This role focuses on continuity of care through in-home visits, patient assessments, clinical and social screenings, patient education, and coordination with healthcare and community partners. The MIH Paramedic functions independently while collaborating closely with physicians, nurses, behavioral health providers, and community agencies to improve patient outcomes and reduce unnecessary emergency department utilization.
