Summary of Essential Job Functions:
Responsible for creating and leading person-centered and interactive activities both onsite and in community settings for participants who attend our ADT Program
Responsible for required ADT documentation and monitoring as designated by the Community Services Manager
Responsible for ensuring that ADT activities promote independence and growth per the participants chosen goals
Assist in participant personal care needs, such as assistance with eating and personal hygiene
Participate in required training sessions and morning meetings as designated by the Community Service Manager
Other duties as assigned
Minimum Requirements:
Must be at least 18 years of age
High School Diploma, or GED
Personal Vehicle, Valid Driver's License and Current Auto Insurance
Physically able to assist in lifting people up to 250 pounds, as well as able to assist in the mobility of wheelchairs containing people up to 250 pounds
Abilities Required:
Must show an ability to demonstrate patience and respect for those we serve as well as co-workers and management staff
Ability to provide accurate and timely documentation

Lead the integration of peer support practices into broader project outreach efforts

Support client success in working with their Empowerment Coach and working through training plans
