Our Behavior Specialist will be successful by completing assignments, developing strategies, writing personalized Behavior Support Plans for individuals with basic to complex behavior, under appropriate clinical supervision.
Other responsibilities include:
Responsible for efficient and appropriate development, facilitation, documentation and revision of Behavior Support Plans, according to the person's Individual Support Plan and per Merakey policy
Completes observations in various settings and conducts comprehensive Functional Behavioral Assessments (FBA) of presenting issues from aggression, self-injurious behavior, adverse mental health symptoms, law offending behavior- sexual or other identified presenting issues
Collaborate with the person, their family and team in the development and maintenance of a behavior support plan.
Trains and mentors, the people support team on how to implement plans.
Completes documentation including but not limited to data collection, progress reporting, development and revision of the Behavior Support Plan (BSP)
Provides timely documentation, per policy, including but not limited to data collection, progress reporting, development and revision of the BSP.

In partnership with FEC staff, families will drive the planning, development, and sustainable co‑ownership of the centers. Families who participate in the FEC programming will be encouraged to share their expertise by volunteering at the center and given meaningful opportunities to engage with neighbors and peers. The goal of the FEC is to strengthen families' protective factors and build community connections, capacity, and resilience.
Position Summary
The Family Advocate is responsible for contributing to the day‑to‑day operation of the FEC including building connections with other staff, residents, and community partners; co‑designing and delivering relevant programming and activities (offerings); providing advocacy and making referrals for participants; and creating a warm and inviting gathering place for families and individuals using the protective factors model to support community resilience.

Position SummaryThe MOUD casemanager will ensure clients have knowledge and access to the full range of Glide Foundation resources to achieve and maintain stabilization. Through intervention and counseling, client populations will be able to access HEAT services and have positive health outcomes. In this role, you will work with Glide participants who have experience with opioid use disorder to connect them to opiate replacement therapies such as methadone, suboxone, naltrexone and other medications that help participants transition from dependence on the use of opiates to self-sufficiency. The MOUD casemanager will also create and develop a caseload of 20 to 30 participants with whom they work to maintain treatment adherence and stabilization; in addition to successful medication adherence, the MOUD casemanager will emphasize assistance with housing resources, identification, mental health access and any needs identified by the client Essential Duties and Responsibilities:
Outreach, educate, and link Glide HEAT clients to methadone, suboxone, naltrexone, Brixadi and other medications that target opiate use disorder at GLIDE, at community events, and mobile outreach efforts.
Build positive clinical rapport to maintain a productive and active caseload to promote medication adherence, increase stability and wellness, and apply harm reduction practices. Link and navigate MOUD clients to primary health care, appropriate mental health services, and assist clients in accessing housing and the necessary documentation to support stabilization and positive outcomes.
Outreach, education and link client caseload to full range of Glide services as well as the relevant external community resources to achieve stability and self-sufficiency.
Plan, conduct outreach for, and facilitate MOUD, treatment and other harm reduction groups on a regular basis.
Document and maintain complete files on clients including updated intakes, assessments, casemanagement notes, group attendance logs, referrals and other required data captures in a timely and accurate manner in accordance with program standards, organization policy, and applicable local, state, and federal laws for MOUD services. Submit monthly activity and data reports that capture contract deliverables in required database systems.
Adhere to standard professional boundaries, understand and follow basic ethics and values of casemanagement regulatory practices, and understand and adhere to HIPAA protocols.
Develop and prepare outreach and educational materials targeted to engage the broader community in HEAT MOUD programs. Assist the leadership team in coordinating special events that promote HEAT MOUD efforts as well as the full array of HEAT and Glide wide programming.

Minimum Qualifications:
Bachelor's degree in social work, public health or related field and/or 2+ years providing MOUD/MAT navigation and harm reduction focused casemanagement services to high-risk populations, including knowledge providing prescriptions for suboxone, naltrexone and other medications (and methadone dispensaries) used to support treatment adherence for clients who access HEAT MOUD services.
Must have experience and training in overdose reversals, de-escalation techniques, motivational interviewing. And experience providing street-based outreach services in urban settings and homeless encampments.

Ability to establish and maintain effective collaborative relationships across Glide wide programs as well as with city agencies across San Francisco and the wider Bay Area.

