Assists the person(s) receiving services in identifying and communicating his or her requests and needs for services and supports. Implements services and supports to address the person's goals and desired outcomes as identified through each person's Person Centered Plan. Relies on knowledge, experience, and judgment to complete tasks with supervision and direction from the Residential Team Leader.

Hours: 2nd shift including weekends
· Pair new employees with them to shadow (the new employee would shadow on 2nd shift during the DSM's shift)
