Job Summary:
The Emergency Room (ER) Registered Nurse (RN) position is responsible for delivering exceptional nursing care to patients by:
Utilizing strong organizational and leadership skills to assess patient status
Determining the priority of the patient's problems and needs

Education:

Certifications:

Job Responsibilities:
Coordinates the primary care rooming process, relevant medical procedures, adult and pediatric patient care including, immunizations, venipuncture, point of care testing, and performs retinal scan images. Follows scheduling decision tree, protocols and policies for clinical procedures and appropriate use of medical equipment. Provides accurate and complete documentation of all facets of care including clinical calls, patient rooming questions, completion of procedures, order entry, prescriptions and patient pharmacy, and workflows. Addresses messages in a timely manner and escalates issues as appropriate. Utilizes and monitors MyChart messaging to support patient communication
Participates as a part of the patient centered medical home team during all patient visits by reviewing the patient chart of clinical gaps in care. Assist with outreach campaigns and tactics to close gaps in care. Supports and completes pre-visit planning and participates in daily huddles with the physician and care team. Embraces the philosophy of wellness and prevention by reminding patients of all screenings and immunizations due by the end of the year. Informs physician of any potential barrier identified by the patient.
Understands population health and value-based contracts. Utilizes key quality and unitization metrics of value-based programs for both wellness and chronic disease management. Demonstrates abilities in the Primary Care quality program including all protocols of well and chronic disease states. Identifies patients "at risk" for change in condition and increased utilization. Attends required population health training and education such as Lunch and Learns and other opportunities
Participates in the longitudinal care continuum of patients through completing post ED/post inpatient discharge outreach on identified risk patient group. Updates care team thorough documentation and works collaboratively with Complex Care RN, Social Worker, CHW, and Population Health Pharmacist. Provides basic community resources to patients with social determinates in health. Supports and provides education and patient coaching of both wellness and chronic disease management (e.g., Diabetes Education, Colon Cancer Screening). Supports facilitating follow-up for post-hospital care, chronic disease management, or specialty referral.

The ED Supervisor is responsible for administration and operational activities of the Sylvania Medical Center Emergency Department and assigned areas. In collaboration with interdisciplinary team members, assures safe, efficient and cost-effective care to patients within the assigned areas. The ED Supervisor provides support for all campus operations and engages appropriate department heads when indicated. The ED supervisor serves as a clinical resource to direct care staff. Assumes responsibility and authority as delegated by the Director of Nursing/VP of Nursing/CNO. Assists in the provision of care to patients of all age groups.

Accurately document any significant situations and forwards to appropriate department head; contacts on-call Administrator for timely/sensitive issues or handling of questionable situations.
Provides input to clinical manager regarding staff capabilities and growth areas and provides input as requested for performance appraisals on a timely basis.
Assists new patient care staff in becoming effective members of the health care team and provides input to their respective clinical manager regarding level of functioning.
Responsible for maintaining knowledge of compliance with local, state, and federal health and safety regulations, as well as other accrediting bodies.
Adheres to the standards and policies of the Corporate Responsibility Program, including the duty to comply with applicable laws and regulations, and reporting to designated Manager (or employer hotline) any suspected unethical, fraudulent, or unlawful acts or practices.

The RN Clinical Float Pool assesses, analyzes, plans, implements and evaluates patient care following established patient care standards for the organization including those for patient assessment and transfer. The primary function of this job, based on hiring tier, is to be available and flexible to float to various departments, and sites throughout the markets within the organization to provide patient care, which could include but not limited to scheduling day of staffing, more than one assignment location within a single shift, travel assignments, or seasonal staffing patterns. Participates as a member of the health care team in cooperation with and under the direction of a clinical manager or designee as defined by the relevant Standards and Scope of Practice for the Registered Nurse.

Job Responsibilities:
Responds to the hospital in a timely manner according to TriHealth guidelines.
Determines the need for advocacy agencies and law inforcement presence if not already available. Collaborates with these agencies to provide quality service to all clients.
Collaborates with the ED staff nurses and physicians regarding the care involving evidence collection for victims of abuse.
Utilizes assessment skills to determine patient needs based on patient information and history. Takes into account the individuals cognitive, physical, cultural, emotional/psychosocial, and spiritual needs when communicating and collecting evidence on patients. Function effectively within an environment of crisis involving sensitive interpersonal violence issues, often in conjuction with physical, psychological trauma and substance use.
Performs technical skills according to policy and procedure and accepted standards within their area of practice. Safely administers medications/treatments as indicated. Uses all equipment in a safe, appropriate manner.
Documents pertinent data on CARES records and provides the documents to the CARES Coordinator. Maintains chain of custody and ensures record is secured prior to finishing case. Will consult with the CARES Coordinator regarding cases requiring court appearances.

Identifies areas for professional and self-improvement through self-assessment and peer review, as well as the continuing education of team members continuing education, formal education, professional organization membership, and/or certification.
