Scheduled Weekly Hours: 40 Work Shift: Days (United States of America) Full Time Days Primary Function Responsible for delivery of high quality services and the functioning of the multidisciplinary team through the provision of specific treatment modalities. Under the direction and supervision of the Manager, assumes responsibility for the case management of those clients assigned by the manager. This includes assessments, treatment planning, clinical treatment, group therapy, intensive outpatient treatment, case review, discharge planning and crisis intervention and other duties as assigned. Essential Functions Case management of clients to included assessment, treatment planning, case review, discharge planning and crisis intervention Direct services to include individual, group, urinalysis screening, and continuing care for chemical dependent and nicotine dependent clients Ongoing follow-up and data collection for clients discharged from the program Provide accurate and timely documentation of services Utilization review of assigned clients Provide education to community and other team members regarding substance abuse, chemical dependency and tobacco dependency Maintain patient safety standards, departmental policies and procedures, and performance improvement standards Other duties as assigned Each employee utilized in positions which are responsible for direct care or supervision of children or adolescents shall be at least eighteen years of age.
