SUMMARY: The Recovery Mentor Specialist will conduct interventions with clients at the Hooper Center and other treatment institutions. The Mentor will escort and provide transportation for clients to initial post Detox treatment episodes, primary care providers, twelve-step meetings, and other outside support meetings. The Recovery Mentor will work with Hooper Staff and other Treatment Providers for potential referrals. The Mentor will work to prepare clients to continue on the path to self-sufficiency such as employment and stable housing.
MINIMUM QUALIFICATIONS:
Requires high school diploma, GED, or demonstrated ability to read and write at a sufficient level.
Must be registered, prior to start, as a Certified Recovery Mentor (CRM) with MHACBO; OR be a certified Peer Support Specialist (or Peer Wellness Specialist) AND be registered as a Traditional Health Worker (THW) on the THW Registry.
Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
Flexible work schedule required, including some evening and weekend work.

Work within scope of training and experience and seek consultation when appropriate.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments, as well as to the program administrator.
Monitor office and answer telephone as necessary.
Participate in compliance, quality assurance and quality improvement activities as directed.
Attend all mandatory CCC trainings in a timely manner.
Perform other duties as assigned.

POSITION DESCRIPTION

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE

Communicate positively and professionally with ODHS staff and work collaboratively with caseworkers to support parents in achieving their goals
Consult with ODHS Child Welfare staff regarding effective culturally specific relevant practices to engage families of the assigned client population.
Encourage and support parent participation in program Parent Support Group events and activities.
Facilitate OSLC-KEEP sessions, as appropriate, following required training and certifications.
Demonstrate a commitment to developing a thorough knowledge and application of the Sanctuary model and other organizational policies & practices.
Participate in staff development, in-services, and training related to workplace effectiveness; model professional behaviors and implement improvements to business practices with awareness of their impact in a trauma-informed behavioral healthcare organization.
Experience/Education Requirements:

Minimum Qualifications

Qualifications

The Recovery Mentor provides case management offering a combination of referral linkage, individual counseling, family support, group counseling and case management services to assist client's successful engagement in treatment and the family's ability to support the client's continued treatment involvement. May work in a residential or outpatient setting.
Location: This position is based out of LifeWorks NW's Beaverton site.
Pay: $21.74 starting hourly pay.
Essential Responsibilities:
* Screens and selects clients to participate in the mentoring program.
* Develop the service plan for all clients including a service reporting plan on each referral.
* Within the first five working days following intake into services, the client and (if appropriate) family members meet with the Recovery Mentor and the Primary Counselor to develop a case management plan.
* The Recovery Mentor should have daily contact with clients for up to the first five weeks after release helping with engagement in recovery support, housing and employment issues, family adjustment issues, legal, housing, medical and benefit issues. Frequency of contact should gradually decrease to the point of twice monthly or monthly contact by the ninetieth day after release.
* Responsible for assisting the family in the initial implementation of the plan, maintaining regular communications with the client's primary therapist, collateral providers, community corrections staff, and linking the client system to needed services within the community.
* Determines when to terminate clients from the program because services are no longer necessary or the client is not responding to the mentor model.
* Meets regularly with staff from social service agencies to update them on LifeWorks NW services identifying unmet services/needs LifeWorks NW may be able to address; review and update referral and case coordination procedures as needed; obtain referral requests and work with referral sources to remove any barriers to treatment; report back if clients fails to attend; obtain relevant case information from the referral for the clinician who will be doing the client's intake assessment; work with referral source and client to facilitate the family's involvement in the assessment and the treatment planning process.
* All services must be provided in a culturally appropriate manner.
* Some evenings/weekends required
* Travels between sites or in the community driving clients when needed.

* High school diploma/GED.
* Must have a current Recovery Mentor Certification and a Traditional Health Worker number from the Office of Equity and Inclusion.
