Develop discharge and continuum of care plans to ensure smooth transitions. Document all counseling services, assessments, referrals, and follow-ups in the patient's clinical record. Maintain patient caseload, including case management and clinical charting. Coordinate support services for patients and families and communicate with referral sources. Attend team meetings, complete required training, and assist with administrative tasks. Requirements Bachelor's or Master's degree in Psychology, Counseling, or Social Work from an accredited college or university (as required by Virginia regulations). Minimum of three (3) years in substance use treatment, mental health counseling, or a related field.
