Greater Nashua Mental Health is looking for a Health Mentor to join our Integrated Care team! The Health Mentor provides evidence-based wellness interventions including goal-setting, fitness activities, basic nutrition, and smoking cessation to and education related to physical fitness, nutrition, and wellness to adults and teens with mental illness. Provides interventions in individual and group modalities in community-based settings.
Duties & Responsibilities:
Delivers interventions that promote and support healthy lifestyle habits including fitness, healthy nutrition, and smoking cessation with fidelity to the InSHAPE model.
Provides individual and group-based direct service to program participants for a percentage of time per week as appropriate for position and as directed by supervisor, in community-based settings.
Conducts and documents initial InSHAPE physical assessments and collects and documents vitals data within the first 30 days of participants' entry into the program and on a quarterly basis thereafter.
Completes documentation in Electronic Health Record within timeframes specified by contracts and agency policy.
Promotes engagement in the program through outreach and coordination with other members of a participant's treatment team.
Works collaboratively with team members by attending weekly treatment team meetings and other meetings as assigned.
Participates in individual supervision with the Director of Substance Use Disorder Services as well as coaching and mentoring from team leaders.
Promotes the InSHAPE program by attending and participating in community events as a representative of the program.
Performs other duties or special projects as required or as assigned.
Requirements
Qualifications:
Bachelor's Degree in Health Science, Social Work, Rehabilitation Psychology, Education or related field
Personal trainer certification within first year of employment
Experience and/or training in motivational interviewing approaches.
Fluency in Spanish strongly preferred
Organizational and time-management skills
Knowledge of Microsoft Office products (Email, Word, EXCEL)
Strong collaborating skills, and an ability to work effectively both independently and as part of a team
Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive
Excellent communication and interpersonal skills

Our program begins during the Academic Year where students will meet with advisers twice monthly in their schools and receive academic, career, and college assistance and guidance. In addition to in-school advising, students receive college & career guidance during grade-level workshops, have the opportunity to attend monthly Saturday Tutoring Academies at UNH and college, educational and cultural visits throughout New England. The Role of an Academic Year Tutor-Mentor
Academic Year Tutor-Mentors work with our students during the school year to provide academic and college tutoring on an as-needed basis, help to staff our monthly Saturday Academies providing academic support and designing college & career workshops, and chaperoning college visits and career activities when available. Job Description:
The InSHAPE team is seeking a Mental Health Mentor who has some flexibility around their work hours to be part of a dynamic health and wellness team that supports consumers of mental health services in addressing their nutrition, fitness, and wellness goals. This is a community-based position working with both youth and adults. Individual sessions will take place in a variety of locations, including, but not limited to, participants' home, a local gym, outdoors, within the Greater Manchester area. Requires a valid driver's license and auto insurance with liability coverage. Certified Personal Trainer certification is preferred at start of position but must be obtained within the first 3 months. Bachelor's degree in social sciences, health, or nutrition required.
Requirements:
Key Responsibilities:
Work with participants to identify individualized health and wellness goals.
Provide education and support around nutrition, exercise, smoking cessation, etc.
Complete paperwork and physical assessments at baseline, and quarterly thereafter.
Develop effective and safe programming for Participants in the gym, in their homes, and/or in the community.
Manage a caseload of 18-25 participants.
Communicate with case managers, doctors, community organizations, etc.
Requirements
Bachelor's degree in psychology, social work, counseling, health sciences, or education/counseling from an accredited college or university.
Personal Trainer Certification is preferred at start or must be obtained within 3 months of start.
Mental Health and/or Personal Trainer experience.
Requires a valid driver's license and auto insurance with liability coverage and reliable car as you will transport participants.
Bilingual candidates are encouraged to apply.
Education: A bachelor's degree in psychology, social work, counseling, health sciences, or education/counseling from an accredited college or university is required.
Qualifications: Candidate must possess a valid driver's license, an automobile with liability insurance, and a clean background and driving record check.

Basic Function Coordinate volunteer mentors and provide 1:1 peer support to mentee. Work closely with the supervisor to support recruitment, training, matching, and developing best practices that promote a positive and productive relationship with the mentee and mentors.
Essential Functions
* Obtain and maintain training as a Peer Mentor.
* Collaborate with United Spinal Association by attending and participating in offered meetings and training courses, sharing information with larger audience of consumers.
* Maintain ongoing contact with consumers, mentors/mentees of the program to determine the effectiveness and satisfaction of the consumers, mentors/mentees in the program.
* Recruit new mentors to the program, conduct presentations and other outreach activities routinely and as requested.
* Provide peer support by phone and in person to consumers or potential consumers as requested by internal or external referral sources.
* Participate and host Peer events in person, or virtually as assigned.
* Track participation of mentors/mentees in Peer events.
* In collaboration with supervisor, identify and or develop training material, and provide training and initial orientation for the mentors.
* Assist in the development, refinement and success of the Peer Support/Mentor Program.
* Network to increase community awareness of the Peer Support/Mentor Program.
* Work with Supervisor to plan and promote Peer events through out the year to engage with Peer Groups, Mentors and Mentees.
* Adhere to and promote the policies and procedures of the Peer Support/Mentor program, acting as a role model to the Mentors.
* Represent GSIL on relevant external and internal committees and task forces.
* Complete required documentation in appropriate system in a timely manner.
* Travel required - Must have reliable transportation to attend in-person events and meetings across the state.
Other Responsibilities:
* Participate in general GSIL events and activities.
* Support and uphold GSIL policies and procedures, workplace regulations and safety practices.
* Share news worthy information appropriately throughout the organization.
* Attend and participate in periodic training as scheduled by GSIL.
* Obtain resources and information on disability related issues to present to group members.
* Conduct other duties as assigned that will increase options for people with disabilities to live independently in their communities.
Required Education and Experience
* Lived experience with a disability.
Preferred education and experience
* Personal experience and working knowledge of Consumer Directed services.
* Experience as a mentor.
* Strong oral and written communication skills.
* Experience with group management, outreach, advocacy, and small group presentations.
* Computer and data entry skills.
* Physical/Mental Demands
While performing the duties of this position, the individual is regularly required to sit, use a computer, a telephone, and attend regularly occurring peer group meetings. Employee may occasionally need to exert up to 5 pounds of force to lift and carry presentation materials. Judgment and creativity are required to make decisions.
