The Patient Benefits Specialist works within the Local Authority and assists those in all service coordination programs including GR/SC, TxHmL, CFC, and HCS with patient benefit information and education on benefits access. II) Essential Functions A) Develops and manages eligibility screening tools and processes for various benefits, including SSI/Medicaid, Social Security/Medicare, TDHS Family Medicaid Programs, CHIP, MDCP, CLASS, CBA, HCS, QMB, SLMB, MAO, QDWI, 1929-B, SNAP, and The Free Medicine Foundation. B) Completes secondary screening of referred patients according to State compliance requirements and assesses application status. C) Collaborates with support staff/leadership to qualify referrals for application assistance and performs other assigned duties as determined. D) Gathers information, documentation, and medical records, and assists in completing forms for application validation. E) Manages payee changes, SSA "in-office" changes, and ensures the inclusion of necessary records with applications. F) Assists patients assigned via referral in completing application forms professionally and ethically. G) Monitors service delivery for timeliness and efficiency according to agency guidelines. H) Processes internal referrals daily, prioritizing program eligibility referrals, and maintains referral status documentation.
