SUMMARY DESCRIPTION OF WORK
This position is responsible for assisting InSHAPE members to develop and implement their personal wellness program. Provides support, education, and motivation related to physical fitness, nutrition, and wellness. Encourages by example. Assists in training of volunteer mentors and other mentors as needed. Provides individual and group activity instruction. Works independently in community settings with individual consumers or leading activities. Collects data on participant progress.
Compensation: between $41,300 to $52,240 per year (salary is based on total years of relevant experience)
Work Schedule: 40 hours per week

MINIMUM QUALIFICATIONS
Personal Trainer Certification
Nutrition-specific Certification and/or training
Current CPR and First Aid Certifications
Bachelors Degree in Health Science, Social Work, Rehabilitation, Psychology, Health Promotion or related field
Ability to perform exercises and movements as demonstrations for clients
Physically active, readily identifies and believes in the holistic benefits of good health.
A commitment to decreased health disparity, and social stigma for individuals with mental illness.
Willing to commit to a work schedule of approximately 10:00am-6:30pm daily.
Essential Duties and Responsibilities
*Provides community and residential-based support services to insure implementation of participant's health goals.
*Manages a caseload of approximately 10-20 clients to establish and implement individual InSHAPE plans.
*Works collaboratively with clients, program staff and the InSHAPE Lead to design and support individuals to promote stable sobriety and economic self-sufficiency for mothers and enhance a strong, protective maternal bond for their children.
*Provides fitness training, monitoring, continuing reassessment of exercise, nutrition and healthcare commitments.
*Work with participants to create healthy meal plans, execute weekly grocery order, and assists in cooking nightly dinner with participants.
*Teach cooking skills and equip participants with health focused recipes.
*Educate and help participants implement healthy, food-related parenting practices.
*Provides transportation to and from the YMCA for exercise group.
*Advise participants about important safety concerns and demonstrate exercises or maneuvers as needed.
*Provide clients with safe, reasonable exercises that they can perform in the gym as well as at home or in the community
*Facilitates peer relationships among participants.
*Actively participates in activities with participant when support is needed.
*Encourages member participation and movement along the continuum of wellness related lifestyle changes.
*Administers fitness assessments as needed and records data.
*Shares member participation and progress other than, or in addition to fitness assessments with InSHAPE Lead to assist in documentation for project outcomes.
*Assumes responsibility for tracking and documenting member participation in activities.
*Works with clients, volunteers and community partners to increase client independence.
*Provides instruction and facilitation for group activities.
*Works with clients to identify and establish wellness support system outside of InSHAPE and Family Works programs.
Display a courteous and caring attitude at all times to the participants, volunteers, and visitors of Heartland Family Service.
Cooperate and collaborate with program area staff, volunteers, and other Agency staff.
Abide by all specific program and Agency procedures, policies and requirements.
Develop personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
