Responsibilities
Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
In this role, you will take on the responsibility for managing the Hospice and Palliative Care SocialWorkers by providing leadership and mentoring guidance to the staff on a day-to-day basis to ensure the provision of Hospice and Palliative Care services are consistent with the philosophy, goals, and policies of the program and MJHS. You will coordinate theprovision of Hospice and Palliative Care psychosocial services and ensure adequate staffing and appropriatepatient assignments. You will maintain the integration of psychosocial services as part of the Hospice and PalliativeCare program's interdisciplinary team approach and you will function as a member of the Hospice and Palliative Caremanagement team and attends management meetings as directed.

Qualifications

Maimonides is seeking a Licensed SocialWorker to deliver brief, evidence‑based caregiver support services as part of the Caring for Caregivers (C4C) initiative for adults who are 65 years old and older and their identified care partners. This per‑diem role focuses on caregiver identification, outreach and consent, needs and quality tracking screening, goal‑aligned (“What Matters”) action planning, tailored resource linkage with teach‑back, EMR documentation, care‑team communication, and prompt time‑limited follow‑ups.
In this role, you will:
Identify and engage caregivers of adults who are 65 and older through admission/rounds flags and referrals; obtain consent and explain program scope.
Conduct brief structured screens using evidence-based tools (e.g., PHQ‑2/9, GAD‑2/7, brief caregiver‑burden check, health‑literacy and SDOH prompts).
Facilitate a concise “What Matters” conversation to elicit the caregiver's priority needs and preferences; co‑create an action plan focused on1-3 priority areas.
Provide tailored resource packets (education, benefits, respite, mental health, community‑based supports); use teach‑back to ensure understanding and feasibility.
Coordinate with the care team (e.g., primary team, case management, nursing, behavioral health) to align plans and communicate a concise summary note.
Track outputs and outcomes (encounters, screening completion, referrals provided, closed‑loop hand‑offs, caregiver‑reported stress/depressive symptoms when available).
We require:
MSW from an accredited School of Social Work; NYS licensure or permit as applicable.
Minimum 1 year in medical, geriatric, primary care, or community health social work experience with brief screening, goal‑setting, and resource navigation preferred.
Motivational interviewing or equivalent brief counseling; familiarity with evidence-based screening tools (eg, PHQ/GAD screening); knowledge of accessible and relevant caregiver resources (respite, benefits, mental health); EMR proficiency; strong communication and teamwork.
Diligent and effective follow-up; problem‑solving and sound judgment.
Hourly Pay: $45.81 - $64.77 (based on experience). Monday-Friday.

Summary of position:

MAJOR FUNCTIONS:

MAJOR FUNCTIONS:
The SocialWorker position requires a wide range of clinical skills including interviewing, individual and group counseling and a thorough knowledge of the substance abuse, mental health and human service system resources. The SocialWorker is responsible for the assessment and treatment of clients that have co-occurring mental health issues requiring a specialized group curriculum.
Additionally, the SocialWorker is responsible for the role of Health Coordinator to assure the provision of education and/or referral for communicable diseases. The SocialWorker functions as part of the Multidisciplinary team, and is responsible for the review of treatment plans as a Qualified Health Professional (other then CASAC). The SocialWorker is on the Quality Assurance team and is responsible for the review of client records to meet regulatory, billing and utilization review compliance.
SPECIFIC DUTIES & RESPONSIBILITIES:
Complete clinical assessment and re-assessments at regular intervals to measure progress towards addressing life areas deficiencies.
Provide individual counseling to address outstanding family, medical, psychiatric, and housing concerns.
In collaboration with each consumer, formulates a treatment plan, monitor achievement of objectives, review and update treatment plans in accordance with regulatory requirements and as clinically indicated.
Conduct evidence-based group counseling services related to a wide range of clinical and community issues.
Maintain up-to-date resident charts including assessments, treatment plans, case notes and documentation of collateral contacts.
Provide crisis intervention services.
Assist in the development and implementation of consumer discharge plans.
Work closely with utilization review staff to ensure completion and accuracy of all necessary health insurance authorization documentation.
Establish community and agency affiliations which address consumer needs and strengthen the delivery of services.

ESSENTIAL JOB FUNCTIONS:
Complies with the Code of Ethics of the National Association of Social Work.
Ensures the completion of the MDS 3.0 for social work per facility policy.
Ensures residents are provided with the least restrictive environment and medication regime to ensure a quality, resident centered experience at ECH.
Provides supportive contact to residents and family members as they acclimate to life at ECH; provides them with the opportunity to express feelings associated with long term care life.
Follows established departmental policies, procedures and standards related to including admissions assessments, supplemental admission documentation, routine progress notes, social work assessments, quarterly summaries, discharge notes, post discharge care plans, Mini Mental State Examination (MMSE), advanced directives (MOLST) and care plans within the established time protocols.
Provides ongoing assessments of residents functional, emotional and behavioral status; communicating changes to family/designated responsible party. Collaborates with interdisciplinary team to ensure residents are provided with quality care.
In collaboration with the hospice team and to ensure quality end of life care, facilitates resident hospice referrals/evaluation, and develops comfort care plan.
Coordinates discharge care plan meetings with resident, family, housing (if applicable, per resident transfer protocol), and interdisciplinary care team to ensure a safe and appropriate discharge plan for residents.
Coordinates appropriate discharge plans with community agencies to ensure safe discharge.
Participates in the interdisciplinary resident care conferences as scheduled; invites resident and family members to meetings per policy.
Provides resident and/or designated representative with transfer policy and appeal process when given notice and transfer is involuntary.
Involvement in the Ethics Committee as appropriate.
Coordinates room changes in accordance with the resident's Bill of Rights and departmental procedures.
In collaboration with the ECH Admissions Coordinator, ensures a smooth admission for each resident and family members/designated responsible person. Discusses ongoing bed availability and room change issues per protocol.
Follows guidelines of HIPAA to ensure confidentiality of each resident.
Assists in the development and implementation of Person Directed Care.
Attends facility in-services and staff meetings.
