ESSENTIAL DUTIES AND RESPONSIBILITIES:
Follow established standards of nursing practice and implement facility policies and procedures.
Interpret existing policies and procedures to nursing assistants, restorative nursing assistants, residents, families and physicians.
Provide direct resident care and give instruction to nursing assistants related to resident care.
Obtain report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary.
Provide report to nurse coming on duty including sufficient information for follow-up action if necessary.
Conduct resident rounds, reporting problems to unit manager/supervisor and initiating corrective action.
Count narcotics, at the beginning and end of each shift.
Identify safety hazards and initiate corrective action immediately.
Identify resident problems and emergency situations, and initiate appropriate action.
Participate in facility education programs, staff meetings, in-service education and committee meetings as assigned.
Assume responsibility for compliance with federal, state and local regulations within the assigned unit.
Counsel nursing assistants and recommend disciplinary action, when necessary, to the unit manager/supervisor.
Participate in the employee evaluation process.
Administer and document medication treatments per physician's orders.
Order medications and replace emergency box medications according to pharmacy protocol.
Perform and document a comprehensive assessment of each resident.
Develop an accurate comprehensive care plan based on each resident's comprehensive assessment.
Review and revise resident care plans as necessary.
Participate in resident care conferences as assigned.
Document resident care provided and resident's response/lack of response to care provided.
Report changes in resident's conditions to unit manager/supervisor, physician, and responsible party as necessary.
Follow facility admission procedures and communicate resident admission to other departments and physicians.
Follow facility discharge/transfer procedures and coordinate with other departments and physicians.
Listen to resident, family or physician concerns and initiate corrective action.
Prepare nursing assistants' assignments with unit manager and explain pertinent resident care.
Update nursing assistant assignment sheets.
Document on residents' progress notes as necessary and on monthly summaries as assigned.
Practice infection control according to facility procedures.
Participate in survey by authorized government agencies.
Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Compliance in the performance of their duties.
Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
Follows all local, state and federal regulations as they pertain to their position.
Adheres to the Organization's Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
Perform other duties as assigned by Unit Manager or Shift Supervisor, as needed.

The Training and Special Projects Nurse is responsible for creating and providing training and technical assistance with special projects for persons diagnosed with intellectual/developmental disabilities (I/DD) and/or Autism Spectrum Disorder (ASD). The Training and Special Projects Nurse will work with agency nurses, health care coordinators, supports coordinators, Office of Developmental Programs (ODP), and the community health care systems. The Training and Special Projects Nurse works within the interdisciplinary team to ensure optimum quality care. Considering the financial availability, PCHC will pay for attendance to the National Developmental Disabilities Nurses Association (DDNA) conference and certification based on PCHC guidelines.
Duties and Responsibilities:
Act as a resource regarding medical/dental care, treatment, follow up and preventative care.
Obtain qualification in developmental disabilities certificate (DDC) and maintain DDC certification status once obtained. Please see PCHC guidelines dated about one-time reimbursement upon certification.
Establish alliances with other nurses and health care coordinators who are working in the community with individuals who have been diagnosed with I/DD and/or ASD.
Remain active in updating skills and use best practice in working with people with I/DD and/or Autism.
Participate in meetings, committees, attend training webinars/seminars as required.
Participate in all Nurse Network Meetings and other health related meetings, at the local, regional and state level.
Work in conjunction with other PCHC nurses and all other PCHC staff members.
Perform other duties as determined necessary by the PCHC Nursing Manager and/or the Director of PCHC.
Assist with the Office of Developmental Program's special projects as needed.
Training:
Prepare and provide training in physical and behavioral health care issues for individuals diagnosed with I/DD and/or ASD, staff members who support them, family members, supports coordinators, and any other supportive personnel.
Identify staff training needs as they relate to health and safety. Provide training on physical and behavioral health care issues utilizing existing training resources and developing training modules, as necessary.
Prepare and provide training for individuals diagnosed with I/DD and/or ASD in topics related to health promotion and disease prevention.
Technical Assistance:
Be a resource to individuals diagnosed with I/DD and/or ASD, agencies, counties, and the state/regional Office of Developmental Programs/Pennsylvania Department of Public Welfare for issues involving health care.
Contribute articles and resources for PCHC website, Health Care Alert newsletter, Health Bytes, social media, etc.
Must have a valid driver's license when operating a personal vehicle during work hours and for work-related activities.
Skills Required:
Ability to work independently.
Excellent communication skills, both written and oral as well as organizational skills.
Ability to prepare and provide training in health-related matters.
Ability to establish and maintain positive working relationships with co-workers and other disciplines outside PCHC.
Nursing skills required for LPN level license
Education and Experience:

Experience in providing training on health care issues.
Physical Demands:
Must have the ability to sit for a minimum of one hour at a time, and to use a computer. Must be able to travel via car or public transportation for off-site home visits and meetings.
