Key Responsibilities

Job Duties
Virginia Beach Health District is seeking a Health Counselor to be responsible for independently identifying and contacting persons known or suspected of being exposed to or at high risk of exposure to communicable diseases such as tuberculosis, syphilis, gonorrhea, HIV/AIDS, and others. This role is responsible for performing field investigations and surveillance and requires possession of a valid state driver's license.
Duties will include alerting people to their health risks, behavior changes needed to reduce risks to the individual or to others, resources and methods available to treat and control the disease as well as to contain further transmission of disease. Conduct Follow up on contacts to determine if they have received treatment, changed their behavior to reduce health risk, and procured other available services as necessary to ensure improved health care.

Gathers information necessary for case management, analysis and problem solving

Educates patients and contacts about communicable disease(s) symptoms and risk reduction behaviors
Refers patients to the appropriate medical and social support services

Analyzes medical and epidemiological information to identify disease intervention or prevention opportunities
Communicates with clients, contacts and health care professionals in a confidential, timely and professional manner

Contacts medical providers, laboratories and other service providers to obtain information about patients or to refer patients or partners for examination and/or treatment

Provides information to clients about medical facilities, community based and social services programs and organizations (shelters, drug rehabilitation programs, mentalhealth, and food pantries) to assist patients with various psychosocial needs
Assesses patient's need for care and initiate the referral process for newly diagnosed patients and for those who are lost to care
Field Investigations
Conducts field visits to confidentially locate and notify persons infected with, exposed to, or at high risk for reportable conditions
Ensures appropriate follow-up in accordance with CDC treatment guidelines
Prioritizes and documents all investigative activities according to VDH program protocols

Provides and assists in educational presentations to medical and health care personnel, schools, community groups and the general public

Fosters professional relationship with pharmacy testing partners to enhance referral process for confirmatory testing
Maintains relationships with community partners to enhance service delivery; encourages community entities to participate in distribution activities; and participates in community outreach events as directed by local program policy.
Completes required VDH sponsored and any additional trainings and maintains TRAIN account according to agency policy
Attends continuing education programs in accordance with development plan
Surveillance & Outbreak Control Efforts

Under general supervision, this position provides crisis intervention and short term counseling services to children, adolescents and adults in acute care psychiatric facilities and regional crisis stabilization residential programs. Reports to the Program Administrator.
ESSENTIAL JOB FUNCTIONS
Conducts intake assessments and diagnostic evaluations; provides coordination and consultative services in cooperation with other community agencies; consults with psychiatrists and/or psychiatric nurses regarding medication and diagnoses; documents case records; and works independently as a clinical practitioner without ongoing clinical supervision.
Conducts crisis intervention, responds to emergency situations; assists other units as needed and implements and maintains rules to provide for consumer safety; assesses disposition of consumers to determine level of intervention required and conducts group, individual and family therapy and works closely with school and hospital personnel on appropriate referrals.
Serves as a liaison to staff members to discuss consumers' progress; adjusts treatment plans accordingly and maintains accurate consumer records; serves as a liaison with other agencies to maintain consumer care and case management; ensures that consumers receive appropriate services and treatment; and works closely with families to develop interventions, referrals, and linkage into the community.

Requires a professional certification in the State of Virginia as a Qualified MentalHealth Professional (QMHP) or Licensed MentalHealth Professional (LMHP). Qualified MentalHealth Professional Adult (QMHP-A) preferred. Applicants may apply that are not currently licensed or certified but hold the appropriate educational attainment and minimum experience while being registered or in supervision with the appropriate professional board to become certified or licensed.
