treatment by communicating with team. Significant changes are reported to nurse immediately.
· Reinforce skills patients learn from nurses, physicians, therapist that promote improved physical and mental wellness.
· Lead or co-lead individual or group sessions as part of specific recreational or therapeutic procedures.
· May participate in rehabilitation and treatment programs as well as teach strategies to promote patient wellness and independence.
· May oversee or assist patients with activities of daily living, including toileting, bathing, dressing, grooming, oral hygiene, meals, snacks, hydration and changing bed linens.
· Deliver care based on the priority of needs and available resources to the specific service population of the facility.
· May help with admissions and discharges.
· Assist in providing guidance and direction to other staff members, including training of new staff.
· May provide transportation for patient or coordinate transportation with appropriate staff member.
· Quality of work exhibited through accuracy, neatness, and timeliness of work completed.
· Adherence to work schedule and overall attendance.
· Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
OTHER FUNCTIONS:
· Perform other functions and tasks as assigned.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
· High school diploma or equivalent required.
· One or more years' experience working with the specific population of the facility preferred.
· 6 months experience performing the duties of a BHA2 preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
· CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
· First aid may be required based on state or facility requirements.

As a member of the Family and Community Partnerships (FCP) department, reporting to MentalHealth Coordinator, responsible for implementation of the program mentalhealth services, inclusive of Easter Seals Blake Foundation (EBF) and EHS-CCP, to children (prenatal to age 5) and their families in the five designated counties in Southern Arizona, for assigned caseload. Provides individual mentalhealth consultation to high need environments, through a trauma-informed Infant and Early Childhood MentalHealth Approach creating individualized behavior plans to meet the child's social-emotional well-being. Implements and assists in maintaining administrative systems and processes that support children with mentalhealth needs and help the agency achieve compliance with Office of Head Start and Arizona state mentalhealth related requirements. Maintains relationships with mentalhealth community providers to enhance the quality of services. Creates and delivers professional development within the agency and the community related to mentalhealth topics.

Implements and supports the development of the agency's MentalHealth system (prenatal to age 5) to prevent, identify, intervene and reduce the impact of mentalhealth challenges for children, families and staff.
Promotes social-emotional well-being through a trauma-informed infant and early childhood mentalhealth approach by:
Providing mentalhealth expertise to classrooms, homes and situations. Assessing, evaluating and creating therapeutic interventions to best support child and family.
Supporting and collaborating with Education Specialists and Coordinators, along with Comprehensive Services Unit (CSU) staff, for the general well-being of children, families, classrooms, and homes.
Facilitates the creation and implementation of individualization plans with teachers, families, and trans-disciplinary team individualized plans for children. Provides ongoing support to ensure successful implementation of the individualized mentalhealth plan.
Facilitates direct consultation, support debriefing groups, develops intervention plans, and provides resources to staff or families.
Facilitates teacher and parent meetings that pertain to mentalhealth issues. Strengthens family and staff capacity to support social, emotional, and relational health of children and families in a range of settings.
Coaches and models developmentally appropriate, trauma-informed strategies and methodologies to establish and maintain nurturing environments and relationships between teaching staff, children, and families.
Coordinates intervention plans with Inclusion staff.
Reconciles services provided by contracted MentalHealth Consultants and Providers.
Supports Department of Child Safety (DCS) reporting process.

Job Summary
The Behavioral Health Clinician (BHC) is a member of the interdisciplinary treatment team in CBI Crisis, Inpatient and Transition Point programs. Skills/Requirements:
Bachelor's Degree in a field related to Behavioral Health. Master's Degree is preferred but not required. Preference given to Master's Degree in field related to behavioral health and/or has been licensed at the associate level with The Arizona Board of Behavioral Health Examiners as Psychologist, Social worker, Counselor, Marriage and family therapist or Substance Abuse Counselor.
1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.

Participate in staffing with the cross-functional team and stakeholders.
Provide orientation to children in care.
When necessary, travel to provide community-based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.