The CaseManager I provides casemanagement/substance abuse counseling/advocacy services to an assigned caseload and participates as a member of an interdisciplinary treatment team to facilitate the client recovery/rehabilitation process. These goals are accomplished by providing individual and group counseling, making appropriate interventions as needed and contributing to the healthy integrity of the therapeutic/program environment.
What You Will Do
Depending on site will perform some or all of the following:
Provides casemanagement/advocacy services to an assigned caseload.
Participates in monitoring therapeutic environment to ensure that behavioral guidelines are followed, a therapeutic milieu is maintained and interpersonal relationships remain positive.
Provides individual counseling and facilitation of caseload group meetings and other assigned groups.
Provides psycho-education and counseling germane to chemical dependency and relapse prevention.
Administers appropriate behavioral interventions as needed.
Participates in assessment screenings to determine client needs/preferences; formulates and implements treatment, transition and discharge/continuing care planning collaboratively with individual clients.
Facilitates re-socialization process of clients by serving as a role model. Provides role modeling regarding ethical and professional conduct.
Assumes staff-on-duty assignments as requested, including observation of urine specimen collection.
Assumes responsibility for medication management including the direct observation of clients during medication pass for adherence to their prescribed medication regime.
Conducts facility/safety runs and room checks as assigned to visually assess clients for use of alcohol/ drugs or environment of care for unsafe conditions.
Ensures maintenance of accurate, complete, timely and high-quality client records and reporting of client information that comply with external regulatory standards and agency policy and procedure.
Complies with 42 CFR confidentiality and HIPAA privacy and security regulations.
Adheres to all responsibilities and duties of a New York State mandated reporter.
Works cooperatively with other staff as a member of the interdisciplinary team.
Functions as a liaison/advocate to clients' families and/or other agencies as needed.
Provides outreach/education/prevention services to the local community, schools, groups, and agencies as necessary and as approved by management staff.
Provides crisis intervention/management, generating referrals to agency or external mental health providers or other service providers, as indicated.
Performs other duties as requested.
High School Diploma or equivalent.
Completion of 350 CASAC clock hours completed preferred.
One year of prior CaseManager experience in a Human Services setting.
Strong individual and group counseling skills.
Working knowledge of substance abuse treatment modalities and client self-help/support modalities.
Strong writing skills, competencies in writing goal based/person centered treatment plans and progress notes.
Ability to participate in CPR and First Aid training.
Good organizational skills
Able to read, write, speak and understand English
At some sites, bilingual in Spanish is preferred
Strong interpersonal skills to interact effectively with clients, staff and outside contacts
Proficiency with Microsoft office (Word, Excel, Power Point, Outlook)
Willing to travel in the community.

ABOUT THE ROLE:
The CaseManager works out of the Lighthouse Transitional Living Program in West Los Angeles. Lighthouse TLP is a housing first low-barrier, high tolerance program providing housing and comprehensive services to youth ages 18-24 who are experiencing housing instability. The CaseManager is responsible for oversight and safety of the property and residents. This includes, but is not limited to, monitoring and maintaining cleanliness of the site and living spaces, carrying out day-to-day safety protocols, responding and de-escalating crises, conflict management among residents, and promoting a holistic approach to the wellness of our residents. This position requires some driving and lifting.
WHAT YOU'LL DO:
Daily site and overall property inspections to maintain curbside appeal and resident safety.
Maintain inventory and storage of program supplies, including food, program supplies, cleaning supplies, etc.
Assist with program intakes and exits, including preparing intake packets, documentation of requirements, and conducting exit service evaluations.
Coordinate and assist with food and meal support in the program, including ordering and receiving Food Pantry donations and picking up food as needed.
Report all issues concerning resident health, safety, maintenance or other areas of concern in a timely manner and complete required documentation.
Interact and build relationships with residents, demonstrating the ability to support resident success, solve problems and enforce house rules.
Facilitate and/or coordinate activities, events, and workshops for residents.
Provide resources and other information to support residents in accessing needed services.
Collaborate with community partners and volunteers to coordinate life skills workshops and programming to the site.
Assist in the preparation of required program reports.
Drive to transport residents to services and community activities.
Other duties as assigned to support the mission of the organization.
Requirements
WHAT YOU'LL NEED:
Bachelor's Degree in Social Work, Psychology, Sociology or related field is preferred. Related work or lived experience of housing insecurity or homelessness will be strongly considered in place of or in addition to a Bachelor's degree.
1-2 years of experience providing direct services to high-risk transitional age youth (TAY) is required.
1-2 years of experience accessing social services (homeless services, DCFS, probations, DPSS, etc.) is a plus.
Best practice models including: Housing First, Positive Youth Development, Authentic Youth Engagement, Motivational Interviewing, Harm Reduction, Trauma-Informed Care, Critical Time Intervention
A self-starter who is prepared to drive innovative solutions within the organization
