Social Worker - St. Charles Hospital
Job Summary:
The Social Worker provides direct patient care under the supervision of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health (BSMH). May provide clinical and administrative supervision.
Essential Functions:
* Provides individual and family psychotherapy, group facilitation, and participates in family meetings and treatment team meetings.
* Handles discharge planning, post-discharge calls, shares community resource knowledge, and promotes collaboration.
* Administers Psychosocial assessments, behavioral health (BH) screenings, and ensures completion of Preadmission Screening and Resident Review (PASRR).
* Responsible for crisis assessment and referral services.
* Participates in continued education and in-service training.

The clinical social worker can assist with emotional support and counseling, family issues/conflicts, safety concerns, discharge coordination, and access to community resources. About Inpatient Psychiatry, located at the College Hill Campus
The Division of Child and Adolescent Psychiatry at Cincinnati Children's acute crisis units focus on stabilizing patients, most of whom stay an average of three to five days. In total, our hospital has 92 inpatient beds available to serve children and adolescents who are 17 years old and younger, who are experiencing a mental health crisis. The units are explained below:
* Adolescent Unit 4.200 (ages 12-17): Patients with a mental/behavioral disorder
* Youth Unit 5.400 (ages 8-17) Patients with both a mental/behavioral diagnosis and developmental disability
* Young Child Unit 5.200 (ages 2-8) Patients who have a mental/behavioral health diagnosis
* Youth with chronic medical conditions in addition to mental health diagnosis
Youth admitted for inpatient treatment can participate in various therapeutic and medical treatment programs depending on their personal needs. Their stay can include group therapy, music therapy, occupational therapy, speech pathology, and chemical dependency education.

Job Summary:
The Advanced Social Worker provides direct patient care under the guidance of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health. May provide clinical and administrative supervision.
Essential Functions:
Provides Individual, Family, & Group Psychotherapy in Ambulatory and Hospital Outpatient settings.
Responsible for psychosocial assessments, Preadmission Screening and Resident Review (PASRR) completion, discharge planning, and post-discharge calls.
Knowledge of involuntary processes such as guardianships, advanced directives, and power of attorney (POA).
Administers patient health questionnaire (PHQ-9), Columbia-Suicide Rating Scale (C-SSRS) and Social Determinants of Health screenings.
Offers community resource knowledge and collaboration.
Participates in continued education and in-service training.

The Social Worker assists patients and families to resolve all aspects which may prohibit or limit the patient from receiving maximum benefit from medical care. Evaluates social-emotional functioning of patient and families, providing counseling when necessary and facilitates the discharge plan. Works cooperatively with other hospital departments, physicians and outside agencies. Provides assistance to neonate, pediatric, adult and geriatric patients.
KNOWLEDGE
1. Work requires a Bachelor's degree in Social Work with licensure from the State of Ohio as a Licensed Social Worker.
2. A relatively high level of analytical ability is required to assess patient needs, develop and manage discharge plan and to effectively utilize community and governmental resources.
3. A significant level of interpersonal skills is required to counsel patients and families concerning sensitive emotional and social issues.
4. Six to twelve months previous experience is required to acquire diagnostic interviewing and counseling skills.
WORKING CONDITIONS
1. Work is generally performed in an office or normal patient care environment where there is some exposures to communicable diseases.
2. On call responsibilities as assigned.
RESPONSIBILITIES
1. Conducts interviews with patients and families to obtain social work assessment and evaluation relevant to medical problems and discharge plan. Collaborates with entire health care team to ensure maximum benefits of medical care.
2. Completes social history and documents, when necessary on patients.
3. Evaluates data and refers patients to appropriate community resources, and if necessary, facilitates placement. Coordinates the exchange of medical and social information necessary to ensure continuity of care.
4. Provides counseling to patients, as needed, to assist them in decision making and acceptance of illness and related problem, e.g. Advance Directives, adjusting to disfiguring illness, permanent disabilities.
5. Maintains ongoing and develops new contacts with outside agencies in order to be used as a referral source.
6. Coordinates financial referrals and counsels patients and their families informing them of various assistance programs and community resources.
7. Participates as a team member in multi-disciplinary conferences.
8. Maintains JCAHO and Department standards. Prepares and submits monthly statistics as directed.
9. Provides service to all age groups. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patient served on assigned unit.
Demonstrates knowledge of the principles of growth and development over the life span by completing continuing education program.
Interprets information need to identify each patient's requirements relative to his/her age-specific need:
a. Neonate
b. Pediatric
c. Adolescent
d. Adult
e. Geriatric