MINIMUM QUALIFICATIONS

JOB PURPOSE:
The Clinician will provide treatment to eligible clients and/or their families who have chronic mentalhealth needs and/or serious mental illness with Endeavors Behavioral Health Department. The Clinician will have training and experience in trauma-focused treatment and co-occurring disorders. They will be proficient in providing individual, couples, and family therapy, with an emphasis on delivering evidence-based care. Provide support to clients to include referrals, scheduling appointments, continuity of care and access to appropriate services.
Qualifications
ESSENTIAL JOB RESPONSIBILITIES:
Under supervision of an assigned Licensed Clinician as required by Arizona Board of Behavioral Health Examiners, provide direct care including a variety of mentalhealth services for individuals and groups
Complete appropriate risk assessment, risk mitigation and monitoring.
Complete comprehensive mentalhealth/ psychosocial assessments, case conceptualizations, diagnostic evaluations, treatment planning, measurement-based assessments, maintain appropriate clinical documentation in Electronic Health Record (EHR) or equivalent and facilitate effective discharge planning.
Deliver trauma informed, evidence-based treatments in collaboration with the Endeavors Wellness Model.
Maintain at least 65% Direct Client Service Time
Collaborates and coordinates with the client, caregiver, physicians, director, case management and all other disciplines of the Behavioral Health Department or in the community to provide the best care possible.
Support and/or attend outreach events to highlight Endeavors Behavioral Health services to raise awareness with community partners and other potential stakeholders and encourage referrals.
Attend weekly Multidisciplinary Team Meetings (MDT), clinical consultation and administrative meetings.
Complete annual required trainings.
Maintain clinical competencies.
Other duties as assigned.

ESSENTIAL QUALIFICATIONS:
EDUCATION: Equivalent to a Master's degree in psychology, social work, marriage and family counseling, or a related field from an accredited college or university.
EXPERIENCE: 2 years of professional experience working in mentalhealth counseling, psychotherapy, and/or related social services in a public, certified private welfare, or health agency preferred. Experience working with adults, couples, and families is required; Knowledge in DSM-5-TR (or latest DSM edition available) diagnoses to include PTSD diagnosis and treatment is required; ability to obtain certification or documented proficiency in Cognitive Behavioral Therapy and Motivational Interviewing within 6 months of employment. Ability to obtain Basic Life Support (BLS) training and certificate of completion within 1 month of employment. Experience working with diverse populations, military populations and/or Veterans is a plus.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.
LICENSES: Driver's License with clear record. Licensed in the State of AZ: Graduate Level license to practice psychology, social work or counseling, LPC, LCSW or LMFT required.
VEHICLE: Must have daily use of a vehicle without prior notice.
OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others. Employee should expect to work one evening a week to accommodate the schedules of the clients being served.
WORK LOCATION: Hybrid work set up can occur; private, secure home office needs to be in place for any remote work. At minimum, initial work from office (in-person) and then a hybrid in-office or in-home work can be reviewed, scheduled, and agreed upon to meet the mission.
Bilingual - Fluent Spanish Language Skills is preferred.

Responsibilities
What You'll Do:
The Behavioral Health Specialist (BHS) is responsible for managing and maintaining the safety and well-being of individuals and staff within the unit, ensuring high standards of care, and support for both adults and youth. This role involves conducting vital signs assessments, assisting individuals with daily needs, and providing exceptional customer service. The specialist will also accurately document services provided and significant interactions in the Electronic Health Record (EHR).
* Conducts security screening and inventories property of individuals upon admission to the facility. Provides orientation to the Observation Unit and Short-term Inpatient Unit. Educates patients on their rights under ADHS regulations, grievance process, provides expectations, and explains rules and regulations.
* Intervenes with escalated individuals using therapeutic techniques for de-escalation and performs restraints/seclusions when needed to ensure the individual's safety and the safety of others. Completes 1-to-1 on a patient in seclusion or mechanical restraints Q15.
* Lifts, moves, and transports individuals using proper body mechanics or lifting devices for accident prevention.
* Assists individuals with activities of daily living to include eating, bathing, dressing, toileting, and transferring.
* Conducts and documents individual's vitals and reports all questionable results to the RN/Physician attending the individual. Conducts and documents all BAL, urine drug screens, and urine pregnancy tests.
* Attends court ordered hearings when subpoenaed. Must be willing to be a witness on a petition and available to provide testimony at court ordered hearings.
* Documents all services and significant individual interactions in the Electronic Health Record. Monitors and documents patients Q15.
* Responsible for maintaining and ensuring the cleanliness of the facility and designated assigned unit. Performs light cleaning of interior center, collects soiled linen, and launders individuals clothing when needed.
* Performs all other duties as assigned.
